Relationship between gonadotrophin secretion, inhibin B and spermatogenesis in oligozoospermic men treated with highly purified urinary follicle-stimulating hormone (uFSH-HP): a preliminary report.

PURPOSE: To study the potential relationship between circulating follicle-stimulating hormone (FSH) gonadotrophin, inhibin B, spermatogenesis and the benefit of highly purified urinary FSH (uFSH-HP) treatment in men with oligozoospermia. METHODS: Twenty-nine normogonadotropic, normogonadal men with oligozoospermia were evaluated. Serum hormonal concentration and spermiogram were analyzed before and after 3-month treatment with uFSH-HP. RESULTS: Our results support the hypothesis that inhibin B is involved in the physiological regulation of FSH secretion and reflects the FSH-stimulated Sertoli cell function. After treating with uFSH-HP for 3 months a significant increase in all semen parameters was observed in most of the patients. CONCLUSIONS: Although the benefit of inhibin B as a prognosis factor in oligozoospermic patients treated with FSH is not defined, it can be considered as an important serum marker in reproductive and testicular functions. Treatment of oligozoospermic patients with uFSH-HP has shown to be an effective and safe therapy.

[Antioxidative state and lipid peroxidation in erectile dysfunction: preliminary study].

OBJECTIVE: The aims of this study were to analyze the cavernosal tissue metabolic status of patients with vascular and psychogenic impotence and attempt to establish metabolic differences between these types of erectile dysfunction. METHODS: 103 patients were classified according to the type of erectile dysfunction into group A (vascular) and group B (psychogenic). Diagnosis was based on clinical history, physical examination, intracavernosal injection test, penile echo-doppler assessment, cavernosography-cavernosometry and nocturnal penile test. Cavernosal and peripheral venous blood samples were obtained from each patient and the lipoperoxide (LPO) levels and total antioxidant status (TAS) were determined. SPSS V9.0 was used for the statistical analysis. RESULTS: The mean age was 62 years (range 32-73). Cavernosal blood lipoperoxide levels were statistically higher (p < 0.05) in patients with vascular impotence (2.45 mumol/L) than in those with psychogenic impotence (1.47 mumol/L). Cavernosal blood total antioxidant status was statistically higher (p < 0.05) in patients with psychogenic (1.40 mmol/L) than those with vascular impotence (1.10 mmol/L). The lipoperoxide levels and total antioxidant status for peripheral blood were 1.68 mumol/L vs 1.60 mumol/L and 1.29 mmol/L vs 1.35 mmol/L, respectively, with no statistically significant differences between both groups. CONCLUSIONS: Cavernosal blood lipoperoxide levels and total antioxidant status can be an indicator of cavernosal tissue metabolic status and function.

[Bladder neoplasms in patients under 40 years of age].

The incidence, presentation and clinical evolution of vesical tumours diagnosed in our Centre over the last 10 years (1986-1995) in patients under forty are examined. This retrospective study included 19 of the 643 neoplasias treated during that period; 7 of these in patients under 30 and 12 in patients over 31. Haematuria was the most frequent reason for visiting the Centre. At the time of diagnosis, all cases were surface tumours and none progressed to the infiltrant stage. Tumours in patients over 30 are characterised by a higher histological grade and greater relapse rate versus those in patients under 30. Therefore, age may be a favourable prognostic factor in patients under 30 versus the older group who follow a course more similar to the remainder of usual patients. Diagnosis and treatment of these neoplasias should be just the same as for tumours in older patients.

Risk factors of infection and digestive tract colonization by Candida spp. in a neonatal intensive care unit.

In this study we determined the risk factors for infection and colonization by Candida spp. in our Neonatal Intensive Care Unit (NICU). We designed a cohort study in the NICU of the La Paz University Hospital. Over a one year period, 153 neonates admitted to the NICU were studied. In the bivariable analysis, hospitalization period, central catheterization, parenteral feeding, parenteral lipid feeding, respiratory support and premature rupture of the membranes (PRM) were statistically associated with infection and colonization; age was only associated with infection. Logistic regression was used to control the confusing factors. The hospitalization period was a risk factor for infection and colonization. PRMs were also colonization risk factors. We developed a statistical equation that predicts the probability of infection or colonization by Candida spp. that are related to a neonate's specific characteristics. The equation helps us to develop preventive procedures.

[Determination of the tumor marker CA 72-4 in gastric carcinoma].

In this study, the authors determined the sensitivity and specificity of the new tumor marker CA 72-4 in detecting adenocarcinoma of the stomach. CA 72-4 serum levels were investigated in 279 patients consisting of: 100 control subjects, 88 patients with benign gastrointestinal disease and 91 patients with gastrointestinal cancer (31 with gastric cancer). We used a cut-off of 5 U/ml. The specificity of CA 72-4 was 91% and the sensitivity was 68%. Highest levels of CA 72-4 were found in patients with more advanced tumor (stages III-IV) and four of 5 patients with stage I had elevated levels of this tumor marker. These findings suggest that the use of serum CA 72-4 measurements may be more useful than other tumor markers in the diagnosis, prognosis and detection of recurrence in patients with gastric cancer. Although CA 72-4 can be elevated in other gastrointestinal adenocarcinomas, it can be useful in alerting the clinician to the possible presence of an intraabdominal neoplasm.
