Subcutaneous low-dose interleukin-2 and intravenous 5-fluorouracil plus high-dose levofolinic acid as salvage treatment for metastatic colorectal carcinoma.

Thirty-three consecutive patients with recurrent and/or metastatic colorectal carcinoma (CRC) refractory to previous chemotherapy have been treated with levofolinic acid (I-FA) 100 mg/m2 i.v. over 1 h infusion followed by 5-fluorouracil (5-FU) 600 mg/m2 i.v. bolus every week for 6 weeks followed by a 2 week interval. Patients also received rIL-2 s.c. at 3 MU daily from day 1 to day 5 of each week for at least four consecutive weeks per cycle. Enrolled patients were divided in two groups: (i) group 1 including patients with progressive tumor refractory to chemotherapy with I-FA + 5-FU given for metastatic disease and (ii) group 2 consisting of patients with diagnosis of metastatic disease within 3 months from the completion of adjuvant chemotherapy with 5-FU + levamisole (LEV) after primary surgery. No objective response was observed in the group of 11 patients with CRC resistant to previous I-FA + 5-FU, thus no further patient with progressive disease after I-FA + 5-FU was included in the trial. In the group of patients pretreated with 5-FU + LEV, four patients experienced a PR with a mean duration of 7.3 months (range > or = 4.0-8.6) for an overall response rate of 18% (95% CI 12-26%). A stabilization of disease was observed in five cases (23%) with a mean duration of > or = 5.6 months (range > or = 2.0-7.0). The remaining 13 patients progressed. No complete responses were achieved. The mean overall survival was > or = 9.5 months (range > or = 2.0-14.0). Toxicity was generally mild. This study demonstrates that the combination of s.c. rIL-2 and intravenous 5-FU + I-FA on a weekly schedule may be safely given to patients with metastatic CRC on an outpatients basis. The addition of low-dose rIL-2 does not modify the toxicity profile of 5-FU + I-FA, even if IL-2-related side-effects such as systemic symptoms or cardiac abnormalities are to be expected. The clinical activity of the combination is not good, at least in terms of response rate, even if the duration of partial responses may suggest to test rIL-2 in a prospective study with response duration and overall survival as the final end-points.

[Effectiveness of econazole on pregnant women with vulvo-vaginal candidiasis].

Among the female patients attending the out patient clinic of the Obstetric Service of the University Hospital of Caracas, Venezuela a hundred were chosen who presented vaginal discharge and itching. The presence of C. albicans and the existence of candidiasis in the vulvo-vagina of the 100 pregnant women was investigated. Those patients where candidiasis was found, were also investigated as to the simultaneous presence of C. albicans in the mouth and the anal skin. Gyno-Pevaryl therapy was performed by administering one 150 mg ovule daily for three consecutive days. At the same time, their partners were treated with Pevaryl 1% cream. The post-therapy control was made 36 hours after conclusion of treatment and further check-up were made one week and two weeks later. Only those patients who attended the post-treatment controls were taken into account for the evaluation of the results. C. albicans was identified in 60 cases by nascent culture of the vulvar secretions, using a bile-agar medium. Of these, 50 had vulvo-vaginal candidiasis (hyphae and blastospores on direct examination). 92% of the women with vulvo-vaginal candidiasis complained about pruritus at the moment of the examination. No significant differences were observed with respect to the age, the period of gestation and parity of the positive and negative patients. Of the 40 patients who attended only the first control, 23 were cured (57.5%), 9 improved (22.5%) and 8 (20%) were not cured. The cure rate increased to 70% and 72.5% taking into account those patients, who also attended the second and the third consecutive treatment sessions, respectively. In those patients suffering from vulvo-vaginal candidiasis, 37.5% were found to be carriers of C. albicans in the mouth and 70% also in the anal region. This seems to indicate that the mouth and the anal region can constitute the source of re-infection in the majority of the cases following topical vaginal treatment.

[Incidence of yeasts isolated from the mouth and toothbrushes in Venezuela].

117 yeast strains from the mouth, and 69 from toothbrushes of 229 patients from the Hospital Universitario of Caracas (Venezuela) were studied. Candida albicans was found to be the most frequent yeast in both materials with 56.4%, and 52.1% in the mouth and toothbrushes, respectively. C. tropicalis with 16.2%, and C. parapsilosis with 7.6% followed C. albicans in the mouth. In the toothbrushes C. parapsilosis with 11.5%, and C. tropicalis with 6.0% followed C. albicans. The incidence of other yeasts was not significative. Torulopsis glabrata was not found in the material studied.

[Primary tumors of the common bile duct. Apropos of 24 cases].

Surgical management of 24 cases of primary carcinoma of the main extrahepatic bile duct are presented. This tumour forms less than 2% of the cases in which bile duct surgery is required and its intermittent appearance makes rational exploitation of surgical management difficult. Obstructive icterus is the main and constant symptom. Preoperative diagnosis of the obstruction necessarily requires transhepatic cholangiography following incision under local anaesthesia. Resection starting from the middle third of the duct was performed in 4 cases. A palliative biliodigestive shunt was applied in 5 and decompression by means of a Kehr tube was adopted in 11. Intraoperative mortality was high, mainly on account of frequent liver cell damage. In some instances, however, survival was measured in years, even in the absence of surgical resection. Surgery must thus be tailored to the patient's strength. When the patient's condition is good and the tumour is situated in the common bile duct or distal hepatic duct, an internal shunt can always be attempted. When higher segment of the duct are involved, a shunt on a Kehr tube or in the form of a U probably offers the best solution.
