[Surgical treatment of different forms of renovascular hypertension].

24 patients with renovascular hypertension were operated from 1995 to 1997. Patients were divided into three groups: group I-atherosclerotic stricture of renal arteries with or without aorta stenosis (13), group II-fibromuscular dysplasia (8), group III-stricture of vascular anastomosis after renal transplantation (5). All patients were disqualified from PTA. Operations of reanastomosis of renal arteries to aorta or prosthesis and TEA dominated in group I, the operation of choice in groups II and III was plastic procedure with patch or venous graft. Authors conclude that surgical treatment of renovascular hypertension is an efficient method of treatment in case of disqualification from PTA and the way of reconstruction should be dependent on the reason of renovascular hypertension.

Lymphoblastic lymphoma in a female with chronic granulocytic leukaemia.

In a 18 years old, Ph1 negative woman a lymphoblastic lymphoma developed after 16 months of treatment for juvenile form of CGL. In the course of the disease we observed twice the myeloblastic transformation and once the low differentiated transformation of CGL. Based on available methods we are not in a position to exclude a particular form of lymphoblastic transformation of CGL involving lymph nodes without leukaemic blood picture.

Fractional administration of adriblastin and modified route of ara-C administration for the treatment of acute leukaemia.

Two groups of AML patients (n1 = 63, n2 = 20) and two groups of ALL patients (n1 = 33, n2 = 15) were treated using polychemotherapy protocols which in each leukaemia subtype differed mainly in adriblastin administration being either in bolus form (30 mg/m2/day i.v.) or fractional form at the beginning 20 mg i.v., then 6 mg/m2 every 6 h. The fractional method of administration was elaborated on experimental data indicating the superiority of continuous infusion of anthracyclines. In AML additional ara-C was given in continuous infusion only on 1 to 3 days, on 4 to 8 days duplicated dose was administered subcutaneously (i.e. 100 mg/m2 every 12 h). In patients given fractional doses of adriblastin and in AML also ara-C in the modified way the statistical analysis revealed a higher CR (ALL - 67%/93%, AML - 46%/60%) and CR + PR rates, a lower rate of infections as the cause of death in the AML group, lower rates of nausea and vomiting as well a lower increase of infections in the course of the induction treatment in the AML group. Another advantage was a lower total dose of adriblastin for remission induction treatment as well as an elevated cumulative dose which allows anthracyclines to be longer used. The efficacy of the modified ara-C administration confirms our earlier observation.

Diagnostic and prognostic value of the terminal deoxynucleotidyl transferase (TdT) activity in treating the blastic phase (bp) of chronic granulocytic leukaemia (CGL).

The blastic cell phenotypes of 26 cases of CGL in blastic phase were estimated and the patients were treated with different schemes. The following methods of typisation of the blast cells were used: cytochemical stainings (POX, Sudan B, PAS, nonspecific esterase), estimation of TdT activity, and in 11 patients the testing with monoclonal antibodies of VI series. Using these methods 10 patients (38%) with lymphoid form of the blastic phase, 11 (43%) with the myeloid type and 5 patients (19%) with undifferentiated type were diagnosed. In the group of lymphoblastic type a longer survival time and complete remissions were observed. High TdT activity in blastic cells did correspond with favourable response to Vincristin and Prednison. The introduction of TdT assessment into the diagnosis of CGL allows the cells to be classified more precisely, thus helping in defining the prognosis and in the choice of treatment programme.

Cooperative randomized studies on the treatment of adult acute leukemia in Poland. A comparison of two remission induction regimes and two maintenance regimes for AML.

In a prospective study 100 AML cases were randomized in two groups and underwent remission induction treatment in 4 departments with "Ara-C c. inf. from the first to the eight day plus Adr. from the first to the third day" or "Ara-C + 6-TG + Adr., 6 + 6 + 2 days". The CR patients were divided into two subgroups. Subgroup A obtained maintenance therapy with 5 day course of Ara-C s.c. in addition to 6-TG, Vincristine plus Prednisone and Adriamycin sequentially, with each cycle repeated at 6 week intervals. Subgroup B obtained the calf thymus extract (TFX-Polfa) between the cycles additionally. The current results of remission induction treatment are similar to "8 + 3" and "6 + 6" regimens as measured by the CR rate (45% and 44%), survival curves (survival after 24 months - 23% and 28% respectively) and relapse rates (3.5%/pt.mo.obs. and 5.3%/pt.mo.obs.). It is still too early to evaluate the efficacy of this maintenance treatment, because the programme is still open to patient entries. At present 13 patients received maintenance regimen A and 12 patients received maintenance regimen B, with an average survival being equal to 14 and 13.5 months respectively.

