Ocular findings in 55 patients with Down's syndrome.

Fifty-five patients with Down's syndrome were examined to evaluate the characteristics and frequency of ocular findings. Of these patients, 29 (52.7%) were hypermetropes, 7 (12.7%) were emmetropes, and 7 (12.7%) were myopes; astigmatism of more than 3.00 diopters was present in 7(12.7%) patients. Strabismus was observed in 12 (21.8%) patients. All but one of these 12 patients also had esotropia. Congenital nasolacrimal duct obstruction was present in 12 subjects (21.8%). Blepharitis was found in 19 (34.5%) cases. Nystagmus occurred in 7 (12.7%) patients. Brushfield spots were detected in only 20 (36.3%) of the patients and were more common in light-colored irides. Lens opacities were diagnosed in 11 (20%) patients, and two underwent successful cataract surgery. On fundus examination, an increased number of retinal vessels crossing the optic nerve head was detected in 21 (38.1%) cases. One of the patients had a retinal detachment. Significant but correctable ocular problems are present in patients with Down's syndrome and may interfere with the quality of life of the patient and with binocular vision. Surgical intervention may be needed for strabismus and for cataracts.

Somatostatin receptors in the orbits.

In-111 pentetreotide has been recommended for the detection of somatostatin receptor (SSR) positive tumors. Sites with increased numbers of activated lymphocytes may also show In-111 pentetreotide with Graves' ophthalmopathy (GO) compared to various other orbital disorders. Five patients with GO, three patients with necrotizing scleritis, and two patients with orbital pseudotumor were imaged at 4 hours, 24 hours, and 48 hours after injection of In-111 pentetreotide. Although planar images did not show considerable uptake, SPECT slices showed marked and persistent uptake in the orbits unilaterally in one patient, and bilaterally in three patients with GO. There was no significant correlation between the In-111 pentetreotide activity and the degree of opthalmopathy determined by Werner's classification. Pseudotumors also showed positive uptake. Early uptake was present in one patient with necrotizing scleritis because of Wegener's granulomatosis, and was completely diminished at 24 hours. Octreotide treatment in three patients with GO did not cause any clinical improvement. Although the presence of SSR, either on the infiltrated lymphocytes or on fibroblasts, implies that octreotide treatment for GO can be considered, the results are not encouraging. Nonspecific binding might be present when uptake patterns of orbits and persistence of activity during octreotide treatment are considered.

Thallium-201 uptake in the orbits.

Thallium-201 uptake patterns in the orbits are not widely known. Diffuse, bilateral orbital localization of TI-201 chloride may be an incidental finding because of orbital muscle or other soft tissue uptake. Differences in orbital blood flow and soft tissue mass may be the cause of the varying uptake intensities in most cases. Focal uptakes, even when bilateral, may indicate specific pathology and should be investigated. Sample cases are presented.

Congenital muscular dystrophy with cerebral involvement--report of a case of "occidental type cerebromuscular dystrophy"?

Cerebral CT scan abnormalities have been seen to be afflicted with some cases of classic occidental type congenital muscular dystrophy (CMD) with normal or borderline intelligence without neurological abnormality. A case is presented with early hypotonia, joint contractures, muscle biopsy features of CMD, normal intelligence and diffuse white matter hyperlucency on CT scan. Every CMD case should be screened with cerebral CT and magnetic resonance (MRI) scans to reach more aspects of this heterogenous disorder.

[Salmonella tester strain TA104 for the detection of mutagenic and carcinogenic chemicals in our environment].

