Lack of oxidative stress in a selenium deficient area in Ivory Coast--potential nutritional antioxidant role of crude palm oil.

BACKGROUND: Previous studies have described an important selenium deficiency in a mountain region (Glanle) in the west of Ivory Coast. AIM OF THE STUDY: To assess the antioxidant capacity of subjects from a selenium deficient area in Ivory Coast (Glanle region). METHODS: This study involved 57 subjects, 18 to 69 years old, living in the Glanle region and 56 healthy controls living in the southern coastal region (Bodou). In the Glanle region families consume basically a vegetarian and crude palm oil diet, whereas in the Bodou region, families eat a fish-based diet with principally refined palm oil. Fasting blood samples were collected to assess the following parameters: lipid status (plasma total lipids; total-, HDL and LDL-cholesterol; triglycerides; phospholipids; fatty acid composition), plasma protein status (total protein, albumin, transthyretin, orosomucoid, CRP, transferrin), antioxidant capacity (plasma selenium, uric acid, retinol, alpha-tocopherol and tocotrienols levels, plasma seleno-glutathione peroxidase (GSHPx) activity) and oxidative stress markers (malondialdehyde (MDA) and advanced oxidation protein products (AOPP)). RESULTS: The mountain region samples (Glanle) were characterized by significantly lower plasma albumin, total-, HDL- and LDL-cholesterol, retinol and selenium levels, plasma PUFA content and GSHPx activity, but significantly higher alpha-tocopherol index and total tocotrienol level, than controls from the coastal area (Bodou). These results suggest a higher exposure risk to oxidative stress for the mountain region subjects. However, the absence of oxidative damage in this group provides evidence of a selenium independent protection mechanism against oxidative stress. This protection is related to lower plasma LDL cholesterol and PUFA content, and to higher alpha-tocopherol index, delta and total tocotrienols. CONCLUSION: The long-term consumption of crude palm oil could be considered as an effective protective factor against oxidative stress.

Associations of cardiovascular disease and its risk factors with age-related macular degeneration: the POLA study.

The POLA study (June 1995 through July 1997) is a population-based study on cataract and age-related macular degeneration (AMD) and their risk factors in 2584 residents of Séte (South of France), aged 60--95 years. Classification of AMD was performed on 50 degrees fundus photographs, according to an international classification. The presence of cardiovascular disease and its risk factors was determined by interviewer-based questionnaire, clinical examination (anthropometry, blood pressure) and fasting plasma measurements. Using a logistic model adjusted for age and gender, late AMD was not significantly associated with a history of cardiovascular disease, diabetes, use of hypocholesterolemic drugs, hypertension, blood pressure or plasma lipids. Obese subjects (body mass index greater than 30 kg/m( 2)) had a 2.29-fold (95% confidence interval (CI): 1.00--5.23) and 1.54-fold (CI: 1.05--2.26) increased risk of late AMD and pigmentary abnormalities in comparison with lean subjects. Finally, the risk of soft drusen was decreased in those subjects with a history of cardiovascular disease (odds-ratio (OR) = 0.72, CI: 0.54--0.97), and increased with increasing levels of HDL-cholesterol (OR = 1.52, CI: 1.14--2.02). None of these results were modified by further adjustments for smoking, educational level and plasma alpha-tocopherol. These results need to be confirmed by other studies, which ideally should be longitudinal and prospective.

[Cured after an intraocular tumor].

We describe the psychological reactions after diagnosis and treatment of the malignant intraocular tumors: uveal melanoma and retinoblastoma. The chapter on uveal melanoma includes general consideration on the treatment of these tumors, the psychological effects on the patients, the professional and social problems, the follow-up after treatment and the results of recent studies on quality of life. For retinoblastoma we describe the treatments and results with the risk of second cancer, the follow-up of the patients, the psychological problems for parents and children and the specificity of the familial cases.

[Oxidative stress and chronic renal insufficiency: what can be a prophylactic approach?].

