[Comparison of clinical and histopathologic diagnosis for early stages of advanced nonseminomatous germ cell testicular tumors].

The authors present their observations concerning application of imaging methods in disease staging procedures in 69 patients with low stage nonseminomatous germ cell testicular tumors (NSGCTT). The results of CT, USG, and lymphangiography examinations of the retroperitoneal lymph nodes were compared with the results of pathological examination of these nodes, removed during retroperitoneal lymphadenectomy. 29 patients had clinical stage I (no node involvement) and 40 patients had stage II disease (node metastases). After lymphadenectomy, in 31% of the patients, diagnosed clinically as stage I, the preliminary diagnosis was found to be false negative, and in 13.5% of the patients, diagnosed clinically as stage II, the preliminary diagnosis was found to be false positive. We assessed the value of the imaging methods by calculating the accuracy, the sensitivity and the specificity of each of them. The results are, respectively: 81%, 76% and 57% for CT; 79%, 76% and 51% for USG; and 77%, 77%, and 69% for lymphangiography. The authors conclude that retroperitoneal lymphadenectomy allows establishing of the most precise diagnosis and appropriate treatment in patients with nonseminomatous germ cell testicular tumors.

[Comparison of computed tomography results and pathological findings after laparotomy with splenectomy in Hodgkin's disease].

In 69 patients (pts) with Hodgkin's disease (HD) in I-IIIA clinical stage results of abdominopelvic computed tomography (CT) were compared with pathological staging. Concordance (specificity) of CT scan with results of macroscopic evaluation of HD lesions in the abdomen during laparotomy with splenectomy was 83%. Accuracy CT results and microscopic findings was 80%. False positive results of CT were found in 11.6% of patients and false negative in 8.6%. Specificity and sensitivity of CT versus to pathological staging were 84% and 63% respectively. The results presented here confirm, that pathological staging after laparotomy with splenectomy in HD was some advantage in precise prediction of some patients to radiotherapy and planning of irradiated fields.

[Usefulness of computerized myelotomography in the diagnosis of discopathy of the cervical spine].

The authors compared the results of preoperative computer-assisted myelography (CAM) with surgical findings in 45 patients with suspected cervical disc disease. In 98% CAM gave full and proper diagnosis confirmed by surgery. Only in one patient, the preoperative diagnosis was extraspinal tumour, and surgery revealed prolapsed nucleus pulposus lying freely on the anterior wall of the dural sac. In authors's opinion, CAM is sufficient for planning of surgical treatment in cervical disc disease, and other radiological studies, myelography or nuclear magnetic resonance tomography are usually not necessary.

Cranioplasty of an extremely large cranial defect caused by transitional meningioma with a knitted polypropylen-polyester prothesis "Codubix".

The authors report a case of transitional cell meningioma of the convexity which destroyed a large portion of the calvarium and invaded subcutaneous tissue. The tumour was totally removed and a large cranial defect/430 cm2 in size/was filled with a polypropylenopolyester knitted prothesis "Codubix" with an excellent result. The problems of chronioplastic closure of such an unusually large skull defect and the advantages of the use of the material "Codubix" are discussed.

Evaluation of dementia in subcortical arteriosclerotic encephalopathy (Binswanger's disease).

Sixteen patients with a clinical diagnosis of subcortical arteriosclerotic encephalopathy (Binswanger's disease) were studied. Mild or moderate dementia was found in 14 cases, with characteristic disturbances of concentration, and psychic retardation with apathy. Neuropsychological studies revealed that signs of diffuse lesions in the frontal lobes dominated. Impairment of memory was significant in all cases. Although intellectual impairment in subcortical arteriosclerotic encephalopathy resembled subcortical dementia, some significant differences were found.

The value of noncontrast computed tomography (CT) in the diagnosis of herniated lumbar disc.

The authors compared the results of preoperative noncontrast CT-scan with surgical findings in 64 patients with lumbar disc prolapse. In 75% of cases CT gave full and proper diagnosis confirmed by surgery. In 12.5% the level of hernia was well established but its direction was different then found on surgery. In 3.1% of cases CT was false negative and false positive in 9.4%. The authors think, that if CT picture reveals the level of hernia which correlates with clinical symptomatology it is absolutely sufficient for planning surgery. In cases with divergence between CT picture and clinical findings, myelography should be performed for establishing a proper diagnosis.

