Severe genital herpes infections in HIV-infected individuals with impaired herpes simplex virus-specific CD8+ cytotoxic T lymphocyte responses.

The specific mechanisms underlying the varied susceptibility of HIV-infected (HIV+) individuals to opportunistic infections (OI) are still incompletely understood. One hypothesis is that quantitative differences in specific T cell responses to a colonizing organism determine the development of an AIDS-defining OI. We evaluated this hypothesis for herpes simplex virus (HSV) infection, a common OI in HIV+ patients. Using limiting dilution analyses, the frequency of HSV-specific CD8+ cytotoxic T lymphocyte precursors (pCTL) and proliferative precursors were quantitated in peripheral blood mononuclear cells from 20 patients coinfected with HIV and HSV-2. The frequency of HSV-specific CD8+ pCTL in HSV+HIV+ individuals was significantly lower than in HSV+HIV- individuals (1 in 77,000 vs. 1 in 6,000, P = .0005) and was not different than in HSV-HIV- individuals (1 in 100,000, P = .24). HIV+ patients who suffered more severe genital herpes recurrences had significantly lower HSV-specific CD8+ pCTL frequencies than those patients with mild recurrences (1 in 170,000 vs. 1 in 26,000, P = .03). In contrast, no significant difference was seen in proliferative precursor frequencies between those patients with mild vs. severe genital herpes (1 in 3,800 vs. 1 in 6,600, P > .5). Quantitative differences in pCTL frequency to HSV appear to be the most important host factor influencing the frequency and severity of HSV reactivation in HIV+ patients. Studies to reconstitute such immunity, especially in people with acyclovir-resistant HSV, appear warranted.

There's a first time for everything: understanding adolescence.

Adolescence has been regarded as a stormy, emotionally turbulent time by many. Some have blamed physiological factors; others have pointed to peer pressure and intimacy. This paper examines the phenomenon of the "first time"--the teenagers first date, first job, first sexual experience. Since these "firsts" are embued with an inordinate amount of emotional investment, they offer an alternative understanding for the "storm and stress"--the emotional underpinnings of adolescence.

Intellectual and psychosocial status of remedial students.

The Kaufman Brief Intelligence Test and Measures of Psychosocial Development were given to assess 49 remedial and 33 average college students. Significant differences were found between the two groups on intellectual and developmental variables. The results are discussed and concerns for staff noted.

Humor in counseling and psychotherapy: a 20-year retrospective.

This article briefly annotates the development of research and theorizing on the use of humor in psychotherapy. Beginning in 1970 and continuing through 1990, the main literature was cursorily scanned. A total of 67 sources are offered for bibliographic use. Sadly, tangential forms of humor such as fun, mirth, frivolity, songs, jokes, puns, witticisms, and other forms of humor are not as readily addressed or investigated.

Personality and scientific promise.

This study examined personality characteristics of 21 boys and 11 girls, children gifted in science and attending a junior high school. Differences between these boys and girls in Grade 6 were noted. Implications for teaching and directions for future research are noted.

Projective drawings as an aid to music therapy.

Music therapy is becoming an increasingly prevalent method for assisting in the mental health needs of a variety of different conditions. However, other tangential aids can greatly facilitate the music therapy process. The use of Projectives techniques to assist in the process of music therapy and to assess the growth and development of clients is described.

Adolescent↗

Adolescent sexual and emotional intimacy.

Teenage pregnancy is a major societal concern. Adolescents are sexually active at younger ages and many become mothers and fathers before age sixteen. While much has been written on adolescent sexual intimacy, little has been said about the issue of mature, appropriate, interpersonal emotional intimacy. The present paper addresses this issue and calls for emotional education in addition to sex education.
