Hepatic abscesses and fungemia from Torulopsis glabrata. Successful treatment with percutaneous drainage and amphotericin B.

A 39-year-old man with severe diabetes mellitus, chronic pancreatic insufficiency, intrapancreatic choledochal stricture, and secondary biliary cirrhosis developed postsurgical fungemia and large hepatic abscesses due to Torulopsis glabrata. These were treated successfully with a combination of amphotericin B and percutaneous drainage of the liver abscesses. We believe this is the first reported case of such infection due to this normally saprophytic agent.

Radical neck dissection for squamous cell carcinoma of the head and neck: early and long-term results of treatment.

An account is given of the use of radical neck dissection in a consecutive series of 147 patients treated during the years 1967-76 in the Head and Neck Unit of the Royal Marsden Hospital. The majority had received previous treatment principally by irradiation to full dosage, some by previous surgery, and a minority by induction cytotoxic chemotherapy. The 5-year determinate survival results for surgical salvage are 28.03%, and 20.5% at 10 years. Prevention of recurrent cancer in the neck was achieved in 70% of all those who later succumbed to their disease.

Leukoplakia of the anal canal.

Two cases of leukoplakia of the anal canal are presented, with illustrations of the typical gross and microscopic appearance. The lesions were asymptomatic, and were discovered by routine rectal examination. Histological findings included hyperkeratosis and acanthosis extending cephalad from the anal verge to the dentate line. Followup after 3 and 15 years revealed no evidence of dysplastic changes, and a review of the literature did not uncover any reported cases with dysplasia or with documented evolution to malignancy. There is little evidence that leukoplakia of the anal canal is premalignant but patients should be followed carefully, since the natural history of this rare lesion is unknown.

Esophageal manometry in oculopharyngeal dystrophy.

Four cases of oculopharyngeal dystrophy (OPD) seen within five years are presented. The cardinal features of OPD are dysphagia and ptosis. The manometric abnormalities in OPD are similar to those seen in other myopathies; low pharyngeal pressures, abnormalities in relaxation and coordination of the pharyngoesophageal sphincter and weak aperistaltic swallows in the proximal esophagus. In addition, weak aperistaltic swallows frequently occur in the distal esophagus and may exacerbate dysphagia. Although there is no specific therapy for ODP, one patient showed marked improvement following cricopharyngeal myotomy.

Acquired double pylorus: a case report.

A case of acquired double pylorus, incident to peptic ulcer disease, is described. Fiberoptic gastroscopy confirmed a scarred antrum, a shallow prepyloric ulcer and a pylorus consisting of two large openings divided by a septum. The duodenum was visualized through each channel. This appearance probably represents a pyloroduodenal fistula caused by penetration of a prepyloric ulcer into the duodenum.