Single vibrissal cortical column in the mouse labeled with 2-deoxyglucose.

Columnar labeling was found in the primary somatosensory cortex of mice after stimulation of a single mystacial vibrissa following 2-deoxyglucose injection. The cortical vibrissal column had a cylindrical shape, passing through all layers of the cortex and was centered upon the appropriate vibrisal barrel. Columnar labeling extended beyond this barrel onto parts of neighbouring barrels, particularly within the same row. The densest labeling was found in layer IV in the barrel hollow. Removal of the non-stimulated vibrissae resulted in a subsequent lowering of 2DG uptake in the barrelfield surrounding the activated column, but did not affect the dimension of the activated column.

Computed tomography, dynamic cerebral scintigraphy and angiography in the diagnosis of cerebral aneurysms and ensuing blood flow disorders. A correlative study.

Twenty-nine consecutive patients were examined 3 to 6 days after subarachnoid hemorrhage, using conventional computed tomography (CT), computed angiotomography (angio-CT), conventional four-vessel angiography, and, in 20 cases, dynamic cerebral scintigraphy (DCS). Angio-CT indicated aneurysms in 21 cases. There was also agreement between angio-CT and angiography as to the localization of the aneurysms. DCS demonstrated blood flow disturbances in 15 cases with or without aneurysms. As opposed to these findings, changes due to blood flow disturbance were observed only in 7 cases at CT. The results suggest that angio-CT is a valuable method for detecting cerebral aneurysms and may limit the use of conventional angiography. DCS is a more sensitive method than CT, but DCS changes do not permit recognition of the type and location of the pathologic changes.

Computed tomography (CT) is effective in the diagnosis of pain recurrency after surgical removal of herniated lumbar disc.

The authors analysed CT-scan with intravenous contrast infusion in 18 patients with recurrent pain after lumbar discectomy. CT showed in these patients the cause of recurrency with 100% accuracy, confirmed by reoperation. Contrast enhancement permitted differentiation of postoperative cicatrix from other causes of lumbar spinal canal stenosis. The authors point out that decision about reoperation should be based upon computed tomography examination.

[Selected clinical and pathogenetic problems of Binswanger's subcortical arteriosclerotic encephalopathy--analysis of 19 cases].

In 19 patients (14 males, 5 females) the clinical diagnosis of subcortical, arteriosclerotic encephalopathy (s.a.e.) was established on the basis of CT scanning, which showed in subcortical white matter low-attenuation areas and, often, focal ischaemic changes. The age of patients ranged from 46 to 85 years, the duration of illness from 6 months to 12 years. Four patients were normotensive (in 1 of these cases the clinical diagnosis was confirmed neuropathologically). In 2 hypertensive patients the acute development of neurological symptoms was related to considerable fall of systemic RR. On the basis of the CT picture s.a.e. may be diagnosed early, before pronounced neurological and intellectual deficits develop.

[Conservative treatment of subdural hematoma].

The authors report the results of conservative treatment of subdural hematomas in 8 cases. In 6 of them the treatment was begun 1 to 7 days after head trauma, the other two with no history of trauma had complained of intracranial disease for two to four weeks before admission to hospital. All patients were treated with dexamethasone and 6 of them with Lasix, and two with mannitol. In 7 cases a quick improvement was observed, one patient, after 7 days treatment without good result was operated on. Control CT-scan, performed 2 weeks to 6 months after conservative treatment revealed the resolution of previously detected subdural hematomas.

[A case of asymptomatic central pontine myelinolysis].

In 1959 year Adams, Victor and Mancella described central pontine myelinolysis (CPM) as a disease of fulminant course and fatal prognosis. In the typical cases progressive tetraparesis with bulbar signs results in severe crippledom or death. The disease was diagnosed before only by autopsy, as soon as the modern radiological technics i.e., CT and MRI were developed it is sometimes successfully diagnosed with patients alive. The authors present the case of 62-year old woman suffering from slight hemiparesis which made use to perform the CT examination. It showed central pontine lesion which was confirmed by MRI examination. The authors suggest that there are not only typical cases of CPM but also the asymptomatic ones as presented above.
