Treatment of hydatid disease in childhood with mebendazole.

The general characteristics of 56 childhood cases of cystic hydatid disease were analysed and the results of mebendazole therapy versus surgery were verified. Pulmonary radiograms and ultrasonography were used in the diagnosis. The cysts were localized primarily to the lungs. Twenty seven patients were surgically-treated, with eight having recurrence after a mean period of 3.6 yrs. Thirty patients received regular mebendazole treatment, in a dose of 50 mg.kg-1 with a mean duration of treatment of 11.7 months. Twenty one patients were cured and discontinued the therapy. Nine still use the drug, seven of whom have had dramatic improvement, while the other two have minimal radiographic changes but subjective improvement in general condition. The lung cysts vanished leaving minimal scars, whilst the liver cysts turned into inactive forms. The surgically-treated and drug-treated groups were similar in age, duration and severity of the disease. The recurrence rate of drug-treated children (1 out of 20) was lower than that of the surgically-treated children (8 out of 27); however, this was not statistically significant.

Plasma levels of mebendazole in children with hydatid disease.

The plasma levels of mebendazole in children with hydatid cyst disease were measured with high pressure liquid chromatography. In 24 children who received mebendazole orally in a dose of 50 mg/kg, the mean (SD) level 4 hours later was 25.76(9.81) ng/ml (87.6(33) nanomole/l). This result was similar to those in most adult series. During more prolonged treatment, the plasma level 4 hours after the dose of the drug rose significantly with respect to the initial level (p < 0.05).

Treatment of pulmonary alveolar microlithiasis with a diphosphonate--preliminary results of a case.

A diphosphonate, disodium etidronate, a compound known to inhibit microcrystal growth of hydroxyapatite was given to a 3.5-year-old girl with pulmonary alveolar microlithiasis (PAM) that was symptomatic. The drug was used for approximately 36 months in a single daily dose of 15 mg/kg. No significant side effects were encountered with somewhat clearing of lung bases in chest radiograms and subjective improvement in general condition. These findings suggest a role of diphosphonates in the treatment of PAM.

[Urinary tract infections in children].

The diagnosis of urinary tract infections were established on fifty children in our hospital in the last year. Most of the patients were between the age of 1-12 month (28 cases, 56%). The ratio of female/male was 3.5. Fever was the most common symptom (17 cases, 34%). In the routine urinalysis, proteinuria and pyuria were revealed in 11 cases (22%), and 44 cases (88%), respectively. E. coli was the most common microorganism isolated from urine cultures (70%). Twenty-two patients were regularly followed up and recurrence was observed in 7 patients (32%).

[Neonatal sepsis].

We noticed coagulase positive staphylococcus is the most common pathogen in the neonatal sepsis in our neonatology unit. We followed 22 cases with neonatal sepsis. Blood cultures revealed coagulase staphylococcus in 9 cases, coagulase negative staphylococcus in 6 cases, Pseudomonas spp. in 5 cases, E. coli in 1 case, Klebsiella spp. in 1 case. The most common symptom was apnea. CRP positivity was noted in 15 cases (68.2%) while increase in immature/total neutrophil ratio was observed in 12 cases (54.5%). However, we detected leukopenia in 2 cases (9.1%). Also, the birthweight of the cases died due to neonatal sepsis were below 2000 gr.

Lactational failure--study of risk factors in Turkish mothers.

Risk factors for lactational failure in puerperium were investigated. Sixty mothers with inadequate daily milk supply for their babies were the lactational failure group, and 60 mothers with similar age having babies with similar age and weight were chosen as the control group. Low prolactin levels, low serum iron and low serum ferritin levels and low aldosterone values and birth in community hospitals were associated with significantly increased risk of deficient lactation. High income of the family, increase osmolality of breastmilk, high systolic blood pressure of the mother, birth by cesarean section were some of the variables that increased the risk that could not reach the level of significance. Prematurity of the baby was not among the risk factors if the gestational ages were more than 30 weeks. It is concluded that physicians awareness of the factors that may adversely effect the initiation and continuation of lactation is vital. Correcting iron deficiency even if it is not overt, sparing more time for the mothers discussing the benefits of breastmilk to their babies, and avoiding cesarean section if possible may help increase the incidence of breastmilk feeding.

Transverse cerebellar diameter and transverse cerebellar diameter/ abdominal circumference index for assessing fetal growth.

In a prospective study of 330 pregnant women, measurements of the transverse cerebellar diameter (TCD), abdominal circumference (AC), biparietal diameter (BPD), head circumference (HC) and TCD/AC ratio and HC/AC ratio were obtained using conventional ultrasonography between 16th and 41st weeks of gestation. Measurement of BPD yielded a strong correlation with gestational age (r = 0.9880 and R2 = 0.9761) by cubic regression analysis. The measurement of TCD also had a very close relation with gestational age (r = 0.9767 and R2 = 0.9539). The ratio between TCD and AC was calculated and found to be 0.1436 +/- 0.0106 (SD) which remained fairly constant throughout pregnancy, while the BPD/AC ratio declined. The 5th and 95th percentiles for this ratio were 0.1279 and 0.1603. Ten of eleven fetuses with TCD/AC ratios exceeding 2 SD (0.1648) were found to have asymmetrical intrauterine growth retardation upon neonatal examination. It is demonstrated that both BPD and TCD measurements are reliable techniques that correlate well with gestational age, and the TCD/AC ratio is valuable in identifying babies with asymmetrical intrauterine growth retardation.
