Malignant mixed mesodermal tumor arising in a benign cystic teratoma.

The occurrence of sarcoma in a benign cystic teratoma is very rare. We report the first poorly differentiated, malignant mixed mesodermal tumor with a component of rhabdomyosarcoma to arise in a benign cystic teratoma of the ovary. The tumor was staged as FIGO IC due to capsule invasion. Although combination chemotherapy of cisplatin, ifosfamide and mesna, was instituted, the disease took a rapidly progressive course. After an unusual metastasis to the scapula was detected, the patient deteriorated and died in the forth postoperative month.

Stem cell factor (SCF) levels in newborns.

We studied Stem cell factor (SCF) levels in 15 mother-newborn pairs, 15 healthy adult controls, and 16 newborn with bacterial sepsis. SCF levels were also determined in six newborns with sepsis before and after completion of treatment. SCF levels (pg/mL) were found to be 2141 +/- 529 in cord blood, 1385 +/- 314 in mothers, 1546 +/- 443 in healthy adult controls, and 1742 +/- 655 in septic newborns. Cord blood SCF levels were significantly higher than their mothers' and healthy controls (p < 0.05). There were no differences in SCF levels between mothers and healthy adult controls. No correlation was found between the SCF levels and absolute neutrophil counts. There were no differences in SCF levels between the before and after treatment levels in six newborn with sepsis. In conclusion, our study suggests that SCF levels were increased in cord blood, and this increase is not a reflection of mothers' levels. SCF levels do not show significant changes during sepsis in newborns.

MR findings in peliosis hepatis.

Peliosis is an uncommon condition characterized by multiple-blood-filled cavities mostly involving the liver. Although the etiology is unknown the condition may be associated with several disease states and medications. We report the MR findings of peliosis hepatis in a patient with Fanconi anemia who had been treated with anabolic androgenic steroids for 3 years. The MR examination of the upper abdomen was performed on a 0.5 T system. The signal intensity of the right lobe of the liver was diffusely increased in all sequences. Within the enlarged liver, multiple foci of brighter signal were seen involving both right and left lobes. The lesions showed contrast enhancement. A cystic cavity with an enhancing rim was seen representing a haematoma cavity. The spleen was spared the patient died of sepsis and the postmortem examination confirmed the diagnosis of peliosis hepatis.

Immunologic evaluation of a neonate born to an immunosuppressed kidney transplant recipient.

There are only a few reports about immunologic evaluation of neonates who have been exposed to immunosuppressive drugs during fetal life. We followed up immunoglobulins, T, B and natural killer (NK) cell levels in an infant of a kidney transplant recipient who has been exposed to cyclosporine, azathioprine, and prednisolone during his fetal life. B-cell percentage, absolute counts, and immunoglobulin levels were normal at birth, but IgG value was lower than our normal controls at 3 months and low normal at 6 months of age. Serum IgM levels were normal in all determinations, but IgA levels were lower than the control values during the follow-up period. B cell percentage and absolute counts were higher than the control values at 3 and 6 months. T-cell subpopulations were normal except for CD4+/CD8+ ratios that stayed high normal due to increased CD4+ cells. CD16+ (NK cells) absolute counts and percentages were below normal at birth and at 3 months. Clinically, increased susceptibility to infection was not observed during the follow-up period. More immunologic studies are needed on possible effects of in utero exposure to cyclosporine, azathioprine, and prednisolone.

Late hemodynamic changes in percutaneous mitral valvuloplasty.

Percutaneous mitral valvuloplasty (PMV) was performed in 57 patients with mitral stenosis. Twenty-three women and 34 men (mean age 28 +/- 10 mean +/- SD) were included in the study. A single-balloon (trefoil or bifoil) technique was used in 49 patients and a double-balloon (trefoil + monofoil) technique in eight. After a 3-month follow-up period, right- and left-sided cardiac catheterization was repeated. In the single-balloon group there was improvement in the mitral valve gradient (16.10 +/- 5.99 to 4.41 +/- 2.03 mm Hg), mean left atrial pressure (22.65 +/- 6.13 to 9.76 +/- 3.01 mm Hg), and mitral valve area (0.89 +/- 0.22 to 1.95 +/- 0.46 cm2/m2). Mean pulmonary artery pressure and mean pulmonary wedge pressure decreased to 19.33 +/- 4.19 mm Hg and 10.73 +/- 2.60 mm Hg from 32.94 +/- 7.90 mm Hg and 21.49 +/- 5.98 mm Hg. Cardiac output increased to 6.86 +/- 0.56 L/min from 5.57 +/- 0.66. All improvements were statistically significant (p less than 0.001). In the double-balloon study group, mitral valve gradient (23.75 +/- 2.77 to 4.50 +/- 1.94 mm Hg), mean left atrial pressure (31.63 +/- 3.57 to 9.50 +/- 1.94 mm Hg), mean pulmonary artery pressure (44.00 +/- 6.36 to 18.88 +/- 7.10), and mean pulmonary wedge pressure (29.25 +/- 3.73 to 10.25 +/- 1.85 mm Hg) all improved significantly (p less than 0.001). Mitral valve area and cardiac output increased from 0.89 +/- 0.15 to 2.44 +/- 0.44 cm2/m2 (p less than 0.001) and from 5.46 +/- 0.76 to 7.15 +/- 0.52 L/min (p less than 0.002), respectively.(ABSTRACT TRUNCATED AT 250 WORDS)
