Potent quinoxaline-spaced phosphono alpha-amino acids of the AP-6 type as competitive NMDA antagonists: synthesis and biological evaluation.

A series of alpha-amino-3-(phosphonoalkyl)-2-quinoxalinepropanoic acids was synthesized and evaluated for NMDA receptor affinity using a [3H] CPP binding assay. Functional antagonism of the NMDA receptor complex was evaluated in vitro using a stimulated [3H]TCP binding assay and in vivo by employing an NMDA-induced seizure model. Some analogues also were evaluated in the [3H]-glycine binding assay. Several compounds of the AP-6 type show potent and selective NMDA antagonistic activity both in vitro and in vivo. In particular alpha-amino-7-chloro-3-(phosphonomethyl)-2-quinoxalinepropanoic acid (1) displayed an ED50 of 1.1 mg/kg ip in the NMDA lethality model. Noteworthy is alpha-amino-6,7-dichloro-3-(phosphonomethyl)-2-quinoxalinepropanoic++ + acid (3) with a unique dual activity, displaying in the NMDA receptor binding assay an IC50 of 3.4 nM and in the glycine binding assay an IC50 of 0.61 microM.

The behavior of the unfused lumbar curve following selective thoracic fusion for idiopathic scoliosis.

The behavior of the unfused lumbar curve was evaluated radiographically in 58 cases of adolescent idiopathic thoracic scoliosis that had a selective thoracic fusion only. The lumbar scoliosis echoed the correction obtained in the thoracic scoliosis both immediately postoperatively and at an average 2 1/2-year follow-up. In the sagittal plane, although correction was obtained in the thoracic hypokyphosis, there were no changes seen in the lumbar lordosis at final follow-up. Segmental instrumentation was more effective in correcting thoracic scoliosis and hypokyphosis, but did not alter the behavior of the lumbar curve. The caudal extent of the fusion relative to the stable vertebra did not measurably alter the operative results in either curve or plane.

Theory building in nurse-midwifery. The care process.

Identifying what it is about the way nurse-midwives care for women that might contribute to healthy outcomes for women and infants requires clear definition of that care process. This article describes our efforts to define the theoretical construct of the nurse-midwifery care process. These efforts culminated in the development of a middle range descriptive theory of nurse-midwifery care. We share those efforts with others in the hopes of encouraging dialogue and the testing of this theory in a variety of clinical settings.
