Treatment of small intestine bacterial overgrowth.

Removal of the predisposing condition, appropriate nutritional support to reintegrate both caloric and vitamin requirements and suppression of the contaminating bacterial flora represent the main of goals of the treatment of small intestine bacterial overgrowth. Generally, the polymicrobic nature of contaminating flora requires the administration of wide-spectrum antibiotics, but as yet we don't know which is the best pharmacological approach, in terms of drug, dosage and duration of therapy. There is no conclusive information regarding the most effective therapy that should be used. This paper reviews the efficacy of the different therapeutic approaches used.

The role of gastric accommodation in the pathophysiology of functional dyspepsia.

BACKGROUND AND OBJECTIVES: The pathophysiology of functional dyspepsia is not yet completely known. Several mechanisms have been suggested as having a role in symptom onset and impaired gastric accommodation to meal is one of the most frequent. This review will focus on the pathophysiological and clinical aspects of this abnormality. EVIDENCE AND INFORMATION SOURCES: Medical peer-reviewed literature was considered to elucidate the role of gastric accommodation in the pathophysiology of functional dyspepsia. STATE OF THE ART: Impaired gastric accommodation to meal is present in around 40% of patients with functional dyspepsia and it was shown to be associated with the presence of early satiety. Unfortunately, a definitive treatment of this abnormality with fundus-relaxing drugs is not yet available. PERSPECTIVES: Further studies are needed to better clarify the role of each single pathophysiological mechanism on clinical manifestations of functional dyspepsia. Research is still ongoing to offer a valid therapeutic approach.

[Congenital intrapericardial diaphragmatic hernia associated with an omphalocele in a newborn infant].

The authors present a case of congenital intraperichardic diaphragmatic hernia and omphalocele, diagnosed on post birth day 11. The patient underwent an emergency procedure with evidenced an abdomino-perichardic hernia with perichardic sca due to the presence of a "breach gap" in the central-anterior region of the diaphragm. This rare malformation could be related to a development anomaly of the septum trasversum.

[Perioperative water-electrolyte therapy in urologic surgery in childhood].

Renal function was evaluated before and after introduction of different intraoperatory methods of hydroelectrolytic re-equilibrium in 172 children who underwent urological surgery. 24 h postoperatory renal function resulted not affected in the treated group, while significantly reduced in untreated patients.

[Circular myotomy in primary anastomosis for the correction of esophageal atresia].

The A. present their experience with the circular myotomy sec. Livaditis in the primary anastomosis of esophageal atresia. Six cases operated on from January 1980 to October 1982 and with a follow-up from 8 to 36 months are analysed. In all the cases there was a distal esophago-tracheal fistula. Gestational age ranged from 36 to 40 weeks and the body weight at birth from kg 1.5 to 3.9. Two patients died for reasons unrelated to the surgical correction of the esophagus (sub-arachnoid hemorrhage, VDS). Both cases at necroscopy revealed a normal esophageal anastomosis. Early an late complications of the remaining 4 cases and long term results are evaluated. In conclusion the A. support the Livaditis esophagomyotomy whenever possible and prefer this technique to the disadvantages of the two-staged esophageal repair.

[The Lester Martin operation in an infant].

A case of Lester Martin repair for total colonic and distal ileal aganglionosis in an infant of 3.4 Kg is reported. The Authors consider and discuss the possibility of application of this operation at a younger age than normally performed considering the satisfactory result obtained in this case.

May there exist specific MRI findings predictive of dementia in multiple sclerosis patients?

Cognitive deficits are present in a substantial number of Multiple Sclerosis (MS) patients, particularly in those with the chronic-progressive type of the disorder. We assessed cognitive decline and its relationship with T2-weighted images on magnetic resonance imaging (MRI). We submitted a group of 26 patients with progressive MS to both MRI and a battery of neuropsychological tests. Cognitive impairment did not correlate with duration of illness or severity of neurological disability, but rather with the presence of extensive periventricular demyelination on MRI, evaluated as area of confluent lesions. These results suggest that cognitive deficits in MS represent a symptom of disease and not a parallel occurrence.
