Small cell carcinoma of the ovary: an immunohistochemical and ultrastructural study with a review of the literature.

This is an immunohistochemical and ultrastructural study of two small cell carcinomas of the ovary with a review of the literature. These cases showed a dimorphic population of small and large cells sharply demarcated from each other. Cytokeratin 18 and vimentin were mainly expressed in the large tumour cells, some of which also stained for alpha-smooth muscle actin. Periodic-acid-Schiff-positive, alpha-1-antitrypsin-positive hyaline globules were present in one case. Ultrastructural findings included filamentous nucleolonema as well as evidence of smooth muscle differentiation. Some of these observations have not been previously reported. Certain of the above features seem to support a germ cell origin of small cell carcinoma, but they cannot be considered specific for germ cell neoplasms. Thus, small cell carcinoma of the ovary cannot be classified into one of the known categories of ovarian tumours at the present time.

Radionuclide blood pool scintigraphy in a child with intestinal arteriovenous malformation (juvenile angiodysplasia). A case report and review of the literature.

Arteriovenous (AV) malformation or angiodysplasia of the gastrointestinal tract is a very rare cause of bleeding in children. These lesions are congenital anomalies and thought to be of hamartomatous origin. We report on a 4-year-old child with an AV malformation of the distal ileum treated by local resection. A brief review of the relevant literature is included. The importance of radionuclide blood pool scanning in the evaluation of these patients is emphasized.

Arteriovenous Malformations

Adenoma of the gallbladder in childhood.

Adenoma of the gallbladder is a rare benign tumor presenting as a sessile or pedunculated lesion with a papillary or nonpapillary structure. Of late, attention has been drawn to its malignant potential; its occurrence has never been reported in children. An 8-year-old girl who was successfully treated by cholecystectomy for a symptomatic adenoma of the gallbladder is reported.

Conjoined hearts.

Thoracopagus twins were delivered at 37 weeks' gestation by caesarean section. Respiratory distress was present and mechanical ventilation was needed; 36 hours after delivery severe lactic acidosis developed and the twins died. The pericardial sac was common and the hearts were conjoined as a single structure with ventricular fusion.

Nearly total absence of pulmonary perfusion with corresponding technetium-99m MDP and gallium-67 uptake in a patient with mediastinal neuroblastoma.

A case of unilateral nearly total hypoperfusion of the left lung in a 13-month-old girl is presented. The combination of the lung hypoperfusion and accumulation of the Tc-99m MDP and Ga-67 citrate in the same area suggested the preoperative diagnosis of mediastinal neuroblastoma. Explorative thoracotomy revealed the presence of a neuroblastoma compressing the left lung pedicle. The described scintigraphic appearance in the pediatric age group is suggested as typical of mediastinal neuroblastoma. This pathology should be included in the following gamuts in nuclear medicine: unilateral decrease or absent lung perfusion, unilateral diffuse chest uptake of Ga-67 citrate, and unilateral pulmonary uptake in bone scintigraphy.

PACP regional quality control program: a general survey.

A regional quality control program operated by the Pennsylvania Association of Clinical Pathologists is described. About 120 laboratories in Pennsylvania and a few neighboring states participated in the program. Two 1000-liter lots of pooled normal and abnormal sera meeting or exceeding the specifications of the National Committee of Clinical Laboratory Standards and the College of American Pathologists were prepared and lyophilized. Participating laboratories determined each constituent in the sera every day for a year, from March 1973 to March 1974. The data was processed by a computer. The inter- and intra-laboratory reports containing the mean, standard deviation, coefficient of variation, Youden plots, etc. for each constituent were made available to all the participating laboratories. Practical applications of these statistical parameters in evaluating the quality of the performance of the laboratories are discussed.
