The effect of macrophage poisoning by silica on experimental pulmonary contusion and its benzo-pyrone treatment.

The effect of poisoning the macrophages with silica on the fine structure of pulmonary contusion was studied in rats. It was found that this causes an increase in the concentration of protein in the interstitial tissue and in the air spaces. It also causes a consequent increase in the amount of interstitial oedema. While the benzo-pyrones normally cause a considerable reduction both in the protein concentrations and in the amount of oedema, they do not act in these ways when the macrophages are poisoned. These results imply that the macrophages normally lyse some of the protein and that this effect is considerably enhanced by the action of the benzo-pyrones.

Fine structural aspects of the influence of lymphatic blockage on cold injury of the brain and skin, and the effects of benzo-pyrones.

The fine structural changes of cold injury, lymphoedema, or both have been studied in the cerebral cortices and facial skin of rats, with and without treatment by a mixture of two benzo-pyrones (coumarin and rutin sulphate salts). It was found that cold injury opens the endothelial junctions of the blood vessels and injures their plasma membranes, thus allowing plasma protein and fluid to enter the tissues, producing oedema. Hence the lymphatics of the skin and the prelymphatics of the brain become dilated, contain excess protein, and the junctions of the former become opened. Macrophages (together with astrocytes in the brain) appear to phagocytise some of the protein. Apart from the injuries to the blood vessels, lymphostasis causes similar effects. The two injuries in combination considerably enhance the effects of either by itself. The benzopyrones greatly reduce the amounts of protein and fluid, particularly in the tissues: the lymphatics lag behind these, especially if they are obstructed.

Experimental blood vascular and lymphatic occlusion in the rabbit ear and the effect of benzopyrones.

The effects of lymphostasis and its treatment by a mixture of benzopyrones were studied in the pinnae of rabbit ears. The blood and lymph circulations to the dorsal surfaces of the ears were largely interrupted by excising a portion of skin down to the perichondrium from one edge to the other. The untreated ears became very lymphoedematous, became necrotic and tell off between 7 and 34 days after the operation. The treated ears were much less lymphoedematous, both to the naked eye and by electron microscopy. Owing to the ubiquity of action of the benzopyrones, the phagocytic capacity of the macrophages is enhanced, removing the accumulating protein, the metabolic products and thus the oedematous fluid. This improves the tissue oxygenation, encouraging lymphatic and vascular regeneration and, to a lesser degree, recanalisation. Also, the red blood cells are protected against lactacidotic rigidification and they and the macrophages are given an increased chance of survival in the remaining stagnant areas by the enhancement of glucose up-take and transport. Upon these factors rests the ability of the tissue to survive. The fact that the ears in the treated groups did not fall off, gives a real indication of the potentiality of the benzopyrones.

[Lymphological aspects of the pathophysiological basis of edema formation].

A merely lymphological analysis of edema, would be as onesided as a merely phlebological, a merely nephrological, or a merely endocrinological analysis. Heart, arteries, veins, microcirculation, lymphatics, and the whole mesenchym, with the connective tissue ground substance and the cells, form an inseparable unity. Only a complex analysis which considers all connections, makes it possible to unterstand the various edemas--in favour of a reasonable therapy.

[Therapy of lymphedema].

The discovery of the role which histiocytes play in the extralymphatic proteolytic mastering of stagnating plasma proteins, renders possible a new, almost causal therapy of lymphoedema as well as of other high-protein oedemas. The effect of this therapy is proven by animal experimental, and has also been clearly demonstrated in secondary lymphoedema of the higher extremities after mastectomy.

The prelymphatic pathways of the brain as revealed by cervical lymphatic obstruction and the passage of particles.

Light and electron microscopy was used to examine portions of the brain, the circle of Willis, and the internal carotid arteries of normal cats and rabbits, of sham-operated ones, and of those whose cervical lymphatics had been ligated. Carbon was injected into the cerebral cortex of some lymphoedematous animals. It was found that lymphatic ligation produced oedema of the brain, and a dilatation of the prelymphatic spaces around the vessels. Carbon was traced in these from the injection site, around the minor and major vessels, in the adventitia of the internal carotid artery, entering lymphatics adjacent to it, and finally in the draining lymph nodes. The oedema and dilated spaces were not present in the control animals. This was taken to indicate that there is a continuous system of non-endothelialized spaces and potential spaces-the prelymphatics-draining the brain into the cervical lymphatics. The protein in these spaces appeared to be increased if the lymph-oedema had lasted three weeks as compared to 24 hours, indicating that one of the major roles of this system is the removal of protein.

[The effect of Tropaacidester Sch 1000 on respiratory resistance (author's transl)].

The effect of Tropaacidester Sch 1000 on respiratory resistance, intrathoracical volume, peripheral blood pressure, and heart frequency over a period of 24 hours were examined in ten patients suffering from obstructive respiratory diseases. Compared with placebo 2 puffs of Sch 1000 administered 6 times significantly decreased respiratory resistance. The bronchodilatatory effects of Sch 1000 and Orciprenalin seemed to be almost identical. Side effects after application of Sch 1000 have not been observed.

A fine structural demonstration that some benzopyrones act as vitamin P in the rat.

In rats fed a diet lacking flavonoids (but which had supplementary vitamin C) definite fine structural alterations were found in blood capillaries and tissues. These fine structural alterations were quite different from those reported in C-avitaminosis and imply a different deficiency. They were largely prevented by feeding the benzopyrones, coumarin or coumarin plus troxerutin, thus pointing to the specificity of the lesions. This implies that, for the rat, benzopyrones are vitamins and that vitamin C and "vitamin P"-deficiency states are qute distinct. In "P-avitaminosis" the basic lesion is the opening of some blood capillary endothelial intercellular junctions. Unlike in C-avitaminosis, the endothelial cells are intact, without pale, grossly swollen cytoplasms.

Treatment of experimental vasogenic cerebral edema with benzopyrones.

Vasogenic cerebral edema was induced in rabbits by cold injury. Benzopyrone treatment, started 15 min after freezing, significantly reduced both brain edema and the increased permeability of the blood-brain barrier. On the other hand benzopyrone-treatment was without any effect in cytotoxic brain edema induced in rats by triethyl tin poisoning.
