Defect of human growth hormone receptors in the liver of two patients with Laron-type dwarfism.
Liver microsome pellets prepared from tissue obtained by elective open biopsy from two patients (aged 4 and 26 years) with Laron-type dwarfism (LTD) showed no specific binding of 125I-hGH (human growth hormone). In contrast, 31 assays of liver microsomes obtained immediately after clinical death from 6 healthy subjects (kidney transplantation donors with a mean age of 24 years) showed a mean specific binding of 14% (range 7.9 to 24%). We interpreted these findings as evidence that in patients with LTD there is a defect in the hGH receptors of the liver and a consequent lack of somatomedin generation. The fact that the liver microsomes from the two LTD patients showed active specific binding of 125I-insulin suggest that, in this syndrome, it is only the receptors for hGH that are defective.

Electroenterography after cholecystectomy. The role of high epidural analgesia.
The electrical activity of the stomach and intestine was monitored during the postoperative period in 30 patients who underwent cholecystectomy. All patients received standard general anesthesia with artificial ventilation during operation; some received high epidural analgesia during surgery and postoperatively, and others, fentanyl analgesia during surgery and nicomorphine afterwards. Electroenterography (EEnG) showed that electrical activity decreased following surgery and returned to base line on the third or fourth day after operation. A marked increase in amplitude and frequency of EEnG oscillations was recorded in 80% of the patients who received bupivacaine injections into the epidural space. A decrease was almost always recorded after nicomorphine injections. During the postoperative period, eating caused a considerable increase in the amplitude and frequency of the electrical activity of the stomach and intestine in patients treated by epidural analgesia, whereas no observable change was recorded in patients treated by nicomorphine injections. It appears that high epidural analgesia may be useful in the treatment of postoperative adynamic ileus.

Serum alpha fetal protein in a three year old child with hepatoma.
A 3 year old child with primary hepatocellular carcinoma and high AFP concentrations is described. Following hemihepatectomy, a sharp decrease and return to normal of serum AFP concentrations indicated the completeness of the surgical procedure. Repeat-normal serum AFP concentrations (less than 19 ng/ml), found during a three year follow-up, correlated well with the absence of clinical, laboratory and x-ray evidence of tumor recurrence. The differential diagnosis of abnormal AFP concentrations in childhood is discussed, and the importance of the AFP assay in the follow-up of post-hemihepatectomy patients for the assessment of the completeness of the surgical procedure, the prognosis, and the early detection of tumor recurrence is stressed.
