[Assessment of myocardial function and metabolism in patients during aortocoronary bypass surgery after hypoxia-reoxygenation].

The myocardial function and metabolism were assessed in 12 patients with coronary heart disease intraoperatively during aortocoronary bypass surgery. The antioxidative properties of the blood of the heart were found to become lower along with activated peroxidation after the major stage of an arrested heart operation, that correlated with depressed myocardial function.

[First clinical test of the use of artificial blood circulation with auto-oxygenation].

Thirteen operations for ischemic heart disease were performed in the All-Union Surgery Centre of the AMS USSR using the method of simultaneous left and right heart bypass with oxygenation of the blood in the patient's lungs. This method was found to provide for a better gas exchange in artificial lung ventilation (ALV) with 30 per cent oxygen in the inhaled mixture, and for the reduction in the perfusion blood damage. The biventricular bypass is more complicated technically than the conventional artificial circulation (AC), but it is believed to be more physiological and cost-effective, which allows for considering it an alternative in aortocoronary bypass surgery.

[New parameters of the oxygen transport function of arterial blood].

New criteria of the arterial blood oxygen transport function suggested by O. Siggaard-Andersen et al. are reviewed. The examples from clinical practice show the possibility to evaluate the capacity of the arterial blood to transport O2 during general anesthesia and intensive care.

Heart Transplantation↗

[The status of hemostasis in patients undergoing surgery with artificial circulation].

Hemostasis has been assessed in 200 patients during and after cardiopulmonary bypass surgery depending on different protamine sulphate (PS) doses injected for heparin neutralization. It has been demonstrated that the use of 1:1 PS to heparin ratio leads to adequate heparin neutralization and does not cause the appearance of residual heparin in the blood. PS administration at a dose not exceeding that of heparin was accompanied by an intraoperative decrease in platelet count and its more rapid recovery in the early postoperative period.

Cardiac Surgical Procedures↗

[Initial experience of work on a program of complete preservation of the patient's blood in heart surgery].

A complex program on complete preservation of a patient's blood was adopted during and after cardiac surgery with extracorporeal circulation. The major principles of such a "hemotransfusionless" surgery are 1) to minimize irrevocable blood loss; 2) to fully return a cardioplegic solution into the blood during extracorporeal circulation; 3) to apply autohemotransfusion controlled by hemodilution and hemoconcentration; and 4) to reinfuse drained blood in the nearest postoperative period. One hundred and twelve patients were treated by the program. The average expenditure of donor blood per patient was 543 +/- 343 ml, donor blood was not transfused to 44 (39%) patients during their entire stay in hospital.

Blood Preservation↗

[Activated coagulation time and the concentration of heparin in assessing the level of blood heparinization during operations using artificial blood circulation].

Blood heparinization was assessed, using two techniques (activated coagulation time--ACT and heparin concentration) in 31 patients during cardiopulmonary bypass surgery. It has been shown that ACT-technique, not always revealing heparin concentration, may serve as a criterion for blood heparinization adequacy during cardiopulmonary bypass, as in every case it reflects individual changes in anticoagulant blood activity.
