[Physico-chemical properties related to gastrointestinal function of 6 sources of dietary fiber].

The physicochemical properties of the six sources of dietary fiber more commonly consumed in Mexico were studied. The knowledge of these properties may allow a prediction of the functional effect of the fibers on the GI tract. The content of dietary fiber varied from 43% in wheat bran (WB) to 87% in Psyllium plantago (PP). Glucomannan (GM) showed the highest proportion of soluble fiber (97%) followed by dried cactus from Opuntia sp (DC) (28%) and PP (13%), whereas WB and soy isolate (SI) showed the highest proportion of insoluble fiber (95 and 92%, respectively). PP and GM showed a higher water holding capacity and viscosity and a lower ion exchange capacity: we anticipate that these fibers would be more effective to regulate plasma levels of glucose and cholesterol in contrast, WB and SI with a lower water holding capacity and viscosity and a higher proportion of insoluble fiber would be more effective to increase fecal bulk. Fibers from DC and from cactus fiber isolate were intermediate in these responses.

[Treatment of dysthymia with sertraline].

We studied 50 patients (40 females, 10 males) diagnosed of dysthymia according to 1CD-10 criteria (F34.1), retrospectively and prospectively. All were treated with Sertraline, with an initial dose of Sertraline 50-100 mg/day, allowing increases to a maximum of 200 mg/day during the follow up. The clinical and therapeutical evolution was measured by HDRS (Hamilton Depression Rating Scale) 21 items, HARS (Hamilton Anxiety Rating Scale), Clinical Global Impression (CGI), subjective patient impression, compliance, secondary effects, complementary treatments when needed, severity, evolution evaluated on days 15, 30, 60 and 90, from the patient and psychiatrist perspective. Seventy two percent of the sample completed the three months of treatment. We obtained a good clinical efficacy with sertraline observed by a statistical significant decrease (p < 0.01) in the scales in the second week of treatment, which continued in following weeks. The mean doses varted from 78 mg/day to 98.7 mg/day during the three months of follow up The tolerance to Sertraline in our sample was good and similar to other data in the literature. Fourteen patients withdrew, but only six (12%) were due to secondary effects In spite of the study's limitations due to the lack of a control group, we could affirm that sertraline was an effective treatment for dysthymic patients and it should be recommended as a first choice in the treatment of affective disorders, facilitating other psychotherapeutical approach due to its favourable tolerance profile.

[Acute correction of nocturnal hypoxemia and sleep pattern using continuous nasal positive pressure in patients with obstructive sleep apnea syndrome].

In order to find out the effectiveness of the nasal CPAP in the treatment of obstructive sleep apnea (OSA), we studied 30 patients with this diagnosis corroborated by nocturnal polysomnography. The average age was 46.37 +/- 11.58 years, 97% were males and 90% were overweight, with a weight of 101.24 +/- 15.57 Kg. All the patients were evaluated before and after the application of nasal CPAP with a pressure of 12.9 +/- 4.35 cm of H2O with a FIO2 of 21%. The sleep efficiency index improved from 0.62 +/- 0.03 to 0.84 +/- 0.02 (p < 0.0001). The maximum duration of the apneas diminished from 65.5 +/- 6.5 seconds to 19 +/- 3.7 seconds (p < 0.0001), and the minimum level of O2 saturation of the hemoglobin increased from 56.8 +/- 3.2% to 84.9 +/- 1.9% (p < 00.0001). These findings showed a clear improvement in the majority of the evaluated parameters in all of the patients, and, in many of them, a normalization. We conclude that the nasal CPAP is effective for the acute improvement of the disorders observed during the sleep in patients with OSA, which supports its therapeutic use in this illness.