Application of series of monoclonal antibodies against human white cell differentiation antigens and the common ALL antigen in the characterization of leukemia cells.

The specificity and the clinical usefulness of the hybridoma derived monoclonal antibodies raised against the differentiation antigens of granulocytes (VIM-D5, VIM-C6), monocytes (VIM-D2), B lymphocytes (VIB-C5, VIC-Y1), erythrocytes (VIE-G4) and CALLA (VIL-A1) was studied in leukemic cells isolated from peripheral blood and bone marrow of 41 adults with acute leukemia by using indirect fluorescence method. The VIL-A1 positivity was observed in 4/9 of ALL and in none of myeloid leukemia. It was accompanied in 3/4 of cases by VIB-C5 positivity and in one case by VIE-G4 positivity. It is important that 2 out 3 unclassifiable cases (PAS-) could be diagnosed as common ALL due to their VIL-A1 positivity. VIM-D5 like VIM-C6 reacted specifially with granulocytic cells only and gave positive results in 20/30 of acute myeloid leukemias. When classified according to the FAB scheme, the proportion of VIM-D5 + patients rose from M1 toward more mature subtypes, including M4/5. It allowed to identify as AML 2/6 of unclassifiable-Mo leukemias, VIC-Y1 appeared to be helpful in characterization of B cell malignancies and of myelomonocytic leukemias. It is concluded that the monoclonal antibodies of VI series are specific and allow a more precise definition of leukemia, thus helping in optimalization of treatment.

Parallel examination of receptors of malignant lymphoma cells using combined tests with mouse erythrocytes and monoclonal VIB-C5 antibody or polyvalent serum against human immunoglobulins.

This report presents the results of studies on two membrane markers of lymphocytes isolated from patients with non-Hodgkin's lymphoma. The combined methods were applied allowing for a simultaneous testing of: a) presence of the membrane receptor for mouse erythrocytes (Em) and membrane immunoglobulins (SmIg) or b) presence of the receptor for mouse erythrocytes and antigen binding monoclonal VIB-C5 antibody (VIB). In 12 patients with the low grade malignant lymphoma, up to 50% of lymphocytes possessed both receptor for mouse erythrocytes as well as membrane immunoglobulins and higher frequency of occurrence of Em+ cells were associated mainly with a high percentage of SmIg-- cells. In 9 patients with low grade malignant lymphoma, 24% of cells were observed to possess Em receptor and antigen detectable with VIB antibody, 34-70% of cells were Em-VIB-lymphocytes and 13-54% were Em-VIB+ cells. The results obtained have revealed that the application of monoclonal antibody renders possible the diagnosis of lymphoma and may be of particular value in the cases with weak expression of receptor for mouse erythrocytes.

Seven day cytosine arabinoside administered subcutaneously plus daunorubicin for remission induction in AML.

51 patients (mean age 40, range 17-71) with previously untreated AML (25% of M4-6 FAB type) were given Ara-C subcutaneously 100 mg/m2 every 12 h for 7 days and DNR 45 mg/m2 on days 1-3 i.v. for inducing remission. After 52 days 49% of patients on an average reached CR. The complete responders were given a 5 day treatment pulses monthly until relapse, which consisted of Ara-C s.c. plus one of the following drugs added in cyclic rotation: 6-TG, cyclophosphamide, CCNU or DNR. After 2 years the intervals between cycles were prolonged step by step. The mean/median of CR duration equals 26/19 months and of the survival of complete responders 32.9/27 months. The relapse rate and the CNS relapse rate equaled 1.9% and 0.36% per patient/month of observation. T and B cell depletion was observed during maintenance. The results indicate that the subcutaneous route of administering Ara-C is effective during the induction of remission.

[Evaluation of the myelogram carried out on the 6th day of remission- inducing treatment of acute nonlymphoblastic leukemia in adults as an indicator of therapeutic effectiveness].

Aspiration biopsy in 31 patients with AML was performed prior to and in the 6th day of induction therapy. Studied group included 17 women and 14 men of 40 yrs mean age. Blastosis prior to and on the 6th day of the observation were compared and the index of blastosis reduction counted. Median value of blastosis reduction was 29.5%, 40% of the patients with the index below the median value showed complete remission, while the patients with the indexes above the median value obtained significantly higher number of complete remission (CR = 81%, p less than 0.05). Statistically significant reduction of leukaemic infiltration in examined myelograms was as s rule the sign of oncoming favourable therapeutic result. In cases of nonaplastic marrow with persistent blastosis (blastosis reduction 29.5%) one should consider a change of the therapeutic regimen.
