[Cardiac involvement in HIV infection: a prospective, multicenter clinical and echocardiographic study].

Cardiac abnormalities are frequently reported in patients with acquired immunodeficiency syndrome (AIDS). Much less is known about the true prevalence of cardiac involvement in patients with human immunodeficiency virus (HIV) infection. We prospectively examined 138 consecutive patients with HIV infection including 41 with AIDS, 49 with AIDS-related complex (ARC), 32 with chronic lymphoadenopathy syndrome (LAS) and 16 with asymptomatic HIV infection. Sixty-one patients had opportunistic infection. The prevalence of cardiac involvement progressively increased from patients with HIV infections or LAS (4%) to ARC (14%) to AIDS (37%). "Major" echocardiographic abnormalities (dilated cardiomyopathy and/or infective endocarditis and/or severe pericardial effusion) were identified in 3 patients (2%), "minor" abnormalities (mild pericardial effusion, hypokinesis of the interventricular septum, mild dilatation of the left ventricle in 21 (15%). Electrocardiographic abnormalities unassociated with echo abnormalities or clinical problems were seen in other 11 patients. End diastolic left ventricular dimension (normalized for body surface area) was higher among AIDS respect to pre-AIDS patients (30.1 +/- 7.1 vs 27.6 +/- 7.5; p less than 0.01) and among patients with respect to patients without opportunistic infections (29.5 +/- 6.5 vs 27.5 +/- 2.4; p less than 0.05). Left ventricular shortening fraction was lower in the subgroup with and absolute CD4 lymphocyte count less than 100/mm3 (31 +/- 7 vs 34 +/- 5; p less than 0.055). In conclusion, in a large, unselected group of patients with HIV infection, echocardiogram discloses cardiac abnormalities in 17% of the cases; their clinical relevance is generally low but in selected patients cardiac tamponade and/or dilated cardiomyopathy (secondary to viral myocarditis) may cause death.(ABSTRACT TRUNCATED AT 250 WORDS)

Clinical evaluation of VDD pacing with a unipolar single-pass lead.

Twenty patients with advanced AV block and normal sinus node function underwent pacemaker implantation, randomly receiving a CPI 910 ULTRA II model VDD pacemaker. The first 13 patients received the implantation of a single lead with a screw-in positive ventricular fixation tip and a unipolar ring floating atrial electrode spaced 13 cm from the tip. A subsequent group of seven patients received a conventional porous tinned-tip lead with a pair of unipolar ring floating electrodes. The second solution was adopted because the best atrial signal was not always in the high or mid-high atrium portion, but sometimes in the middle or mid-low position. With the modified double-electrode lead, the floating atrial electrode that detects the best signal can be selected, cutting out the pin of the one not used. The comparisons between minimal atrial slew rate and maximal ventricular slew rate, as well as those between minimal P wave amplitude and maximal R wave amplitude, show a highly significant range difference, as large as P less than 0.01. Surface electrocardiograms, stress tests, and 24-hour Holter monitoring showed the correct functioning of the system with an average sensing failure from 0.05 to 1%. In conclusion, VDD stimulation is feasible with a single unipolar lead and a floating atrial electrode in conjunction with a pacemaker generator (CPI 910 ULTRA II) originally designed for permanent twin-lead implantation.

[Pulmonary eosinophilic granuloma in adults. Case histories and a review of the literature].

Histiocytosis X is a disease of unknown origin which usually affects multiple organs, including the lung. The age of onset, the clinical course and the pattern of spread allow a distinction to be made between 3 varieties: Letterer-Siwe, Hand-Schüller-Christian and eosinophilic granuloma. The latter form, in adult patients, may predominantly or solely affect the lungs. The authors reviewed clinical, radiographic and CT findings of 7 adult patients with pulmonary eosinophilic granuloma, picked out of a series of 265 cases of interstitial lung pathology, diagnosed since 1973. Typical pulmonary involvement is bilateral, symmetrical and predominates in the upper areas. Honeycomb pattern was found in 1 patient at the onset of symptoms, and in 2 cases during the follow-up, without severe reduction in pulmonary volumes. Pneumothorax was observed in 3 cases and bone lesions in 2. CT added new and important informations such as presence, size and wall thickness of "cystic" lesions. New laboratory tests and bronchoalveolar lavage demonstrated minor diagnostic usefulness than radiological findings. The authors conclude by discussing such problems as prognostic factors and differential diagnosis.