Cardiovascular diseases represent the first cause of mortality in chronic renal failure patients treated by hemodialysis. Alterations in lipid metabolism and oxidative stress are recognized as vascular risk factors. Their corrections could be of interest for atherosclerosis prevention. In order to evaluate interest of an therapeutic intervention, we have analyzed oxidative metabolism in hemodialysis patients by determining the production of oxygen reactive species (ROS), the level of defense mechanisms, and the balance between nitric oxide (NO) and ROS, responsible for anti- or proxidant effects of NO. During dialysis sessions performed with cellulosic membrane (Cuprophan) an increase in hydroperoxide production by platelets was noted (12 HETE) (5.62 +/- 0.94 pg); similarly, superoxide anion (O2(0)-) production by monocytes (fluorescence index: 115 +/- 24) and by polynuclear cells (fluorescence index: 115 +/- 24) was enhanced. On the other hand, anti-oxidant defenses were significantly reduced with a decrease in RBC SOC activity (0.92 +/- 0.06 U/mg Hg) and in RBC vitamin E (0.7 +/- 0.07 mg/l) concentration. We have demonstrated a profound alteration in the L-arginine/NO pathway consequently to an accumulation of NO synthases inhibitors or activators. The necessity to reduce the production of ROS during dialysis sessions justifies the use of more biocompatible membranes, such as modified cellulosic or synthetic membranes, decreasing leucocyte activation. In addition, NO synthetase inhibitors can be preferentially eliminated by convection. Finally, a supplementation with an exogenous anti-oxidant, such as oral vitamin E (500 mg/day for 6 months) normalizes RBC vitamin E levels and concomitantly allows a decrease in MDA concentrations In conclusion, oxidative metabolism alterations observed in hemodialysis are multifactorial: preventive measures include the use of a more biocompatible material, the reequilibrium of the NO/ROS balance, and supplementation with exogenous anti-oxidants.

[Quality of life of schizophrenic patients and clozapine].

The authors review the various methods of evaluating quality of life in schizophrenics and note the limitations of that type of method in terms of both form and content. In practical terms, the patients' cognitive disorders may constitute a bias when the questionnaire is addressed. Using a 'subjective quality of life profile' by Gerin et al., the authors interviewed 22 patients presenting with 'recalcitrant schizophrenia' and treated with clozapine for more than 3 years. The patients were asked to assess the change in their quality of life by comparison with previous neuroleptic chemotherapies. For all the items investigated, the responses were in favor of clozapine.

[Pathology of post-adolescence and early adulthood. Role of the parental function].

That study views a developing perspective and touches on the end of the adolescence like a crucial time during that the subject, often remained in a kind of latency from the beginning of his adolescence delay his entry in the adult world. As a result certain particularities of the psychological functioning peculiar to that period and a pathology of 18-25 years considered in the incidence of the failures during the processus of separation-individuation, beginning in the infancy, and has to draw to close. Different situations are considered concerning the role of the parents, and the therapeutic perspectives implicating the families.

[Psychopathologic aspects of tetanic seizures].

In that work, the tetanic's depresses are taken out from the sphere of spasmophilie , concept that the authors take as no pertinent. Those depresses find again the sphere of the nevrosis . It is an answer to the anguish and to the oedipus-problematic of the subject. The authors, by making clear their remarks with several observations, circle the clinic constants escorting the birth of depresses, the bodily expression of the anguish, the psycho-motive peculiarity of the tetanic and the phenomenon of repetition. The authors conclude with a thought over the sense of the symptoms, and over a psycho-somatic medicine that essentially refers to the relation between the doctor and the patient.

[For a clinical approach to impulsion phobias].

Having first recalled the problems involved in classifying impulsion phobias, (problems which have been experienced in the course of all the work done on this subject), the authors report, from a strictly clinical viewpoint, on a series of studies of patients whose problems are very different but who all show the symptoms of impulsion phobia. They next setout a phenomenological approach to the problem, with emphasis on the products of the patient's imagination, his passive fascination with the image of a terrifying act constituting the object of the phobia, the external world in relation to the subject, and the ambiguity of dependance on others which is mirrored by aggressiveness. Lastly, the subject's entire existence is put symbolically at stake, and the phobia is seen to spring from his narcissistic relationship with the external world at a deeper level than that of any oedipian relationships. The first stage in the treatment is, indeed, to build up this narcissistic relationship, and later treatment will be varied according to the patient's fundamental needs.

[Stroop test and schizophrenia].

Utilization of the Stroop test with schizophrenic subjects is reviewed from 32 studies published since 1960. This test, in which subject has to name the color of color-words which are congruent or incongruent, is much used as a selective attention test. Its specific disturbance in schizophrenics would confirm the overload of consciousness with unsignificant non filtered information, probably due to lowering of frontal functions. Stroop effect is not always raised in these patients, especially when general slowing is taken into account. Technical differences modify the test power and can explain contradictory results. The thought disorder symptom seems the most correlated with the results of the test, and particular sensitiveness of paranoïd form has been argued. Neuroleptics do not seem to act on Stroop effect. Factors explaining the great variability of patients in this test are not fully put in light. One hypothesis, already raised, is that the test is linked to a particular attentional constitutional aptitude, and whose relations with the illness are to be explained.
