[Antibodies against hepatitis C virus in hemophiliacs: correlation with peripheral blood lymphocyte populations].

The presence of antibodies (Abs) against hepatitis C virus (HCV) was analyzed in 26 haemophiliac patients; a prevalence of 38% was found in the total series and 56% in the treated patients. There were no cases with a positive serology among the three patients who had only received pasteurized factor. The frequency of detection of anti-HCV was higher in the patients who had a positive serology for HIV (67%) and hepatitis B virus (56%) than in the seronegative patients (24 and 29%, respectively). The patients with HCV Abs showed a decrease in the helper (CD4+) lymphocytic population, mainly due to the decrease in the helper inducer (CD4+/Leu8-) cells, such alterations being more evident in the patients who also were HIV+ who showed as well an increase in the T-cell activated lymphocytes (CD3+/la+) and a decrease in CD16+ natural killer cells. These results suggest that the lymphocytic alterations found in the patients with anti-HCV Abs are not specific and would rather be related to the presence or not of HIV infection.

Brucellar and tuberculous spondylitis. A comparative study of their clinical features.

The clinical data from 19 patients with brucellar spondylitis and 15 with tuberculous spondylitis were compared. The former disease affects males whose occupations expose them to Brucella. The lumbar spine is usually involved and there are other symptoms of brucellosis. Tuberculous spondylitis is not usually accompanied by general symptoms. The dorsal spine is more frequently affected and may exhibit vertebral collapse and paraspinal abscesses. These differences permit a presumptive aetiological diagnosis, but the definitive diagnosis depends upon bacteriological tests.

[Clinical characteristics of gastric infection caused by Helicobacter pylori].

In a series of 150 patients submitted for diagnostic gastroscopy, a prospective study of nine anamnesis variables and four exploratory ones was done. Endoscopic and histopathologic diagnoses, as well as the clinical data, were obtained by blinded observers. Helicobacter pylory (Hp) presence in antral mucosa was determined by culture. A clear relationship between Hp presence and peptic ulcer disease, bulboduodenitis and histologic gastritis was found, as expected, but no clinical variable correlated positively with it. We conclude, therefore, that Hp presence in antral mucosa cannot be predicted clinically, a fact probably related to the unspecificity of symptoms in gastroduodenal disease and not to a lack of pathogenicity of this organism.

Seasonal changes in the levels of anti-Dirofilaria immitis antibodies in an exposed human population.

A prospective follow-up study to define seasonal changes in the level of anti-Dirofilaria immitis antibodies in an exposed human population in Western Spain was conducted. Specific antibodies were detected using an enzymelinked immunosorbent assay (ELISA). Ig M antibodies predominated in Summer, during maximal vector activity, while Ig G rose to a maximum in Winter. The sole risk factor associated with higher optical densities in the ELISA proved to be age, indicating that repeated exposure to the parasite is the rule. It is concluded that, for serodiagnosis the endemic or non-endemic nature of the geographical area and the season of the year should be considered to define the basal level of positivity, and that as well as Ig G, Ig M should always be determined.

A seroepidemiologic survey of human dirofilariosis in Western Spain.

The objective of this work is to define the prevalence of IgM and IgG antibodies against Dirofilaria immitis in an exposed human population. 395 individuals attending primary care facilities in an area endemic with canine dirofilariasis were studied, as well as 100 non exposed individuals. An ELISA method was used for the detection of IgG and IgM antidirofilarial antibodies. Sera were preadsorbed with antigens of Toxocara canis, Ascaris suum, Echinoccocus granulosus and Fasciola hepatica to prevent cross reactions. Global seroprevalence was 9.3%. IgG antibodies were observed in the oldest sector of the population, while IgM antibodies were found only in its youngest half. No sex differences were observed. This result show that dirofilariasis is common in humans from endemic areas. Repeated contacts with the parasite are common and begin at a young age.

Influence of sex, age, and menopause in serum osteocalcin (BGP) levels.

Osteocalcin (bone Gla protein) is a promising marker of bone turnover useful in the diagnosis and follow-up of high turnover osteoporosis. Conflicting results have been reported about its physiological variations according to sex, age, and menopause. Several, but not all, authors have found increased levels in males, with aging, and after menopause. We measured serum osteocalcin in 126 healthy subjects, 57 males and 69 females, aged between 45 and 88 years. Osteocalcin was higher (P less than 0.01) in males (6.24 +/- 0.36) than in females (4.32 +/- 0.34). This sexual difference was significant, too, in subjects younger and older than 60 years. Osteocalcin increased with age, linearly in males (P less than 0.05), and exponentially in females (P less than 0.05). Although there was a difference in age (P less than 0.05), no difference in osteocalcin levels between premenopausal women and women in their first two postmenopausal years was detected, while osteocalcin was significantly increased in women more than two years into menopause. We conclude that osteocalcin in healthy subjects is higher in males than in females and increases with age after 45 years in both sexes. Osteocalcin levels increase in women more than two years beyond menopause, but not only as an effect on aging.

