[Colorectal cancer: has its diagnosis improved in the last decade?].

OBJECTIVE: To assess whether any shift in pathologic stage, tumor resectability and need of emergency surgery has been observed in colorectal carcinoma patients over the last ten years. EXPERIMENTAL DESIGN: Retrospective study. PATIENTS: We compare 201 patients treated in our Service from January 1981 to December 1983 (Group I) with 306 patients operated from January 1991 to December 1993 (Group II). RESULTS: No change has been noticed in any of the parameters analyzed. CONCLUSIONS: We think that the efforts in earlier detection of CCR should be increased to improve the prognosis of the disease.

[A comparative study on the prognostic value of the nuclear expression of protein p53 vis-à-vis histopathology in colorectal cancer].

OBJECTIVE: To determine the predictive value of p53 nuclear overexpression in comparison with established prognostic pathological features in colorectal adenocarcinoma. PATIENTS AND METHODS: 61 patients operated on for cure between January 1989 an December 1991 were included. Expression of p53 protein was examined by immunohistochemistry in paraffin-embedded sections. Tumor localization, depth of bowel wall involvement, lymph nodes metastasis, vascular invasion and PCNA Labelling index were studied in all patients. RESULTS: Nuclear staining was detected in 27 (44.2%) cases. Positivity was more frequent in tumors with venous invasion and in rectal cancer. p53-positive tumours exhibited a higher likelihood of relapse and lower survival. After adjustment for the other covariates, p53 overexpression was the only factor showing independent prognostic significance on the risk of recurrence. None of the factors analysed evinced independent significant relationship with the risk of death. CONCLUSION: Nuclear p53 protein overexpression is closely related to the development of postoperative recurrences and has higher predictive value than standard pathological variables.

Further insights on chromosomal pairing of autopolyploids: a triploid and tetraploids of rye.

Chromosomal pairing of one triploid and three tetraploid plants of rye, Secale cereale, was analyzed by electron microscopy in surface-spread prophase I nuclei and compared with light microscopic observations of metaphase I cells. Prophase I is characterized by: (i) the weak alignment showed by the three or four unsynapsed or partially homologous synapsed axes; (ii) the low number of pairing partner switches (PPSs) displayed by both trivalents and quadrivalents; and (iii) the existence of complex multivalents in which up to 13 chromosomes in the triploid and 22 chromosomes in the tetraploids were involved. However, only few heterologous chromosomal associations were maintained at metaphase I. The results obtained are discussed under the assumptions of the random end pairing model with some modifications.

Synaptic patterns of rye B chromosomes. IV. The B isochromosomes.

Pairing of B isochromosomes in rye was studied by surface spreading of synaptonemal complexes in pollen mother cells from plants containing one small iso-B (iso-BS), one large iso-B (iso-BL), two iso-BL, one iso-BS plus one standard B (iso-BS+stB), one iso-BS plus two standard Bs (iso-BS + 2 stB) and one iso-BL plus two standard Bs (iso-BL + 2 stB). The main characteristics of iso-Bs were as follows. (1) Their location in the surface-spread nuclei was peripheral. (2) Their period of pairing relative to that of the A-set was delayed. (3) The commonest pachytene configuration was a symmetrical hairpin loop, although they could also undergo nonhomologous synapsis. (4) Synapsis could start at the chromosome ends, at the putative centromeric region or at both regions more or less simultaneously. (5) The frequency of iso-BL bivalents was only 46 per cent rather than the 66 per cent predicted by the random-end-pairing model. Interchromosomal pairing was frequent in these configurations. (6) In pairing competition situations involving three and four doses of the short arm of the standard B (iso-BS+stB, iso-BS + 2 stB), a large predominance of self-synapsed iso-BS univalents (63 per cent and 80 per cent respectively) was found. On the other hand, there was no apparent pairing preference among four doses of the long arm of the standard B in the iso-BL + 2 stB combination. Similarities and differences between the synaptic behaviours of iso-Bs of Crepis capillaris and Secale cereale are discussed.

[The prognostic value of common predictive variables in rectal cancer].

BACKGROUND: A study was undertaken to identify common clinical variables, easy to obtain in an out-patient office, that complement the prognostic estimation on survival offered by the staging classification in patients operated on for rectal cancer. PATIENTS AND METHODS: 236 patients were included. Twenty-one variables related to symptoms, examinations, type of operation, tumor stage, pathology and follow-up were evaluated, all of them were collected prospectively. The relative predictive value was analyzed by means of the Cox' proportional hazards progression model. RESULTS: Age, carcinoembryonic antigen (CEA), alcaline fosfatase (FA), deshidrogenase lactate (LDH), elective surgery, showed independent predictive value. The risk of death raised by 1.026 (CI 95%: 1.005-1.047) for each year of age at the moment of surgery, serum CEA over 5 ng/ml raises the risk by 2.32 (IC 95%: 1.385-3.893), LDH over 190 mU/ml by raises the risk by 1.64 (IC 95%: 1.026-2.639), FA over 250 U/ml raises the risk by 2.16 (IC 95%: 1.027-4.578), elective surgery reduces the risk by 0.32 (IC 95%: 0.106-0.965). CONCLUSIONS: Age, CEA, LDH, FA and elective surgery, complement the prediction on survival offered by the tumor stage in patients operated on for rectal cancer.

Synaptic patterns of rye B chromosomes. III. The deficient B.

