[The hyperfunctional thyroid nodule].
Authors report their experience of 22 years regarding 284 patients with hyperfunctional thyroidal nodules (279 operated), representing 12.4% of the 2,289 cases of thyroidal (875 hyperthyroid) diseases treated during this period. The diagnosis was established on the basis of the confrontation of the anamnestic symptomatology and of that found at the clinical examination--which confirmed the pure character of thyrotoxicosis by showing the presence of a thyroidal nodule--with the study of the global glandular function and the scintigram, completed with dynamic tests. Among the typical clinical and scintigraphic, usually severe, forms (with cardiothyreosis) may be distinguished asymptomatic or atypical forms (with dissociated or intermittent manifestations). The treatment of the toxic thyroidal adenoma is surgical, except some contraindications of a general order. The surgical treatment is aimed at achieving the permanent cure of the disease, without sequelae or functional and/or morphological relapses. The subtotal lobectomy (210 cases) is preferred as an exeresis procedure to the enucleoresection (45 cases). In most operated cases (93%) the immediate clinical and biological results were favourable.

[Adenocarcinoma in Barrett esophagus: its recurrence and long-term survival].
Adenocarcinoma arising in ectopic gastric mucosa has been reported more than 40 years ago. This report reviews our experience with Barrett's adenocarcinoma over 8 years. The definition remains subject to controversy. More than 50% of patients in our study had unfavorable course of disease. The hope for cure lies almost in early detection of tumors at which point a truly curative resection could be done. There is no difference in terms of survival comparing to the adenocarcinoma arising on normal esophagus.
[Klatskin's tumor].
During the period 1989-1998, 27 patients with primary proximal bile duct cancer were treated (17 females and 10 males with a mean age of 61 years). The main presenting symptoms were: jaundice (96.2%), itching (92.5%), weight loss (74%) and atypical pain (33.3%). All the patients underwent ultrasound (US) examination, 7 patients endoscopic retrograde cholangiopancreatography (ERCP) and 4 patients computed tomography (CT) examination. US examination revealed the tumor in 51% cases; most frequently a dilatation of the intrahepatic bile ducts was diagnosed with all methods. 8 patients underwent resection of the tumor and one a biliary-enteric anastomosis, in 15 cases a stent was inserted. In 3 cases the extension of the malignancy did not allowed any procedure. Three patients died during immediate postoperative period (mortality 11.2%). The mean survival was 13 months after stenting and 22 months after resection of the tumor. The authors recommended an aggressive surgical therapy for Klatskin tumors.

[Rare location of a hydatid cyst- in the upper mediastinum migrating into the spinal channel].
The authors report a rare and difficult case of hydatic cyst. The case was rare due to the location of the hydatic disease in the upper mediastinum and because of the onset of the disease with a complication: a spinal cord compression syndrome produced by the migration of the hydatic cyst into the spinal channel. The case was difficult because of the diagnostic problems--especially to establish the topography of the lesion and surgical management.

[Prevention and therapy of postoperative thyrotoxic crisis].
Out of a group of 361 patients with various anatomo-clinical forms of hyperthyroidia--of which 328 had underwent surgery--hospitalized in the Surgical Clinic from Jassy between 1965 and 1974, a total of 17 observations are analised, from the clinical and the therapeutical viewpoint, that showed postoperative thyreotoxicosis crisis. The fact is stressed that the prophylaxis of the crisis is dominated by the principle that no case of hyperthyroidia should be operated on before it has been perfectly balanced from the endocrinological viewpoint. The complex treatment of the cases that are presented included a symptomatic component to which were associated oxygen therapy, tonics, vitamins, hepatotrop preparations, eventually antibiotics. The results obtained--complete recovery in all the cases--can be compared with the best from the literature.
[Colonic lipoma].
The authors present 6 cases of colonic lipoma, 2 in females and 1 in male, aged between 61-62 years. The benign lipomatous tumour was in all cases unique, located in the right segment of the colon, ileocecal valve, and on the sigmoid once respectively. The right colonic lipomas had clinical signs of intestinal invagination while the sigmoidian one were characterized by abdominal pain and bleeding. The surgical solution consisted in limited ileocecal resection, right hemicolectomy, and segmentary colectomy respectively.
[Unusual localizations of hydatid cysts].
In a total of 448 hydatic cysts that have been treated in the I-st Surgery Clinic of Jassy between 1947 and 1977 rare localizations were noted in 61 cases. Of these 47 were primary echinococcoses and 14 were secondary. The clinical and therapeutic aspects are discussed for each particular localization: the splenic localization was encountered in 16 cases, the peritoneal one in 10 cases, the diaphragmatic in 6 cases. There were also 2 renal localizations, 3 retro-peritoneal ones, 3 ovarian and salpyngean localizations, 3 thyroid, 7 endo-thoracic but extra-pulmonary, 5 muscular and 6 subcutaneous localizations. Radical surgery had immediate and late results that were quite satisfactory. No deaths or recidives were noted.
