Non-achievement of clinical targets in patients with type 2 diabetes mellitus.

The study was conducted to determine whether the clinical targets for the control of diabetes recommended by American Diabetes Association can be met in the context of routine diabetes practice. This cross-sectional study was undertaken on 211 type 2 diabetic patients at the Outpatients Diabetes Clinic, Hospital Universisti Sains Malaysia (HUSM) Kubang Kerian, Kelantan between the year 2001-2002. Patients' physical examination and their medical history as well as their family history were obtained by administering a structured questionnaire. Samples of patients' venous blood during fasting were taken and analysed for plasma glucose, glycated haemoglobin and lipid profile. Analysis showed that many patients had comorbidities or complications. A large number of them had poor glycaemic control (73%). Systolic and diastolic blood pressures of 75% and 85% subjects were > or = 130 and > or = 80 mmHg, respectively. Body Mass Index (BMI) values of 66% of the patients were outside the clinical target (BMI > or = 25 in male and > or = 24 kg/m2 in female). The lipid profile showed that 96% of the patients had at least one lipid value outside the clinical target level. In this study, 70% of the patients had total cholesterol > or = 5.2 mmol/L, 87% had LDL cholesterol > or = 2.6 mmol/L, 57% had HDL cholesterol less than the normal range, < or = 1.15 mmol/L in men and < or = 1.4 mmol/L in women, while 46% had triglycerides > or = 1.71 mmol/L. Complications of diabetes were observed in 48% of the total number of patients. As for the patients' systolic blood pressure, age and duration of diabetes were found to have significant effects. Older subjects with a longer duration of diabetes were more hypertensive. Variables that had significant effects on BMI were age, duration of diabetes, glycaemic control and gender. Younger females and newly diagnosed subjects with better glycaemic control (A1C < 7%) were found to have higher BMI values. The overall clinical targets were suboptimal. The prevalence of hyperlipidaemia and hypertension was high. It is imperative that better treatment strategies and methods be adopted to enhance diabetes control and reduce long-term complications of the disease.

Expanding the phenotypic spectrum of the Baller-Gerold syndrome.

We report on two sisters off-spring of healthy consanguineous parents, where their major clinical features absent thumb, radial aplasia and craniosynostosis led to a diagnosis of Baller-Gerold syndrome BGS (OMIM:218600). Syndromes with associated preaxial reduction defects mainly Fanconi pancytopenia, VATER association, Rothmund-Thompson and Roberts phocomelia syndrome were excluded by proper clinical and cytogenetic studies. In addition to craniosynostosis and radial deficiency, our studied cases had absent or hypoplastic thumbs, postaxial polydactyly in the left foot, genital anomalies and orodental manifestations. Review of the literature depicted phenotypic variability of BGS. The presence of affected sibs the offspring of consanguineous parents confirms autosomal recessive inheritance. The observation of associated postaxial polydactyly, blue sclera, rotatory nystagmus, other skeletal and orodental anomalies broadened the spectrum of phenotypic variability. Awareness of the expanded phenotypic spectrum will improve the diagnosis and genetic counseling of BGS.

Voltammetric determination of nitrosocimetidine in simulated gastric juice.

OBJECTIVE: To develop a voltammetric method for the determination of nitrosocimetidine in simulated gastric juice (SGJ). METHOD: Measurement of the polarographic wave of the nitroso derivative at the dropping mercury electrode in 0.1 m HCl. RESULTS: A well-defined diffusion-controlled cathodic wave was obtained at -0.34 V vs. Ag/AgCl electrode. The current-concentration relationship was found to be rectilinear over the range 10-80 microg/mL and 2-50 microg/mL with percentage recovery of 101.24 +/- 0.79 and 100.79 +/- 0.56 for the direct current (DCt) and differential pulse polarography (DPP) modes, respectively, with a minimum detectability (S/N=2) of 0.16 microg/ml (5.6 x 10-7 m) using DPP mode. CONCLUSION: The proposed method was successfully applied to study the nitrosation of cimetidine in SGJ.

LC determination of dinitrosopiperazine in simulated gastric juice.

A simple and specific reversed phase HPLC method for the determination of dinitrosopiperazine in simulated gastric juice using UV detection was reported. The chromatographic resolution of the analyte and the internal standard isosorbide dinitrate was performed without extraction from the gastric juice on a reversed phase ODS column. Isocratic elution was carried out with methanol-0.02 M sodium dihydrogen phosphate (60:40 v/v, pH 3.0) at a flow rate of 1.0 ml min(-1) with UV detection at 238 nm. The calibration graph was linear over the concentration range 0.072-2.88 microg ml(-1) of dinitrosopiperazine with minimum detectability (S/N=2) of 0.01 microg ml(-1) (8 x 10(-8) M). Inter-day and intra-day precisions calculated as % RSD were in the range 0.32-0.38% and 0.19-0.25% respectively. Inter-day and intra-day accuracies calculated as % error were in the range 0.18-0.21 and 0.08-0.11% respectively. The proposed method was successfully applied to the study of the possible in-vivo production of DNPZ under the standard nitrosation conditions recommended by WHO.

