We present technical and clinical experiences with a modified scheme of Ingram treatment using a new UV-B phototherapy unit. In addition to application of anthralin, eleven patients suffering from psoriasis vulgaris have been treated by means of this cabinet equipped with 40 UV-B lamps (Sylvania STF 756085 W UV 6). In a bilateral comparative study, we investigated the therapeutic results of standard increase of the UV-B doses and constant low UV-B dose (0.150 J/cm2), respectively. We discuss our results and therapeutic consequences of this therapy.
Two patients are reported who had the clinical picture of localized erythema scarlatiniforme desquamativum recidivans. The lesions were thought to be drug induced in a 56-year-old female patient. In the other 54-year-old male patient, staphylococcal infections may have caused the rash.
In 77 patients with malignant melanoma and 74 control subjects without skin tumors caused by sunlight, the minimal erythema dosage (MED) in the UV-C and UV-B areas, immediate pigment darkening (IPD), and minimal tanning dose (MTD) were determined according to skin type. Direct (MED-UV-B, MED-UV-C) and inverse (IPD, MTD) correlations were established according to skin type. These parameters showed no differences between patients with malignant melanoma and the control group.
In a 34-year-old patient who had been suffering from hydroa vacciniforme since he was 4 years old, typical skin lesions were induced on his arm, back and in the oral mucosa by means of repeated UV-A exposure. His eyes were found to have intracorneal scars.

To investigate ketoconazole penetration into skin, a single oral dose of 200 mg was administered to six healthy subjects. Plasma and cantharides blister fluid (CBF) levels were measured for 11 to 24 hours. In suction blister fluid (SBF) ketoconazole was determined 3, 6, and 9 hours after administration. Mean peak plasma levels of ketoconazole were found at 1 hour, they amounted to 3.03 +/- 0.55 micrograms/ml (n = 5). Apparent half-life of elimination was 1.8 hours. In cantharides blister fluid maximum ketoconazole concentrations were obtained at 6 hours being 1.05 +/- 0.32 micrograms/ml and declined rather slowly (half-life 4.2 hours; n = 3). Maximum concentrations in suction blister fluid were obtained 3 hours after administration (0.914 +/- 0.123 micrograms/ml). After 9 hours ketoconazole levels had declined to 0.216 +/- 0.044 micrograms/ml (n = 3). These concentrations exceed the ketoconazole concentrations proven to be effective in vitro up to 25 fold.

We compared the patient acceptance and efficacy of 60 mg extended-release fenfluramine and placebo before the evening meal in a 10-week, double-blind trial. All 51 participants were 130% to 180% of ideal body weight. They received instruction in diet and behavior modification for 2 wk before the beginning of and during the medication period. Mean weight loss was 5.9 kg (8.0 +/- 4.6% of initial weight) in the fenfluramine group and 3.3 kg (5.5 +/- 3.5%) in the placebo group. Fenfluramine-treated participants reported lower hunger ratings and greater fullness in the target supper-to-bedtime period than participants receiving placebo. Both groups reported dry mouth, dizziness, drowsiness, fatigue, and diarrhea. Although the fenfluramine group reported more complaints, these diminished to less than half after 2 wk of treatment. Four of the fenfluramine and three of the placebo group dropped out for drug-related reasons. In all, 10 fenfluramine and 8 placebo participants dropped out. Fenfluramine participants had a higher benefit score with no difference in risk scores. The fenfluramine group's global evaluation was better than that of the placebo group. Participants viewed the study and the dosing regimen positively but had negative ideas about anorexiants in general. Extended-release fenfluramine taken in the evening was well tolerated and maintained its efficacy as measured by standard and novel techniques.
Immediately after the argon laser impact one finds necrosis of epidermis and superficial dermis. Dilated vessels down to a depth of 0.8 mm are filled with coagulated erythrocytes. After 2 days there is initial restoration of epidermis, and dilated vessels are filled with agglutination thrombi. With and after the 6th day fibroblasts and capillary blood vessels grow into the thrombi, and later on vessels are completely replaced by granulation tissue and after 2-4 weeks by newly formed fibrous tissue.

Infections with Sarcoptes scabiei induced changes of serum immunoglobulin concentrations in 25 of 35 patients (71%). According to the persistence of the symptoms in this disease, the patients were divided into two groups (shorter or longer than 3 weeks). In the initial phase elevated IgE levels were found in 14 of 15 patients. Increased IgE concentrations were demonstrated only in six of 20 patients of the second group. IgA, IgG, and IgM did not reveal significant alterations in the serum concentrations of either group.
Aromatic retinoid Ro 10-9359 1 mg/kg body weight was given to an 11-year-old girl with xeroderma pigmentosum. Therapy resulted in disappearance of actinic keratoses and basal cell carcinomas. The occurrence of new tumors could be prevented in this patient while she participated in this preventive therapy for 7 months. The reduction of the aromatic retinoid to 0.4 mg/kg body weight every other day resulted in the reappearance of four basal cell carcinomas an one keratoacanthoma within 6 weeks.
Acanthoma fissuratum cutis is a common, however not widely recognized cutaneous lesion induced by minor, constant mechanical traumas. Lesions occur typically on the side of the nose or behind the ear, as most cases are caused by poorly-fitting spectacle-frames. Clinical features of the benign reactive tumors, which occasionally may be mistaken for basal cell epitheliomas, are presented.
Trachyonychia (20-nail dystrophy) is described in four patients with clinical, histological, and scanning electron microscopy techniques. Trachyonychia is a polyetiological symptom: psoriasis vulgaris, lichen planus, alopecia areata, atopic dermatitis, 20-nail dystrophy of childhood, and severe nail dystrophy can all lead to trachyonychia-like nail changes. Nail bed biopsies are necessary for proper diagnosis. In our patients, lichen planus, or psoriasis were excluded clinically and histologically. Spongiotic dermatitis of nail matrix and nail bed with column-like parakeratosis within the nail plate are found in trachyonychia due to alopecia areata, atopic dermatitis, and in its idiopathic form.
Kerion Celsi (scalp ringworm) is a highly inflammatory, suppurative fungal infection of the scalp caused by zoophilic dermatophytes transmitted from animals to man. The clinical picture extends to patchy infiltrated suppurative lesions in which hairs are broken or eliminated completely. Diagnosis is performed by detection of the organisms in or around the hairs involved and through culture. Kerion Celsi is treated with griseofulvin orally in combination with local symptomatic and antimycotic therapy. In the beginning systemic glucocorticoids might possibly be of help reducing the risk of scar formation and irreversible alopecia.

Patients with onychomycosis (tinea) of finger- and toenails underwent combined treatment: avulsion of nail plate, local antimycotic and systemic griseofulvin treatment. Avulsion of the involved nails was performed by the patients themselves chemo-mechanically using 50% KI-ointment. 1,500 nails were successfully removed. Out of 250 patients 101 were clinically and mycologically cured after a median delay of 6.5 months. Only 50% of the 101 patients remained cured up to one year after cessation of therapy. Atraumatic nail avulsion in combination with chemotherapy is a reliable, inexpensive, painless, and effective alternative method to surgical excision treating onychomycosis.
