Giant sigmoid diverticulum.

The case of a giant diverticulum of the sigmoid colon in a 74 year-old woman is reported. The diverticulum had perforated into the urinary bladder and required a 3-phase operation with resection of the sigmoid colon and urinary bladder. Giant diverticulum is considered a rare complication of diverticulosis. Generally, it is a pseudo-diverticulum that grows due to a valve mechanism working in the neck of the diverticulum. The differential diagnosis lies mainly between intestinal duplication and pneumatosis cystoides. Surgical therapy yields favorable results.

[Use of polypropylene (Marlex) prosthesis in the surgical treatment of inguinal and crural hernia].

High biocompatibility, low cost and easiness in use are most important reasons for the spread of biological prosthesis in inguinal and femoral hernia repairs; this fact has changed the surgical approach to this kind of pathologies. This new approach consists in re-creating a normal anatomic function of the abdominal wall, without new tension between muscles and aponeurotic structures. The authors present their experience after 379 inguinal and femoral hernia repairs between January 1992 and December 1993.

Growth and endocrine function following bone marrow transplantation for thalassemia.

Twenty two patients with thalassemia major who received successful bone marrow transplantation (BMT) were followed to verify the impact of the transplant procedure on subsequent growth and development. The transplant preparative regimen consisted of busulphan and cyclophosphamide. Growth and endocrinological function were assessed during the first 4 years following BMT. At the time of transplant most patients showed growth retardation. The median difference between chronological age and bone age was -9.5 months for the boys and -8.5 months for the girls. Patients > 7 years old at the time of BMT showed a significant worsening of their growth delay at 48 months following BMT compared with 12 months before transplantation. Patients < 7 years at the time of BMT had their growth retardation constant over time span after transplantation. Moreover six of 11 younger patients showed an improvement of their growth delay compared with one of 11 older patients. The outcome of height standard deviation score at 24 and 48 months following BMT was strictly correlated with the level of serum transaminases and ferritin. Sixteen patients had impaired growth hormone secretion after a provocative test evaluated at 24 months after transplant. At 48 months there was no significant increase in the mean peak GH levels. This study confirms that the growth retardation of patients with thalassemia major is multifactorial.

Percutaneous alcohol sclerotherapy of simple hepatic cysts. Results from a multicentre survey in Italy.

The increased use of Ultrasonography (US) has led to increased detection of simple hepatic cysts. For symptomatic cysts treatment is necessary. Until some years ago surgery was the only therapy. We have treated a large number of patients with Percutaneous Alcohol Sclerotherapy (PAS) and evaluated retrospectively the efficacy of this approach. Data on 21 patients with symptomatic simple hepatic cysts were reviewed retrospectively. Cysts had a mean diameter of 9 cm (range: 7-15 cm). PAS was always performed under local anesthesia and US guidance. 25% of the volume was replaced with 95% ethanol and then completely aspirated after 20-30 minutes. No complications or deaths occurred. In all patients symptoms disappeared after treatment. In 15 out of 21 cases there was no evidence of residual cyst on US, computed tomography (CT) or magnetic resonance (MRI). In 6 patients with shorter follow-up, cysts showed a mean reduction in diameter of 50%. The mean follow-up was 18 months (range 6-60 months). We conclude that PAS is easy with low risk for the patients and with good long-term results; it should therefore become the procedure of choice for simple hepatic cysts.

[Modified Gilbert's technique in the treatment of persistent inguino-scrotal hernias in aged patients].

In elderly males, and in particular in very old men, the anatomical changes produced by senility encourage the formation of large inguino-scrotal hernias. Their repair may be achieved using Gilbert's technique to which the authors have proposed a number of variants. The good results show that the "tension-free" technique may be correctly applied in elderly patients with inguino-scrotal hernias, thus allowing a straightforward postoperative recovery with no complications.

[Immunological variations after thymic stimulation with special reference to IL2-RS in patients operated for neoplasms].