[Histopathologic changes associated with the presence of Helicobacter pylori in antral mucosa].

Helicobacter pylori is a gram negative bacteria which has recently been associated to tissular changes of the upper digestive tract, however, the causal role has not yet been determined. Of 150 patients studied, 63 had tissular changes associated to Helicobacter pylori (Hp), 8 had Hp without tissular related changes (of whom 3 suffered bulbar ulcus and 1 gastric ulcus); the rest of the patients had hiatus hernia associated to distal esophagitis or pyloric stenosis; and only one patient was found to have normal tissue. A clear associated to distal esophagitis or pyloric stenosis; and only one patient was found to have normal tissue. A clear association between Hp and chronic or atrophic gastritis was determined, but no association was found between Hp and gastric cancer.

Transient solitary pulmonary nodule caused by Dirofilaria immitis.

The second described case of solitary transient lung nodule caused by Dirofilaria immitis is reported. Diagnostic thoracotomy can be avoided with the use of enzyme-linked immunosorbent assay (ELISA), and hence a conservative serological follow-up is warranted in endemic areas.

[Serology of human dirofilariasis].

The development of an immunoenzymatic technique for the detection of specific IgG against Dirofilaria immitis in humans is reported. Two groups of rabbits were used as controls; one of them was hyperimmunized with D. immitis antigens and the other was not inoculated. Subsequently, the reactivity to D. immitis and T. canis of the sera of three patients with pulmonary nodules and 25 apparently healthy individuals was evaluated. In two of the patients with pulmonary nodules the reactivity against D. immitis was significantly higher than that of healthy individuals. Reactivity against T. canis was not found in any case. These initial results seem to indicate that, in the reported conditions, this technique could prevent crossed reactions between D. immitis and T. canis and could help in the differential diagnosis of other causes of pulmonary nodules.

[Liver abscesses due to Klebsiella pneumoniae and acute anterior uveitis].

Liver abscesses are not very frequent and those produced by Klebsiella spp. are quite exceptional. These germs have been related to the development of acute anterior uveitis (AAU) in patients whose susceptibility has been linked to HLA B27. We present a case of a female, HLA B27 negative, who developed AAU during a course of liver abscesses caused by Klebsiella pneumoniae.

T lymphocyte subsets in Mediterranean spotted fever.

Several studies have previously suggested the possible role of a T lymphocyte suppressor population in infections by species of the genus Rickettsia. In 15 patients with Mediterranean spotted fever (MSF), we quantified, during the acute and convalescent phases of the disease, the peripheral blood lymphocyte populations using monoclonal antibodies that recognize CD3+, CD4+, CD8+, CD38+ and CD20+ cells. In three cases a reversal in the normal ratio of T lymphocyte helper-inducer/suppressor-cytotoxic subsets was detected lasting, in two of them, up to the fifth week of the disease. This disturbance was always weak and lacked clinical significance.

Inhibition of growth hormone and glucagon release by Phe-4-somatostatin in patients with acromegaly.

We have studied growth hormone (GH), insulin (I), glucagon (Gl) and glucose in basal conditions, after somatostatin (SS) (3 micrograms/ml) and Phe-4-SS (0.75 microgram/ml) infusion and IV bolus of the latter in 3 acromegalic patients. In two patients, Phe-4-SS inhibited glucagon and growth hormone secretion in a similar way as SS, without modifying insulin secretion. These effects were short-lived and ceased after the infusion was stopped. From these results it appears that Phe-4-SS is both more potent and more specific than SS with respect to inhibition of GH and glucagon release, but it does not have prolonged biological action. Lack of suppression of GH by SS or Phe-4-SS in the acromegalic patient with post-surgical recurrence of the disease could be due to the existence of receptor abnormalities, and suggests a diversity in the physiopathology of the disease.

A dosing study of nonoxynol-9 and genital irritation.

The objective of the study was to assess the symptoms and signs of genital irritation produced by different frequencies of nonoxynol-9 (N-9) use. Thirty-five women were randomized to each of 5 groups and used a vaginal suppository for 2 weeks. Group 1: N-9 once every other day; Group 2: N-9 once a day; Group 3: N-9 twice a day; Group 4: N-9 4 times a day; and Group 5: placebo 4 times a day. Study women were examined at admission, one week and 2 weeks with a colposcope for erythema and epithelial disruption, and were interviewed about vaginal itching and burning. The rates of reported symptoms for N-9 users were not significantly different from that of placebo users. The rate of epithelial disruption for women using N-9 every other day was essentially the same as that of women using placebo. The rates of epithelial disruption for women using N-9 1/day and 2/day were 2.5 times greater than that of placebo users. The rate of epithelial disruption for women using N-9 4/day was five times greater than that of placebo users. Genital irritation was located primarily on the vagina or cervix, and vulvitis was not a significant problem. Women who infrequently use N-9 products may not experience an increase in genital irritation. Women who choose to use N-9 frequently may experience an increase in epithelial disruption.
