[Rehabilitation of anoxic brain injured patients after reanimation].

Authors report rehabilitation outcomes of anoxic brain injured patients, following reanimation. In the past 12 years (1988-1999) they treated 54 patients. The patients were selected from other hospitals after successful reanimation, but sustained diffuse anoxic brain injury. The CT and MR examination was generally negative, but small hypodensities were shown rarely (18/54). At the time of admission 21 patients were quadriparetic, 5 patients were hemiparetic, 2 were paraparetic and there were no any paresis in 26 cases. The mean age of the patients was 32 years (7-61). At the time of admission 31 patients were dependent in basic activities of daily living, the mean Barthel Index score was 15 (0-100) but at the time of discharge it was 45 (0-100). 41 patients were discharged into their home and 13 patients had to be transmitted to another hospital for further treating, but later 5 of them died. At the time of discharge only 7 patients had score 100 of the Barthel Index, but they were not able for independent living. Family support, mainly guidance was necessary. Memory deficits, apraxia, attention disturbance, ataxia were the main problems during the rehabilitation process.

[Synthesis and reactivity of debenzorutaecarpine derivatives].

A series of 7,12-dihydropyrimido[1',2';1,2]pyrido[3,4-b]indol-4(6H)-ones was prepared by Fischer indolization of 9-arylhydrazono-6,7,8,9-tetrahydro-4H-pyrido[1,2-a]pyrimidin-4-one s. Quantumchemical calculations (ab initio and AM1) indicate that position 3 of 7,12-dihydro-pyrimido[1',2';1,2]pyrido[3,4-b]indol-4(6H)-one can be involved in electrophilic substitutions, while position 2 is sensitive towards nucleophilic attack. Bromination of 6-methyl-7,12-dihydropyrimido[1',2';1,2]pyrido[3,4-b]indol-4(6H)-o ne (16) with bromine afforded 3-bromo derivative (25), which was reacted with cyclic amines to give 2-amino-7,12-dihydro-pyrimido[1',2';1,2]pyrido[3,4-b]indol-4(6H)-ones (26-30) in an addition-elimination reaction. Vielsmeier-Haack formylation of compound (16) give 12-formyl (31) and 3,12-diformyl (32) derivatives (an N-formyl-1-aza derivative of nauclefidine alkaloid (34) at 60 degrees C and 100 degrees C, respectively. 3,12-dformyl compound (32) was oxidized to 3-carboxyl derivative (33). The quaternary salt (35), obtained from compound (16) with dimethyl sulphate, suffered a ring opening on the action of aqueous sodium hydroxide. The new compounds have been characterized by elemental analyses uv, 1H nmr and in some cases by 13C ruler, CD spectra and X-ray investigations.

[Molecular modeling studies of prolyl endopeptidase inhibitors].

Prolyl endopeptidase, a serine protease is considered to play an important role in the degradation of neuropeptides capable of changing the performance in learning and memory tasks in both animal and human. The inhibitors seem to be promising drug candidates to treat and prevent diseases with associated memory loss such as senile dementia. In the last decade advanced and improved new technologies have appeared to stimulate ideas in the design and synthesis of new drug molecules. The goal of this short communication is to review our results and observations, exemplified by our research on the inhibitors of prolyl endopeptidase. Among them qualitative and quantitative structure-activity relationship studies using conformational analyses, NMR measurements, pharmacophoric plots and CoMFA models are summarised.

[Late post-tracheostomy airway complications in patients with central nervous system injuries].

At decannulation or often weeks later some of the patients undergone long-term mechanical ventilation through tracheostoma reveal symptoms of airway stenosis. Posttracheostomy airway complications detected during the rehabilitation of 34 patients with central nervous system injury are presented. Endoscopic examinations (total 130) were performed with Olympus BF B3 and OES 20-type fiberscopes and Friedel-type rigid bronchoscopes under local or general anaesthesia. Stridor (20 cases) and possible decannulation (10 cases) were the main indications of the first endoscopic examinations. Stridor was caused by tracheal (12 cases) and laryngeal (8 cases) stenosis. With six patients decannulation was possible after the first endoscopy. Removal of granulomas and sutures was successful in 8 cases. Rigid bronchoscopic dilatations were temporarily successful in 7 cases, but in five of them it had to be repeated on emergency. Further therapeutic interventions were: recannulation (3 cases), retracheostomy (4 cases), implantation of silicon T-endoprothesis (5 cases), tracheal resection (4 cases). Three patients are discussed in details. Eliminating airway complications largely contributed to rehabilitation of the nervous and musculoskeletal system, improved the quality of life, and in acute cases it was a life-saving measure. In order to prevent the development of late airway complications or detect them in time, it is recommended to have an endoscopic protocol following every tracheostomy: 1. at the time of decannulation 2. at closing the trachea and 3. 2-3 months after decannulation.

Preservation of ejaculation in stage I non-seminomatous testicular tumours.

The loss of ejaculation is the most serious postoperative complication of patients having stage I non-seminomatous testicular tumours. In order to preserve the capability for ejaculation even after retroperitoneal lymphadenectomy (RPA) the preservation of paravertebral sympathetic ganglia but that of the sympathetic nerves in the territory of aortic bifurcation and common iliac arteries to be ulcessary.

Transvesical and transabdominal closure of vesicovaginal fistulas.

Between 1973 and 1985, a total of 46 patients were operated for vesicovaginal fistula. Depending on the extension and localization of the fistula, transvesical or transperitoneal closure was performed. In 12 patients ureteral neoimplantation was made. Fistula recurrence was observed in 6 cases. Based on their favourable experience, authors recommend this surgical solution for the closure of fistulas adjacent to the ureteral orifices and/or for that of larger fistulas.

Vascular anomalies and retroperitoneal lymphadenectomy (RLA).

In the past 7 years a total of 90 RLAs were performed in patients with testicular tumour and the vascular anomaly of the retroperitoneum was diagnosed in 10 cases. Attention is called to these vascular disorders which render the performance of lymph dissection difficult but can never influence the question of operability.

Bilateral urological tumours.

In the 5-year period between January 1, 1977 and December 31, 1981, 6 patients with bilateral urological tumours localized in various organs were treated at the Department of Urology, Debrecen University Medical School. As compared to earlier results, the survival of patients with bilateral adrenal renal and testicular tumours may be improved by the early diagnosis, immediate and adequate treatment, by a careful follow-up lasting for years and, when needed, by a delayed intermittent chemotherapy.

Problems of the indications of radiotherapy in renal tumours after radical nephrectomy.

The complex therapy of renal tumours is discussed. While pointing to the advances in this field over the last decade, the fact is emphasized that it is primarily in the radicality of the operations were progress has been achieved. Radiotherapy is viewed with a critical eye. Its hazards are illustrated by observations of four lethal cases consecutive to complications of postoperative irradiations, which had been administered without any need when malignant changes were no longer present.
