Psychotropic drug use in long-term care facilities: a review of the literature.

We reviewed 19 studies of psychotropic drug use for residents of long-term care facilities, located through a MEDLINE search of the literature published between 1978 and 1990. The review compares each study in terms of its sample, research design, and findings. The studies reviewed found that a substantial proportion of nursing home residents are taking psychotropic drugs on a regular, long-term basis and this usage is accompanied by prescribing and monitoring problems and many potential adverse risks.

Aged

Hodgkin's disease involving the large bowel.

A case is described in which Hodgkin's disease occurred in the descending colon and adjacent mesenteric nodes as the only site of relapse. This rare presentation is discussed, with a review of the available literature.

Adult

The ionic components of normal human oesophageal epithelium.

The distribution of cations and anions in normal human oesophageal epithelium has been investigated with the pyroantimonate and silver-osmium tetroxide techniques. There is a discontinuous distribution of both ions in the intercellular space. The ions are associated with various organelles, as has already been described in the literature. Specifically, in the oesophageal epithelium, there are a few deposits of pyroantimonate and occasional silver in the membrane coating granules, but here is no apparent relationship of either ion with the tonofilaments or glycogen particles. The superficial cells are leaky and contain fewer ions than the deeper functional layer cells.

Anions

The distribution and mobility of surface anionic groups of normal human oesophageal epithelium following interaction with cationized verritin.

Oesophageal epithelial cells from biopsies from normal patients showed the presence of randomly distributed anionic groups, mostly sialic acid on the cell membrane in fixed material shown by cationized ferritin. When biopsies were pulse labelled, patching occurred in all three cell layers. Patching was energy dependent and did not occur at 4 degrees C. Pulse labelled material incubated on an unlabelled medium showed progressive loss of cationized ferritin from the cell membrane. This was mostly into the medium, although some was internalized in membrane profiles. A second pulse of cationized ferritin produced further patching suggesting regeneration of cell membrane. Superficial cells were leaky, but their organelles were not.

Cortical gyrification in the rhesus monkey: a test of the mechanical folding hypothesis.

A quantitative measure of the degree of cortical folding was used to test the mechanical hypothesis of cortical folding and to analyze structural properties of the rhesus monkey cortex. The rhesus monkey cortex has both its maximal degree of cortical folding and the largest ratios of supragranular laminae to the lower granular and infragranular layers in the caudal cortex, over the posterior parietal-anterior occipital regions. Low values for cortical folding and for the ratios of inner and outer cortical layers characterize frontal regions. Topographically intermediate regions are intermediate in both sets of values. Ratios of the amounts of white and gray matter have a topographic pattern that differs from those of cortical folding, suggesting that the sizes of subcortical axonal bundles are not directly associated with the degree of cortical folding. Whereas differences in mean degrees of cortical folding are correlated with brain weights among species of primates, the amount of folding is not associated with brain weight within the species.
