Peritoneal dialysis in infants.

A commercially available closed dialysis system and a new peritoneal cannula with potential advantages for infants have been developed. The dialysis set includes three dialysate bags that may be connected to the filling burette; the warming coil of the set is placed in a thermostatically controlled water bath. The peritoneal catheter comprises a flexible tube with side holes and a sharp short bevelled needle with obturator. Advantages of the new equipment over previously available equipment are that the cannula is easier to insert; there is less risk of contaminating the dialysate since the tubing set is unbroken from supply to cannula; the mix of the dialysate may be changed easily without interrupting dialysis; and the equipment may be assembled easily by unskilled staff.

Cell-mediated immune response to Ross River virus in mice: evidence for a defective effector cell response.

Spleen cells recovered from mice during a primary infection with Ross River virus showed a virus-specific proliferative response when challenged in vitro. The majority of the proliferating cells were T-lymphocytes. In contrast, there was little detectable migration inhibition factor production by these spleen cells and little or no virus-specific cytotoxic activity. Sub-cutaneous challenge with virus elicited a weak delayed type hypersensitivity reaction.

Detection by ELISA of IgM antibodies to Ross River virus in serum from patients with suspected epidemic polyarthritis.

An ELISA method for the detection of IgM antibody to Ross River virus in the serum of patients with suspected epidemic polyarthritis has been developed. None of the reagents is infectious or toxic and the use of purified virus minimizes the risk of non-specific reactions to non-viral products. In a positive reaction the ELISA substrate changes colour from yellow to purple, enabling serum antibody titres to be read by eye.

Familial congenital Cushing's syndrome due to bilateral nodular adrenal hyperplasia.

Two siblings with congenital Cushing's syndrome due to bilateral nodular adrenal hyperplasia are described. The older, a boy, presented with severe hypertension and died soon after subtotal adrenalectomy. His sister, who had clitoral enlargement and showed persistent hyponatraemia, had a two-stage total adrenalectomy and is still alive. Investigations in the second case showed grossly elevated urinary cortisol metabolites, 17-oxosteroids and 3 beta-hydroxy-5-ene-steroids. These were not suppressed by dexamethasone, and plasma ACTH was undetectable, indicating that the disorder was not due to excessive ACTH secretion. Cell culture studies on the resected adrenals failed to demonstrate an abnormal pattern of steroid synthesis in vitro, and normal trophic responses were obtained with 1-24 ACTH and monobutyryl cyclic AMP. No stimulation of steroid synthesis was obtained with a range of polypeptide hormones, and the cause of the adrenal hyperplasia remains unknown.

Immunological interference of the synthetic aromatase inhibitor 1,4,6-androstatriene-3,17-dione (ATD) and its metabolite(s) in the radioimmunoassay for testosterone.

Radioimmunoassay (RIA) for testosterone (T) in unchromatographed plasma extracts from ATD-treated rats gave spuriously high values for T. Cross-reaction and chromatographic studies subsequently showed that ATD and, to a much greater extent, its metabolite(s) were responsible for this overestimation. Celite column chromatography proved to be an effective way of separating T from ATD and its product(s) of metabolism.