The paper reports a study which was carried out to assess immunological function by assaying IL2 in cancer-operated patients treated with thymostimulin. The results of the study show that, used in normally fed, immunodepressed patients, this immunomodulator is able to normalise levels of lymphocyte subpopulations and to bring serum concentrations of IL2-RS to the levels of normoergic patients.

[Effect of pre-treatment with pyridostigmine on the stimulation of growth hormone by clonidine and GRF].

The large availability of biosynthetic GH suggested the need to define the more accurate way to make diagnosis of GH deficit. Only one stimulation test by clonidine or insulin is not enough to define a GH deficit, and this because often it's possible to get "false negative" tests. The GH is regulated by the influence of GRF and somatostatin that respectively are under the adrenergic and cholinergic control, for this reason we studied how and in which measure a cholinergic agonist (pyridostigmine) acts on GH release during the clonidine and GRF stimulation tests. We studied the area under the curve (AUC), the peak and the mean of the value of GH after clonidine or clonidine and pyridostigmine, and after GRF or GRF and pyridostigmine: we got the following results: 191 +/- 71.33 (AUC), 5.42 +/- 1.68 (peak), 2.44 +/- 0.54 (mean) after clonidine stimulation test; 1048 +/- 442.37 (AUC), 19.5 +/- 10.15 (peak) and 7.96 +/- 3.2 (mean) after clonidine and pyridostigmine (p less than 0.01); 1499 +/- 887 (AUC), 21.1 +/- 11.8 (peak) and 11.11 +/- 6.6 (mean) after GRF test and 2370 +/- 332 (AUC), 31.4 +/- 3.49 (peak) and 18.22 +/- 3.27 (mean) after GRF and pyridostigmine. The pyridostigmine effect on the simulation by clonidine and GRF is able to potentiate the stimulation of GH and allowed a more accurate diagnosis of GH deficit.

Our experience with strictureplasty in the surgical treatment of Crohn's disease.

Strictureplasty is a recent surgical procedure that permits surgical cure of chronic intestinal strictures due to Crohn's disease without the need for bowel resection. The authors report the results of 19 stricture-plasties in eight patients, all of whom were affected by Crohn's disease in the chronic phase and showed subocclusive symptoms due to ileal strictures and fistulas. After surgery all patients improved, and none showed occlusive or subocclusive symptoms or signs of relapse after a mean follow-up of 16.5 months. Radiographic examination of the small bowel, when performed, showed normal transit in all patients. The authors stress the importance of performing strictureplasty only during quiescent periods of the disease and in patients in a good nutritional state, in order to avoid complications and recrudescences. If these requirements are met, stricureplasty can be very useful for saving bowel length in the surgical treatment of Crohn's disease.

Lateral high abdominal ovariopexy: an original surgical technique for protection of the ovaries during curative radiotherapy for Hodgkin's disease.

An original surgical method for gonadal protection in young women given pelvic radiation for Hodgkin's disease is presented. Lateral high ovarian transposition (LHAO) consists of the transposition of the ovaries into the paracolic gutter during staging laparotomy, after disconnecting the gonads from the fallopian tubes by dividing the tubo-ovarian vessels. The technique's effectiveness was assessed by a study using clinical investigation, radioimmunoassay (RIA) determination of sex hormones, and dosimetry; of 18 patients treated, 10 participated in the study. All but one have normal menses and hormone values, and one pregnancy occurred. We also calculated the doses absorbed by the ovaries and proved that, during inverted Y irradiation following LHAO, the ovaries are exposed to nearly one-half the dose they receive after traditional medial transposition. During subtotal nodal irradiation after LHAO, the irradiation dose is higher than after medialisation, but absolute values are minimal and castration is not induced.

[Immuno-oncologic monitoring of patients with bronchial carcinoma. IV. Aspects of the immunocompetence system].

Some aspects of the immunocompetent system are tested in 84 lung cancer patients at time of diagnosis and during the natural course of the disease. Results show no significant variations for many immunological tests during the course of the neoplasia. Significant alterations are observed in T-lymphocyte and macrophage evaluation. Moreover, some immunological data are related to lung cancer post-surgical survival.
