Using cost accounting data to improve clinical value.

Value in health care can be defined as the provision of appropriate care at appropriate cost. While appropriate care is the province of clinical providers, cost data usually is maintained by financial managers. Collaboration between clinical providers and financial managers therefore is essential if optimum value is to be created. To this end, cost data are frequently given to providers for use in clinical care improvement projects. One such project has been implemented successfully at Virginia Mason Medical Center, Seattle, Washington.

Gastric rupture: an unusual complication of the esophageal obturator airway.

The esophageal obturator airway (EOA) has been used in the Jerusalem area by the ambulance services approximately 1,000 times in the past six years. We report an unusual complication of the EOA in the case of a 50-year-old man rescued from drowning who received both basic and advanced cardiopulmonary resuscitation. He was ventilated by an EOA attached to an Ambu bag; however, resuscitation attempts were unsuccessful. Autopsy revealed gastric rupture.

The preplacement medical evaluation of hospital personnel.

Preemployment physical examinations necessarily focus on problems bearing on an individual's ability to safely perform in his work setting. A review of 3,599 examinations of prospective hospital workers reveals that the most frequent observations of significance involved susceptibility to communicable disease--specifically, inadequate diphtheria immunization in 52% and inadequate rubella antibodies in 16% of those tested--and the potential for disease transmission, including tuberculin reactivity in 14% and intestinal protozoa in 13 prospective food handlers. Other findings included 14 individuals with positive serological tests for syphilis, but only two of whom required anti-luetic therapy. An additional 14 were found to be hepatitis B surface antigen carriers. Only 2.3% of those examined were found to exhibit diastolic hypertension at the time of their examination, possibly because of the relative youth of the group studied.
