[Role of corticoid-antibiotic combinations in local treatments. Critical study].

Current literature about steroid antibiotic combinations has been critically reviewed. The use of such combinations may be considered in infected steroid sensitive dermatosis. Definition of super-infection is however impossible on clinical data only. A single study demonstrated the superiority of the combination upon the steroid alone in well defined experimental conditions. This superiority is not demonstrated beyond 8 days of treatment or in case of pretreatment with local antiseptic solutions or baths. Theoretical side effects of local antibiotics include, sensitization, selection of antibiotic resistant germs, systemic toxicity. They are not troublesome in current practice. However adjunctions of an antibiotic can diminish the steroid efficacy when infection is not present. The real usefulness of the combination in current practice is still to be demonstrated.

[Sarcoid nodules of the skin at the site of venipuncture (author's transl)].

In a 56-year-old female patient with sarcoidosis, venipuncture for withdrawal of blood specimens resulted in the formation of a histologically confirmed sarcoid granuloma at the precise site of the puncture. Eight similar cases are reported in the literature. They concern women aged 37 on average and presenting with pulmonary lesions. The sarcoid nodules, large or small, occurred earlier at the sites of venipuncture than on visible scars. One may wonder whether the formation of these nodules could be due to a hypersensitivity reaction developing as a result of blood cells extravasation during an active phase of the disease.

[Two cases of superficial pemphigus induced by pyritinol (author's transl)].

Pyritinol is presently prescribed in rhumatoid polyarthritis because of its structural analogy to D-penicillamin. Only benign cutaneous side effects had been mentioned up to now. We report two cases of pemphigus occurring during pyritinol therapy given in one case for rhumatoid polyarthritis, in the other case for memory deficiency. Clinical aspect was that of a superficial pemphigus. The histologic picture was in some instances that of a superficial acantholysis, and in other instances, that of an eosinophilic spongiosis. Definite proof of pemphigus was shown by presence of anti-intercellular substance antibodies in the serum and in the skin of both patients.
