Autoantibodies from Sjögren's syndrome induce activation of both the intrinsic and extrinsic apoptotic pathways in human salivary gland cell line A-253.

Sjögren's syndrome (SS) is an autoimmune rheumatic disease that targets salivary and lachrymal glands, characterized by a high concentration of serum autoantibodies directed against nuclear and cytoplasmic antigens. It is known that autoantibodies can enter viable cells and this phenomenon has functional consequences including activation of apoptotic process. The objective of this work was to explore whether autoantibodies contained in IgG purified from Sjögren sera trigger apoptotic process in an experimental model represented by the human salivary gland cell line A-253. To define if the intrinsic or extrinsic pathways are activated, we examined which caspases are critical for inducing cell death. The results have demonstrated that morphological changes and DNA laddering, consistent with apoptotic cell death, occurred in A-253 cells treated with IgG from Sjögren sera. Sjögren IgG induced cleavage and activation of the effector caspase-3 and degradation of the caspase-3 substrate poly(ADP-ribose)polymerase. Both the intrinsic and extrinsic apoptotic pathways were activated, since both caspase-8 and caspase-9 cleavages occurred. In conclusion, autoantibodies contained in IgG purified from Sjögren sera mediate apoptosis of the A-253 cell line in a caspase-dependent manner.

Receptor activator of NF(Kappa)B ligand/osteoprotegerin (RANKL/OPG) system and osteopontin (OPN) serum levels in a population of apulian postmenopausal women.

AIM: Osteoporosis is a bone disease, characterized by a reduction of bone resistance; in postmenopausal period bone metabolism is imbalanced. Several parameters have been proposed as markers of bone metabolism; the attention have been recently placed on the receptor of activator of NF(Kappa)B ligand receptor (RANKL) and osteoprotegerin (OPG), namely RANKL/OPG system. The aim of this paper is to evaluate changes in postmenopausal women in serum concentration of OPG, RANKL, and their ratio (i.e. RANKL/OPG ratio), osteopontin (OPN), bone-type alcaline phosphatase (BAP), serum-N-Telopeptide of type I collagen (serum-NTX); and their correlations with bone mineral density (BMD). METHODS: An Apulian population group of 163 native postmenopausal women were followed at the Osteoporosis Centre of Policlinico of Bari (Southenrn Italy). Patients were classified into 3 separate groups, according to T-score: osteoporotic, osteopenic and control group. Serum concentrations of OPG, RANKL, RANKL/OPG ratio, BAP and NTX have been calculated. RESULTS: No differences were found in OPG and BAP values. Significant correlations were found in the osteopenic group between OPG and RANKL (negative), and between RANKL and OPN or serum-NTX, OPN and serum-NTX or RANKL/OPG ratio, BAP and serum-NTX, serum-NTX and RANKL/OPG ratio (positive). In the other groups a significant correlation was observed between BAP and NTX. CONCLUSIONS: In postmenopausal women, important modifications of bone metabolism markers (i.e. RANKL, OPG and OPN) could be due to serious engagement of bone turnover, especially in the pre-osteoporotic phase. Low bone density in postmenopausal women should be identified as soon as possible, and urgent measures are needed to reverse the process. Parameters namely RANKL e OPG may become an important index for the evaluation of the activity of drugs against osteoporosis, old and new like AMG 162 (anti-RANKL action).

[The strange case of a patient affected by acromegaly with osteoporomalacia without hypogonadism].

Acromegaly is a rare disease that, in the majority of cases, is due to the presence of a benign growth hormone (GH)-producing tumor of the pituitary. Growth hormone has profound effects on linear bone growth, bone metabolism, and bone mass. In acromegaly, the skeletal effects of chronic GH excess have been mainly addressed by evaluating bone mineral density (BMD). Most data were obtained in patients with active acromegaly, and apparently high or normal BMD was observed in the absence of hypogonadism. The Autors describe a case of patient affected by acromegaly without hypogonadism with serious osteoporosis and biological signs of osteomalacia.

[Postmenopausal osteoporosis: risk factors and possible therapies].

