[Causes of fatal outcome in acute appendicitis].

For the last 10 years, 97,542 patients were operated on for acute appendicitis, 132 (0.14%) died. Complications of purulent infection (peritonitis, intraabdominal abscesses) and its sequelae (acute commissural ileus, intestinal fistulas) conditioned by late hospitalization were the main cause of lethal outcomes.

[Means of reducing mortality in acute appendicitis].

The results of operative interventions conducted for acute appendicitis in 81,656 patients are analysed. Postoperative mortality reduced from 0.2 to 0.08% in the last decade. Peritonitis and intraabdominal abscesses accounted for 60.9% of fatal outcomes, thrombosis of the pulmonary artery--for 7.8%, intestinal fistulas--for 2.6%, and hemorrhages--for 1.7%. Improved training of doctors of emergency ambulance service, district medical officers, pediatricians, and specialists in infectious diseases in the diagnosis of acute surgical diseases is a reliable reserve in reducing the frequency of diagnostic errors in patients with acute appendicitis.

[Treatment of suppurative puerperal mastitis].

The treatment of 122 patients with acute puerperal mastitis consisted in wide opening of a purulent focus, excision of the necrotized tissues and rational drainage of the purulent cavities. After the operation, the antibiotics, intravenous infusion, drugs enhancing the specific and non-specific resistance of an organism were used. Average time of treating in hospital was 11 days, in infiltrative form of mastitis--6.5 days.

[Diagnosis and surgical treatment of acute gastrointestinal hemorrhage].

Of 596 patients with acute gastrointestinal bleeding, it developed from the gastric and duodenal, and peptic anastomotic ulcer in 382 (64.1%). The authors consider, that at every regional centre, it is expedient to hospitalize the patients with gastrointestinal bleeding in a single surgical unit, equipped with the endoscopic apparatus and all the necessary facilities for giving emergency surgical aid.
