[Nitroblue tetrazolium test. Neonatal infections and antibiotic therapy].

The authors report the results of a prospective test of NBT carried out by an original method already described, and now used for seventy newborn babies. Eleven of them (group I) were tested by simultaneously taken samples from the cord and the heel in the labour ward. The levels obtained according to the position from which the samples were taken showen no significant difference. In 59 other infants (group 2) three successive tests were carried out in the first two weeks of life. These results were classified according to whether the children were: infected, perhaps infected and non-infected. The results confirm correlations between raised levels and infections. Giving antibiotic therapy does not alter significantly the levels in non-infected babies, nor the high positive scores in infected babies that were revealed in the four days after the start of treatment. Following this delay, levels of infected babies' blood did drop significantly, which was a probable indication of the efficacy of the antibiotic therapy.

Fibrin-fibrinogen degradation products in cerebrospinal fluid as an indicator of neonatal brain damage.

The occurrence of brain damage in 57 full term neonates was investigated. The infants were submitted to a prospective protocol involving clinical (neurological and electroencephalographical) and biological examinations, including the determination of the fibrin-fibrinogen degradation products in the blood and cerebrospinal fluid on D1, D3, D8 and D15. Psychomotor events were followed up for a year. Traumatic lumbar taps did not disturb the results significantly. Blood samples were not found to contain noticeable amounts of FDP. Conversely, the existence of FDP in CSF was not infrequent near to delivery and was shown to be closely related to brain damage as evaluated by the psychomotor events.

Qualitative cytology of cerebrospinal fluid as an indicator of neonatal brain damage and psychomotor outcome.

The authors report a double blind study on 57 full-term neonates prospectively subjected to clinical, electroencephalographical, cerebrospinal fluid and developmental examinations. Usual neonatal pleiocytosis depends on histiomonocytic cells which probably are a reflection of constant small brain damage during delivery. Infants suffering neurological sequelae at age one are recognizable as early as the 60th to 84th hours of life in view of persisting high histiomonocytic counts greater than 10 M. elements/l) and granulocytic peaks (greater than 2 M. elements/l) in clear samples. This method is then of interest, despite its invasive nature and limits (traumatic punctures, time-limits).

[Signification of fibrinogen degradation (FDP) product in CSF during the neonatal period (author's transl)].

Results of a prospective study on neonatal brain damage are reported. 57 full term newborns were subjected to clinical examination with a one year follow-up. Blood and CSF were sampled for FDP determinations on days J1, J3, J8 and when necessary, J15. Traumatic punctures do not alter results. FDP measurement were negative in the whole blood samples and most CSF ones. High FDP values in early samples are linked to brain damage and values above 4 micrograms/ml are significantly correlated to neurological sequelae.

[Rapid screening for neonatal infection by employing new techniques for the use of the nitroblue tetrazolium test (NBT) (author's transl)].

A new method for carrying out the nitroblue tetrazolium test (NBT) has been used on micro-specimens taken from 116 newborn infants for the purpose of carrying out a prospective enquiry into neonatal infection. 44 children aged from 1 to 6 days were selected at random from a population that was said to be normal in the maternity unit and were the control group. 72 children of less than 48 hours of age were made to object of a prospective study of neonatal infection, an enquiry that allowed them to be divided into 3 groups: non-infected, infected and suspect. The study showed a very strong correlation between raised scores (higher than 100) and the presence of an infection, whereas scores in the controls and the non-infected infants were low. This test, which is hardly invasive and easy to introduce as a routine when carried out early is a good screening test for infection and can be best used on the newborn population in a maternity unit.

[Oral protozoans and diabetes: study in 117 patients].

A study on the presence of oral Entamoeba gingivalis and Trichomonas tenax has been carried out on calculus or dental plaque from 117 diabetic subjects. Statistical analysis of results shows no correlation between diabetes and protozoa. The same frequency was found between diabetic and normal subjects. Nevertheless, a significant relation has been found between the diabetes and the yeasts.

[A study of the frequency of protozoa and yeasts isolated from the parodontium of 509 subjects (author's transl)].