Risk factors for osteoporosis and the therapies (including those still being validated) utilisable in the primary and secondary management of patients with declared osteoporosis or those at risk of the disease are reviewed. The role of oestrogens in the pathogenesis of postmenopausal osteoporosis is well documented and replacement hormonal therapy prevents the rapid bone loss that accompanies the menopause, albeit with the well-known limitations. A valid alternative would seem to be provided by a diet rich in phyto-oestrogens which as they are natural SERMs, are able to produce effects similar to the oestrogens. The drugs of the new generation are also taking on an increasingly important role in the treatment of postmenopausal osteoporosis, although it is reasonable to think that in order to define them as effective and innocuous it will be necessary to await the conclusion of ongoing trials and an adequate number of years.

Efficacy of a soy rich diet in preventing postmenopausal osteoporosis: the Menfis randomized trial.

OBJECTIVES: To compare the effect of a soy rich diet and hormone replacement therapy (HRT) on the main biomarkers of bone turnover and bone mineral density (BMD) at postmenopausal age. METHODS: 187 healthy asymptomatic postmenopausal women, aged 39-60, were recruited and randomized into a soy rich diet group, a HRT group, and a control group. Bone biomarkers and BMD were evaluated at baseline and after 6 months at the end of the study. RESULTS: Diet is not as effective as HRT in reducing the postmenopausal turnover; however diet stimulates bone osteoblastic activity, as evidenced by significant increase in osteocalcin concentrations. BMD decreases significantly only in the control group, but not in the intervention groups. CONCLUSIONS: Our data suggest that soy products could be effective in reducing the risk of osteoporosis in asymptomatic postmenopausal women, but our findings should be confirmed before recommending the diet as a valid alternative to HRT.

The "submerged" connective tissue diseases.

BACKGROUND: Many patients present with laboratory abnormalities suggestive of connective tissue diseases (CTD), but with a few clinical symptoms, therefore not fulfilling the criteria for a specific diagnosis. This fact led us to suppose the existence of a changeable long "preclinical" phase in CTD development. In the past, we have already dealt with this subject, but the existence of a few reports underlining this concept led us to describe some "paradigmatic" clinical cases, and we suggest the definition of "submerged connective tissue diseases (Sub-CTD)". METHODS: During the last 5 years, we observed 25 patients (23 females and 2 males), with a mean age of 32 years, who were enrolled in a prospective study, with a mean follow-up of 23 months. All the patients were ANA-positive and went to our observation presenting only 1 clinical symptom and other laboratory abnormalities suggestive of CTD. RESULTS: The following development of the clinical picture allowed us to make a specific CTD diagnosis in 22 cases. The follow-up study shows the correlation between the starting symptoms, the serological markers (detected at the first examination) and the definitive diagnosis. CONCLUSIONS: From this point of view, we emphasize the importance of the detection of very sensitive and specific serological markers, which are useful for an early diagnosis, particularly in this "submerged" phase of the disease.

[Tumor markers in some chronic inflammatory diseases in rheumatology: a statistical evaluation].

BACKGROUND: Since tumor markers can be high in the course of many inflammatory diseases, the aim of this study is to verify if it also occurs in the course of rheumathologic chronic inflammatory diseases, and if there is any statistical correlation between tumor markers and inflammatory indices. METHODS: Seventy-nine patients (51 females and 28 males) with rheumatologic chronic inflammatory diseases, aged 17-92 years, were studied, all of them took 4 mg of prednisone. alphaFP, CEA, TPA, CA19.9, CA15.3, CA72.4, CA125, ferritina, beta2 microglobuling, betaHCG, and free and total PSA in males, were evaluated as tumor markers; and VES, PCR and Fibrinogen, as inflammatory indices. RESULTS: The results obtained showed that there is a significative correlation between ferritin, beta2 microglobulin, TPA and PCR, and between free and total PSA and Fibrinogen. CONCLUSIONS: PCR is a very good index of an active disease and it can be helpful, along with tumor markers, in the monitoring of chronic inflammatory diseases.

Systemic sclerosis stimulates angiogenesis in the chick embryo chorioallantoic membrane.