We used the Salmonella mutagenicity test for detecting chemical carcinogens as mutagens in the Salmonella typhimurium tester strain TA104 . The mutagenicity of several compounds was assessed by induction of histidine revertants in the TA104 . In each experiment we routinely included positive mutagenesis controls using three different concentrations of known mutagens. The mutagenic chemicals such as sodium azide, hydrogen peroxide and hydroxylamine were found to be mutagenic to TA104 at very low concentration (10(-4) mg/ml). Their mutagenic activity decreased while their concentrations were increased. The effect of acridine orange, 2, 4, 6-trinitrobenzene sulphonic acid, 2- phenylnaphthalene and 20- methylcholanthrene were also found to be mutagenic to TA104 at the concentration of 10(-2) mg/ml. The mutagenicity of other materials such as hair dyes, meat- broth preparations+ and cigarette smoke condensates were also tested, and all of them were found to be mutagenic to TA104 . The highest mutagenic activities were observed at the concentration of 10 mg/ml for two different hair dyes and of 1 mg/ml for cigarette smoke condensates.

Choroidal folds associated with a sellar mass.

We report a case with progressive visual loss whose ophthalmic examination revealed bilateral optic atrophy and choroidal folds in the left eye. Neuroimaging demonstrated a sellar mass compressing the optic chiasm and the left optic nerve was to be stretched. We assume the traction created on the optic nerve by the mass stretched the optic nerve and caused the choroidal folds.

A preliminary report of thyroglobulin release after fine needle aspiration biopsy of thyroid nodules.

To demonstrate whether or not fine needle aspiration biopsy caused release of thyroglobulin into the circulation and if there was a correlation between histological diagnosis and thyroglobulin release from thyroid during aspiration, serum thyroglobulin concentrations were determined in 12 patients with solitary cold nodule or multinodular goitre. Of the 12 patients studied, seven had significant (P less than 0.05) elevations of serum thyroglobulin after aspiration. It is suggested that circulating thyroglobulin may increase after minor trauma to thyroid tissue such as following fine needle aspiration.

Long-term results of various anterior capsulotomies and radial tears on intraocular lens centration.

BACKGROUND AND OBJECTIVE: To evaluate the long-term effects of various anterior capsulotomies and radial tears on intraocular lens (IOL) centration. MATERIALS AND METHODS: Ninety-five eyes of 87 senile cataract patients operated with extracapsular technique were evaluated in IOL tilt and decentration with a new method of measurement. According to the type of anterior capsulotomy and number of radial tears, five groups were constituted as, can opener, envelope, continous curvilinear capsulorrhexis (CCC), CCC with one radial tear (relaxing incision at quadrant 12), and CCC with two relaxing incisions (relaxing incisions at quadrants 6 and 12), respectively. RESULTS: Early decentration and tilt in groups CCC and CCC with one radial tear were significantly lower than the other groups (P < 0.05). Late decentration and tilt in the CCC group were significantly lower than the other groups (P < 0.01). Highest values of tilt and decentration were determined in envelope capsulotomy. Additional symmetric relaxing incision at quadrant 6 revealed no effect on the prevention of decentration and tilt compared to one relaxing incision. CONCLUSION: CCC with one radial tear is not ideal but sufficient for IOL centration. All other anterior capsulotomy techniques, other than intact CCC, do not guarantee the IOL centration.

Evoked potentials in Guillain-Barré syndrome.

A prospective evaluation was made in a total of 19 patients with Guillain-Barré syndrome (GBS) by neurologic examination, visual evoked potentials (VEP), brain-stem auditory evoked potentials (BAEP) and somatosensory evoked potentials (SEP) during the acute phase of the disease, three weeks later, and then again three months after the onset of symptoms. The results of electrophysiologic studies of the patients were compared with those of 19 healthy children. There were no statistically significant differences found in the mean VEP and BAEP values between the patient and control groups. However, the VEP and BAEP values were abnormal during the first and second stages of the disease in only a few patients. There was a statistically significant prolongation of the response in tibial and median SEP during the first and second stages of the disease when compared with the third stage and the control group. The Erb-Cervical 7 (C7) interwave latency was elevated in the median SEP. It was concluded that the prolongation of the response in SEP was diagnostically important in the acute phase of GBS. There was no correlation found between SEP and clinical progression.