The protozoa Entamoeba gingivalis and Trichomonas tenax together with yeasts of the genus Candida were investigated in the mouth (gums or neck of tooth) of 509 healthy or diabetic subjects. A study of the possible correlation between presence of these parasites and various local or general factors, showed that neither the sex, maxillo-facial anomalies nor smoking had any influence on parasite incidence. Entamoeba gingivalis was encountered in 85 per cent of subjects free from parodontopathy. Numerous factors influenced the presence of Trichomonas tenax: age, social status, alcohol consumption, dental condition and gingival pathology. Presence of Candida was associated with diabetes, poor buccal hygiene and dental condition.

Use of yeasts in affinity chromatography.

The use of the yeast Candida lipolytica as a support in affinity chromatography for the purification of lectins of Ricinus communis seeds has been studied. A comparison was made with columns of erythrocyte stromas and Sepharose 4B with respect to protein yields, the agglutinating activity yield, and the electrophoretic characteristics of the eluted substances. Even though the yield is less than with other supports, it does appear that the yeast columns represent an effective method for the purification of ricin lectins.

Diagnostic and therapeutic problems associated with hereditary deficiency of the C1 esterase inhibitor.

Six patients in a family with a history of hereditary angioedema reported swelling of the extremities and recurrent abdominal pain occurring spontaneously or after trauma. Attacks of oedema involving the airways, the greatest danger with this disorder, were present only in one case. This autosomal dominant disease is due to deficient activity of the inhibitor of the first component of complement. Low levels of C4, and absence of C1 esterase inhibitor confirm the diagnosis. Two asymptomatic cases with the appropriate biochemical abnormality are reported in this study. For short term prophylaxis of attacks (before surgery expecially), fresh frozen plasma is used, or better still, C1 esterase inhibitor. For long term prophylaxis of attacks antifibrinolytic and hormonal drugs are used: in two cases, the authors obtained good results with methyltestosterone after failure of tranexamic acid.

[Erythroleukemia revealed by diabetes insipidus].

A 4 year-old girl presented simultaneously with an erythroleukemia and a diabetes insipidus. The cytologic signs of dyserythropoiesis, the increase of some red blood cell glycolytic enzymes and of fetal hemoglobin, led to discuss the existence of a double population in the red blood cell series. The revealing character of diabetes insipidus, not yet reported, is emphasized.

[Fixation of various lectins at the surface of Crithidia luciliae].

Suspensions of Crithidia luciliae have been treated with 30 lectins: protozoans are agglutinated only by lectins inhibited with oses of structures I an II according to Mäkelä, and by lectins the site of fixation of which are unknown. The use of 5 lectins conjugated to fluorescein corroborate that lectins in congruity with group I and II, contrarily to those of group III, fasten upon the membrane and the flagella of Crithidia luciliae.

[Kinetic study of human lymphocytes transformation in culture by lectin from Pisum sativum (author's transl)].

Some vegetable extract have haemagglutinating activity and are able to transform lymphocytes in culture. The comparative activities of lectin from Pisum sativum and other lectins already used (phytohaere shown. This lectin can be isolated in a highly purified form which is described. Purification of the human blood lymphocytes is obtained through nylon columns and measurement of transformation and proliferation by means of 3 H thymidine incorporation. Normal lymphocyte stimulation can be obtained within a range of concentrations of stimulants can be obtained within a range of concentrations of stimulants and the curves with PHA or lectin from Pisum show a peak of blastogenic activity. The activity of lectin from Pisum sativum was studied in this way on blood cultures of 8 chronic lymphocytic leukaemias and 5 immunodeficient patients. It appeared that lectin from Pisum and PHA have the same activity. The response of human peripheral lymphocytes to various mitogenic agents in vitro does not point out the proliferative abnormal cell and may not be used to detect different populations of human lymphocytes.

[Erythrocyte stroma included in polyacrylamide gel. Applications to affinity chromatography].

Stromata prepared with human or animal red blood cells are suspended in acrylamide solution. Gel as prepared for electrophoresis is dispersed, before use, and can be used in chromatographic columns for the retention of agglutinins. Lectins, e.g., where first absorbed and then eluted with solution of inhibitory sugar or with acid buffer. Some applications are mentioned.