Skin biopsies from patients with systemic sclerosis (SSc) were investigated for their angiogenic activity by using the chick embryo chorioallantoic membrane (CAM) assay. Ten samples of SSc and 10 of normal skin from age- and sex-matched subjects were grafted onto the CAM, and the angiogenic response in pathological and control implants was assessed on histological sections by a planimetric point-count method 4 days after grafting. The vascular counts in the area underlying the SSc were significantly higher than those of normal skin and a dense mononuclear cell infiltrate was detectable around the blood vessels in pathological specimens. These results suggest that SSc may promote angiogenesis, perhaps leading to the release of several angiogenic factors. Moreover, the role played in the angiogenic response by the inflammatory cells forming the cellular infiltrate is suggested by this study.

[Benign edematous polysynovitis in the elderly (the RS3PE syndrome). Current knowledge].

On the basis of their own and other researchers' knowledge, the authors describe the clinical and laboratory characteristics of benign edematous polysynovitis in the elderly. They suggest that the term polysynovitis should be used in preference to polyarthritis since it gives a better idea anatomic and pathological characteristics of RS3PE syndrome.

Validity of echographic evaluation of cartilage in gonarthrosis. Preliminary report.

We studied an echographic technique by which precise reproducible measurements of articular cartilage thickness of the knee is possible. Two groups of individuals were studied: a group of 18 patients with gonarthrosis and a control group of 10 normal individuals. The group of 18 patients with gonarthrosis was studied by ultrasound (US) before knee prosthesis surgery. The cartilage thickness was measured within the weight-bearing area. US re-evaluation and histological measurements were made on the pathological specimen following the operation. Results of pre- and post-operative US data were compared with histological data. A good correlation between these measurements was found [P(t) > 10%]. In order to have comparative reference values of the articular cartilage within the weight-bearing area of the femoral trochlea a group of 10 control subjects was also studied with US as above. We found that the articular cartilage thickness of the femoral trochlea in the weight-bearing area has a mean of 2.2 +/- 0.3 mm for the lateral condyle and 2.3 +/- 0.2 mm for the medial condyle. The intra-observer and inter-observer difference in measurements was evaluated with Student's t-test. Our data demonstrate that US measurements of articular cartilage thickness of femoral condyles is a sensitive and reproducible technique which permits early diagnosis and management of knee arthropathy as well as quantification of cartilage damage.

Echographic study of Tietze's syndrome.

An echographic study of Tietze's syndrome was carried out. Three patients with clinical evidence of Tietze's syndrome were examined. Ten normal volunteers were studied as controls. In Tietze's syndrome a dishomogeneous increase of the echogenicity in pathological cartilage and an increased thickness vs. the opposite normal side were observed. A moderate thickness and a decreased echogenicity of surrounding soft tissues was observed in one case.

Genetic diversity at an oncogene locus and in mitochondrial DNA between populations of cancer-prone Atlantic tomcod.

It has been reported that Atlantic tomcod (Microgadus tomcod) from the Hudson River exhibit an extremely high incidence of liver tumors. More than 90% of spawning 2-year-old fish display hepatocellular carcinomas. In contrast, representatives of this species from a relatively pristine environment show a much lower incidence of tumors. Genomic DNA and mitochondrial DNA (mtDNA) were isolated from tomcod from the Hudson River, New York, and the Saco River and Royal River, Maine. We found a statistically significant difference in the frequency of PstI-generated restriction fragment length polymorphisms in the abl cellular oncogene between Hudson and Maine tomcod. Allelic variation was observed at two of the three abl domains scored. A single composite genotype seen in approximately 40% of Hudson River fish was seen in only one Maine fish. This polymorphism enabled us to differentiate a Hudson River population from that encountered in the Maine rivers. This is the first demonstration of a population-specific polymorphism at a cellular oncogene locus in any species. In contrast, no restriction site polymorphisms were seen in mtDNA between the populations. The lack of mtDNA diversity in these fish is consistent with the geological history of the area. In combination, these results suggest that the genetic diversity observed at the c-abl oncogene locus must have been a fairly recent event and that oncogene loci may be particularly sensitive to mutational change.

[Bone densitometry. A critical review and the authors' own experience].

After an updated review of computerised bone densitometry, personal experience of the bone density found in healthy subjects and rheumatoid arthritis patients both those treated with prednisone and untreated is reported. In addition certain biohumoral parameters like bone calcium, PTH, CT and l'1-25 (OH)2D3 were investigated. The results show significantly lower levels of bone calcium in treated than untreated patients.
