Reye syndrome in siblings.

Reye syndrome in siblings was seen in three of 85 families; the incidence of RS in these family groups appears to exceed that of the general population. The interval between development of RS in the first and second siblings was two to 11 days and related to the incubation period of the initial viral infection. In five of the children this infection was chickenpox and in two, an unspecified upper respiratory illness. To assess the role of genetic factors, HLA typing was performed on these siblings; a common genetic marker indicating susceptibility to RS was not identified. All families resided in rural and suburban areas; exposure to a common environmental toxin was not identified.

Picornaviruses: rapid differentiation and identification by immune electronmicroscopy and immunodiffusion.

Immune electronmicroscopy (IEM) was used to identify human picornaviruses rapidly and to differentiate enteroviruses from rhinoviruses. Human sera, diluted 10- to 50-fold beyond the neutralisation endpoints for homologous virus, readily agglutinated C-type antigens of seven human picornaviruses. Human sera did not react by IEM with a control animal picornavirus. By IEM after acid treatment, differentiation of a human enterovirus from a human rhinovirus was possible. There was an excellent correlation between the results of IEM and immunodiffusion (ID) tests for the presence of antibody to human picornavirus group antigens. By ID, only one of 21 human sera reacted with one of six animal picornaviruses. Immune electronmicroscopy appears to be a sensitive and simple techinque for the detection of picornavirus C-type antigens, and may be useful for identifying viruses belonging to groups comprising many serotypes and sharing a common group antigen.

Thymidine kinase activity in cerebrospinal fluid of rabbits with Herpesvirus hominis encephalitis.

The rabbit model of Herpesvirus hominis (HVH) encephalitis was utilized to determine whether thymidine (TdR) kinase (EC. 2.7.1.2.1), AN EARLY ENZYME IN THe HVH replicative cycle, is present in the cerebrospinal fluid (CSF) of rabbits with HVH encephalitis. The mean TdR kinase activity in CSF from infected rabbits was 2.30 plus or minus 1.69 pmol product/120 min/100 mul CSF, while the mean TdR kinase activity of uninfected CSF was 0.34 plus or minus 0.25. The enzyme appeared to be cell associated since the TdR kinase activity in the sedimented cell suspension was 5.08 plus or minus 2.93 (seven values) while the supernatant activity was 0.14 plus or minus 0.31 (seven values). To allow a comparison of TdR kinase from infected rabbit CSF with HVH and rabbit mononuclear cell TdR kinase, the mean ratio of TdR kinase activity at pH 6.0/pH 8.0, for infected rabbit CSF is intermediate between the ratio for HVH TdR kinase (P less than 0.001) and rabbit mononuclear cell TdR kinase (P less than 0.05).

Glycerol: a review of its pharmacology, pharmacokinetics, adverse reactions, and clinical use.

Glycerol is a potent osmotic dehydrating agent with additional effects on brain metabolism. In doses of 0.25-2.0 g/kg glycerol decreases intracranial pressure in numerous disease states, including Reye's syndrome, stroke, encephalitis, meningitis, pseudotumor cerebri, central nervous system tumor, and space occupying lesions. It is also effective in lowering intraocular pressure in glaucoma and shrinking the brain during neurosurgical procedures. Hyperosmolality with rebound cerebral overhydration is of concern, especially in patients with altered blood brain barriers. They may be avoided if glycerol is administered on an intermittent rather than a continuous basis. Intravascular hemolysis does not occur with oral use. When administered intravenously, hemolysis can be minimized by using glycerol 10% in dextrose 5% with normal saline at rates of 6 mg/kg/min or less. However, intravenous doses of 1-2 g/kg every 2 hr can be administered safely in severe cases of elevated ICP. In such patients, glycerol serum concentration, serum osmolality and ICP monitoring are required to optimize glycerol therapy.

Bone scan patterns in acute osteomyelitis.

Sixteen patients with acute hematogenous osteomyelitis underwent serial Tc-99m-diphosphonate bone scanning on admission and at intervals of four to seven days thereafter. Scans were analyzed by computer-assisted determination of the ratio of activity in the lesion to that in a corresponding area of normal bone. Two patterns were observed. In seven children this ratio was low initially and increased during hospitalization despite clinically adequate therapy. In nine children the ratio was high initially and decreased during therapy. Lack of focal uptake within bone on the initial scan performed within the first three days of the onset did not exclude the diagnosis of osteomyelitis.