[Exogenous lipid pneumonia. Clinico-radiologic characteristics].

Exogenous lipid pneumonia (ELP) is caused by the aspiration of animal, vegetal or, more often, mineral oils. Even though it may also be acute, ELP is most frequently a chronic disease, affecting people with predisposing factors, such as neuromuscular disorders, structural abnormalities and so on; very often exogenous lipid pneumonia is found in tracheotomized patients. The pathology of lipid pneumonia is a chronic inflammatory process evolving in foreign-body-like reaction, and eventually in "end-stage lung" condition. Clinically, most patients are asymptomatic; few cases only present with cough, dyspnea and chest pain. Eight cases of ELP, studied over the past 3 years, are described in this paper. All the patients were examined by chest radiographs and standard tomograms; 3 patients underwent CT. X-ray features were mono/bilateral consolidation of the lower zones, with air bronchogram and variable reduction in volume. CT density was not specific for fat tissue. In all cases the diagnosis was confirmed at biopsy. In 5 patients, followed for at least one year, clinical-radiological features showed no change. Thus, complications of ELP (especially malignant evolution) could be excluded. The authors conclude that lipid pneumonia must be considered in differential diagnosis of patients with history of usage of oils and compatible X-ray findings. The usefulness of an accurate follow-up is stressed.

[Diagnosis of bronchiolo-alveolar carcinoma].

Clinical and radiographic findings in bronchiolo-alveolar carcinoma are presented, with particular emphasis on differential diagnosis between diffuse and localized forms. A series is presented of 18 patients, observed in our Istitutes since 1980. Diagnostic role of radiology and bioptic techniques is discussed and the leading importance of broncoscopic biopsy is stressed.

Critical analysis of the vectorcardiogram's reliability in the quantitative diagnosis of old myocardial infarction.

The purpose of this work is to verify if the vectorcardiogram (VCG) can be used for the quantification of old myocardial infarction (OMI). 50 cases of acute myocardial infarction were studied by mean of seriate determination of MB-CK isoenzyme and Sobel Index (SI); 4 weeks later, Frank's VCG was recorded. The studied cases did not show any conduction disturbances or other electrical abnormalities besides the sign of OMI. The 50 cases were divided into two groups according to the site of the necrosis: Group A, including 27 cases of inferior and/or posterior infarction and Group B, including 23 cases of septal and/or anterior infarction. On the VCG records, the variables related to OMI and to the general shape of ventricular depolarization and repolarization were measured. The electrical variables and the SI were compared by means of the methods of linear regression and of principal components analysis. No significant correlations were detected. Therefore, we conclude that the VCG is useless in the quantitative assessment of OMI; that such terms as "extensive, localized" and so on, at present widely used in electro- and vector-cardiography, are misleading if based only on surface electrical records obtained at distance from the acute phase of infarction and should therefore be avoided; that the work is consistent with the hypothesis that the electrical pattern of OMI could depend mainly upon peripheral intraventricular conduction disturbances.

[Radiologic and clinico-functional correlations in chronic lung diseases during congestive heart failure].

Two groups of patients were studied with serial correlated radiological functional analysis. Each group had a previous history of chronic obstructive pulmonary disease; however, on admission, only one group exhibited the clinical signs of this superimposed disease. The results of this research showed that chronic pulmonary disease is better defined by functional studies while cardiac failure (even in early stages) is always revealed by radiological examination.