Norms for experiencing emotions in different cultures: inter- and intranational differences.

Within- and between-nations differences in norms for experiencing emotions were analyzed in a cross-cultural study with 1,846 respondents from 2 individualistic (United States, Australia) and 2 collectivistic (China, Taiwan) countries. A multigroup latent class analysis revealed that there were both universal and culture-specific types of norms for experiencing emotions. Moreover, strong intranational variability in norms for affect could be detected, particularly for collectivistic nations. Unexpectedly, individualistic nations were most uniform in norms, particularly with regard to pleasant affect. Individualistic and collectivistic nations differed most strongly in norms for self-reflective emotions (e.g., pride and guilt). Norms for emotions were related to emotional experiences within nations. Furthermore, there were strong national differences in reported emotional experiences, even when norms were held constant.

Polarographic determination of some penicillins through nitrosation.

Direct current and differential pulse polarography DPP were used for the determination of three penicillins, namely, ampicillin, benzylpenicillin and carbenicillin, in pure form and in their dosage forms. The method is based upon treatment of penicillins with nitrous acid followed by polarographic measurement of the produced derivatives polarographically. The nitroso derivatives formed exhibited reduction waves over the whole pH range in Britton-Robinson buffers. The waves were characterized as being diffusion-controlled and free from adsorption phenomena. The current-concentration plots were rectilinear over the concentration range 8-200 and 2-160 micrograms ml-1 for DCt and DPP, respectively, for all the studied compounds. The proposed method was further applied to determine penicillins in pharmaceutical preparations, and the results obtained were in good agreement with those given by the companies.

Cortisol and behavior: 2. Application of a latent state-trait model to salivary cortisol.

In two studies we applied a new stochastic model for the measurement of latent states and traits to salivary cortisol in healthy subjects. Data were analyzed by Linear Structural Relations (LISREL). In the first study, 48 students collected saliva samples on two occasions at 1300h with an interval of six weeks between occasions. Salivary cortisol levels were found to be mainly influenced by situational and/or interactional effects, which determined approximately 75% of the total variance of the hormone data, while only approximately 21% were determined by the latent trait. Measurement unreliability was found to be approximately 4%. In the second study, 54 young mothers collected saliva at 0800h, 1500h, and 2000h for three consecutive days, starting on the second day after delivery of a healthy baby. Early morning salivary cortisol showed high consistency across the three days. Approximately 60% of the total variance in the early morning cortisol samples was determined by the latent trait. For the 1500h and 2000h samples, however, the corresponding proportions of variances amounted only to 33% and 28%, respectively. These results appear to reflect the absence of a clear-cut circadian rhythm in some of the women on the second day after delivery.

New renin inhibitors homologous with pepstatin.

Four homologues of pepstatin, the potent but poorly soluble inhibitor of aspartic proteinases, were synthesized by coupling to the C-terminus of the natural pentapeptide the following amino acid residues: L-arginine methyl ester, L-aspartic acid, L-glutamic acid and the dipeptide L-aspartyl-L-arginine. The peptide-coupling reagent we used, benzotriazolyloxytris(dimethylamino)phosphonium hexafluorophosphate, allowed us to obtain readily pure pepstatin homologues with high yields (60-83%). Pepstatylarginine methyl ester and pepstatylglutamic acid were about one order of magnitude more water-soluble than pepstatin. The four homologues and pepstatin were tested in vitro as inhibitors for highly purified pig and human renins acting on the N-acetyltetradecapeptide substrate. The 50% inhibitory concentrations (IC50) of the homologues were ranged from 0.01 to 1 microM against porcine renin at pH 6.0 (pepstatin IC50 approximately 0.32 microM) and from 5.8 to 41 microM against human renin at pH 6.5 (pepstatin IC 50 approximately 17 microM). By three different graphical methods we showed that pepstatin and the four homologues behaved as competitive inhibitors for porcine renin. The most potent inhibitors were pepstatylaspartic acid and pepstatylglutamic acid, with inhibitory constants respectively 2- and 10-fold smaller than that of pepstatin. By coupling glutamic acid to pepstatin, the ratio solubility/Ki was increased by two orders of magnitude.