Chromosomal pairing of a deficient B chromosome in rye has been studied by surface spreading of synaptonemal complexes in pollen mother cells from plants containing one deficient B (dB), one deficient B plus one standard B (dBstB) and one deficient B plus two standard Bs (dB2stB). Single dBs are often located peripherally in surface spread nuclei, and they undergo extensive non-homologous synapsis at pachytene. In the dBstB bivalents, homologous and non-homologous synapses occur simultaneously throughout pachytene without axial equalization. Competition for pairing between the dB and the two stBs results in a large predominance (80%) of stB bivalents. This finding is explained by the identical origin of the standard Bs and/or by the minor homology between the stB and the dB chromosomes.

[The results after extensive radical resection of locally advanced colorectal carcinoma].

The results of extended resections for locally advanced colorectal cancer and the factors influencing the long term survival and recurrence rate are analyzed in 69 patients. All of them underwent radical en-bloc resection including the primary tumor and the adjacent affected structures. The existence of tumoral invasion was confirmed in 42 cases (60%). In the remaining, inflammatory adhesions were presents. Overall 5 year survival did not show significant differences between the groups of patients with benign or malignant infiltration (71% y 51% respectively). Within the same tumor stage, the results were similar for the two types of infiltration. Significant differences were found when lymph nodes metastases were considered: 65% 5 year survival in patients with negative lymph nodes versus 35% in patients with positive nodes. The probability of metastases and tumor recurrence was significantly higher in the group with malignant infiltration. Patients with positive lymph nodes showed lower disease-free interval rate. These data show that long term control of the tumor in locally advanced colorectal carcinomas can be achieved by an aggressive surgical approach. The presence of lymph mode metastases is a more useful prognostic factor influencing survival and relapse rate that the local infiltration by it-self.

Synaptic patterns of rye B chromosomes. I: The standard type.

Chromosome pairing of standard B chromosomes (Bs) in rye has been studied by synaptonemal complex surface spreading of pollen mother cells containing from one to eight Bs. The main characteristics exhibited by the Bs are: (i) They are often located peripherally in surface spread nuclei and do not show presynaptic alignment. (ii) The period of pairing is delayed relative to that of the A set. (iii) Bs undergo extensive non-homol-ogous pairing at pachytene. (iv) The frequency of trivalents and quadrivalents in plants with 3Bs and 4Bs is about 66% following the expectations on the basis of random pairing among the short and the long arms with a single distal pairing initiation site per arm. Some quadrivalents containing two partner switches were observed, but on the same basis of randomness there was a deficiency of pachytene multivalents in plants with 5-8 Bs. The possible causes of the discrepancy between these observations are discussed. (v) There is a reduction in the frequency of multivalents between pachytene and metaphase I.

Continent colostomy by means of a new one-piece disposable device. Preliminary report.

A study was established concerning a continent colostomy system consisting of a new one-piece disposable plug. Twenty patients participated: 13 men and 7 women, who ranged in age from 35 to 87 years. All patients had an end colostomy located on the left lower quadrant. The colostomy age ranged from 2 months to 7 years. Of the 20 participants, 17 found the new procedure to be safer and more comfortable, as well as a considerable improvement for their quality of life. The other three patients found no advantage in its use. In no case was there any perception of local, secondary, or general adverse effects owing to its use. Taking into consideration that the use of this new device does not require any type of special conditioning, we consider that it can contribute to the better quality of life of a colostomy patient, the one best able to evaluate the worthiness of its use.

[Renal amyloidosis in a case of Crohn disease with colonic and rectal involvement].

A clinical case characterized by renal amyloidosis, Crohn's disease of the colon and rectum, multiple chronic anal fistulae and ankylosing spondylitis is reported. This association has been infrequently recognized. In this patient Crohn's disease became manifest at 20 years of age and underwent a chronic and relapsing course. Proteinuria and renal amyloidosis were detected after eight years of evolution. Panproctocolectomy was performed. The patient progressed satisfactorily and six months later he remains asymptomatic, free from medication and with normal creatinine clearance. The authors recommend early surgical treatment on the diseased intestinal segment once the association has been diagnosed and before serious impairment of renal function has occurred.

Serum and tissue trace metal levels in lung cancer.

Copper, zinc, magnesium, calcium and iron were measured in serum and lung tissue - tumor mass and histologically nonneoplastic tissue - from lung cancer patients and compared with serum concentrations in healthy subjects and control lung tissue obtained from patients with nonmalignant lung disease. Lung cancer patients showed a significant increase in serum Cu and Cu/Zn ratio levels and decrease in serum Zn and Fe concentrations. These findings were correlated with TNM stage of the disease, but not with histologic type of tumor. Malignant lung tissue showed a higher level of Cu, Ca, Mg, and Cu/Zn ratio and lower Zn level than that found in control samples, as well as an increase in Cu, Mg and Cu/Zn ratio concentrations with regard to histologically nonneoplastic tissue samples from the same patient. Tissue concentration of trace metals was not significantly influenced either by histologic type of tumor or clinical TNM stage. Significant correlation coefficients between serum and tissue trace metal levels were not found.

Three-year prospective study of intestinal pathogens in Madrid, Spain.

During the period July 1980 through June 1983, in a General Hospital in Madrid, the following organisms were detected from 6,970 patients with gastroenteritis: 710 Salmonella spp. 506 Campylobacter jejuni, 379 Shigella spp., 12 Yersinia enterocolitica, 1,466 rotavirus, 134 Giardia lamblia, and 4 Entamoeba histolytica. Chloramphenicol showed good activity against most tested strains of Salmonella spp., Shigella spp., and C. jejuni. The incidence of Salmonella spp. and Shigella spp. was very marked in the hot dry months of the year, rotavirus predominated during the cold months, and no seasonal variations of importance were seen for C. jejuni and G. lamblia.
