[Morphologic and morphometric study of the superficial peroneal nerve. Use of material from autopsies and amputations].

A series of 100 human peroneal nerves proceeding from autopsies and amputations were studied by histologic and morphometric techniques with a qualitative and quantitative analysis of each being performed following the morphological criteria of Dick et al (1985). After evaluating the results the authors consider that the peripheral nerve proceeding from autopsies and amputations is useful for basic morphologic study providing it is adequately processed within the first 29 hours post-mortem and post-amputation. Previous studies demonstrating the existence of minimum morphological alterations in normal nerves is corroborated. In order to perform a correct morphological evaluation of the peripheral nerve a mere qualitative analysis is not sufficient but rather statistically evaluated quantitative parameters must be employed.

[Prophylactic sclerosis of esophageal varices. Results of a controlled prospective study].

BACKGROUND: Endoscopic sclerosis of esophageal varices is effective both to treat active hemorrhage and to prevent recurrences. By contrast, its usefulness for the prophylaxis of the first episode of hemorrhage is not well established, the available data being contradictory. METHODS: Overall 46 patients with cirrhosis and esophageal varices of type B or higher, without previous upper gastrointestinal hemorrhage, were randomized to be treated with prophylactic sclerosis or not (n = 22 and 24, respectively). Both groups were comparable in age, sex, etiology of cirrhosis (mainly alcoholic), Child's degree of hepatic failure, coagulation studies, laboratory data and endoscopic findings. RESULTS: In a mean follow up of 16 months, the patients treated with sclerosis had a higher frequency of hemorrhage from gastroesophageal varices (27%) than those in the control group (8%) (n.s.). However, this difference reached significance (p = 0.02) when only patients from Child's classes A and B were considered (31% in the sclerosis group vs 5% in the control group). The mortality rate was similar in both groups, with a probability of one year survival of 80 and 76% in the sclerosis and control groups, respectively. CONCLUSIONS: The results indicate that prophylactic sclerotherapy does not improve short-term survival or reduce the risk of a first hemorrhage due to gastroesophageal varices of type B or higher in hepatic cirrhosis. It may even facilitate the development of this severe complication in Child's A and B class patients.

[Deficiencies of essential microelements in Hispanic-American diets].

The Spanish-American diets are classified in four types, according to the prevailing cereal in each of them. The content of the most important microelements in agreement with analytical data and food composition tables was determined in each type. Regional deficiencies were determined and references are made specially to Peru. Solutions based on modern pharmaceutical resources are proposed.

Activity of cefepime, cefotaxime, ceftazidime, and aztreonam against extended-spectrum-producing isolates of Klebsiella pneumoniae and Escherichia coli from Chilean hospitals.

Resistance of Gram-negative bacilli to third-generation cephalosporins has been increasing due to the production of extended-spectrum beta-lactamases. In this work, the activities of cefepime, cefotaxime, ceftazidime, and aztreonam, alone and in association with clavulanic acid, against isolates of Klebsiella pneumoniae and Escherichia coli are compared. These isolates produce extended-spectrum beta-lactamases as shown by the synergy tests and by the decrease in the MICs of cephalosporins in the presence of clavulanic acid. Cefepime was the most active compound against these microorganisms. In addition, the microorganisms exhibited lower frequencies of resistance to this cephalosporin.

[Comparison of different detection methods for methicillin resistance in Staphylococcus aureus].

The investigation of methicillin resistance in Staphylococcus aureus (MRSA) is a serious problem for the physician and microbiologist. Accurate and rapid detection is essential for the use of appropriate antimicrobial therapy and for the control of nosocomial spread of the resistant strain. The performance characteristics of the MicroScan Overnight Conventional Pos Combo 12 panels (MOCP), BBL Crystal MRSA ID (CR), E-test and agar screen plate (Muller Hinton agar with oxacillin 6 micrograms/ml and 4% NaCl) (AS) were evaluated for the detection of oxacillin resistance. Thirty S. aureus clinically significant strains with different PFGE (Pulse Field Gel Electrophoresis) banding pattern were tested, and 22 of them were mecA positive by PCR. These strains were also analyzed by mecA and Tn554 polymorphism. All mecA positive strains were classified as methicillin resistant by MOCP and E-test. CR and AS failed to detect oxacillin resistance in 2 strains. One false positive was only detected by E-test. Accurate testing for the presence of MRSA may reduce the need for empiric therapy with vancomycin for patients with staphylococcal infections. According to our results the best performance was obtained with MOCP. However, as a rapid method, CR gave acceptable sensitivity for clinical purposes.

Bactericidal activity of ciprofloxacin upon Escherichia coli and Acinetobacter baumanni.

The mechanisms of bactericidal activity of ciprofloxacin (mechanisms A and B) upon cells of a strain of Escherichia coli and one strain of Acinetobacter baumannii were investigated under different conditions. The killing of E. coli cells by ciprofloxacin was significantly reduced by chloramphenicol, but this antibiotic showed almost no activity upon killing of A. baumannii cells by this quinolone. Similar results were obtained when rifampicin was added to ciprofloxacin. Bactericidal activity of ciprofloxacin upon nondividing cells of E. coli was lower and that upon non-dividing cells of A. baumannii was not affected when compared with activity of ciprofloxacin upon dividing cells of both microorganisms. These results demonstrate that the antibacterial activity of ciprofloxacin upon A. baumannii is independent of protein and ARN synthesis, a fact which suggests that this quinolone exerts only bactericidal mechanism B upon A. baumannii. This finding might explain, at least in part, the lower susceptibility of this microorganism to ciprofloxacin.
