Percutaneous transperitoneal nephrolithotomy.

The removal of a recurrent calculus from the pyeloureteral junction of a malrotated kidney by means of laparoscopically controlled percutaneous transperitoneal nephrolithotomy is described.

Diagnosis of urothelial tumor by ureterorenoscopy.

In the past, radiologically detected filling defects in the upper urinary tract were correctly interpretable only by means of cytological techniques. The introduction of ureterorenoscopy has provided a reliable method for the detection of tumors in the ureter and renal pelvis. In contrast to transurethral ureterolithotripsy, diagnostic ureterorenoscopy entails few complications. It thus fills a diagnostic gap and facilitates therapeutic planning. A report is presented on 15 patients with obscure hematuria, or unclear filling defects in the upper urinary tract. In 10 patients, the unequivocal diagnosis of urothelial carcinoma was endoscopically and bioptically established by ureterorenoscopy. Small nonradiopaque concrements were found in 3 cases. The origin of hemorrhage could not be determined in 2 patients.

[Transurethral ureterolithotripsy in the treatment of ureteral calculi].

We report our experiences with transurethral ureterolithotripsy for treatment of ureteral calculi in 68 patients. 67% of the ureteral stones were extracted successfully. Failures and complications occur often by higher localisation of concrements. Only in 2 patients major complications occurred. Late complications (Ureter Manipulation Syndrome-UMS) were not observed in our patients. In cases with ureteral perforations or mucous membrane lesions ureteral stent must be used.

[Antireflux-plasty in neurogenic bladder-emptying disorders].

14 patients (20 ureter units) with neurogenic bladder and reflux in the upper urinary tract were treated with antireflux procedures. In general, ureterocystoneostomies according to the method of Cohen, 2 to that of leadbetter-Politano, and 1 according to the technique of Glenn-Anderson were performed. Infravesical obstruction, high intravesical pressure, and low bladder capacity have to be treated before an antireflux operation is done. The purpose of the antireflux procedure is to prevent reflux nephropathy and to restore urinary continence, by performing intermittent sterile self-catheterization and implantation of an artifical sphincter. Urinary diversion is seldom necessary.

Specificity and sensitivity of human chorionic gonadotropin radioimmunoassays. A comparison of two different approaches.

Two different procedures for determination of HCG in plasma from patients with testicular germ cell tumors, patients with benign disease, and normal controls were applied: (1) measurement of the amount of HCG and its beta-subunit, and (2) assay of the isolated beta-subunit alone. The test which measured the free beta-subunit and the HCG molecule gave 13% more sensitivity in patients with nonseminomatous germ cell tumors. The specificity in blood donors was 100% for both tests. In benign diseases the measurement of the complete molecule gave 100% specificity, whereas the other procedure had a specificity of 95%. For these reasons, the determination of the complete molecule seems to be preferable for patients with nonseminomatous germ cell tumors.

[Heterotopic excretion of Ioxitalamate (author's transl)].

The relatively rare case of heterotopic excretion of a recently introduced contrast medium easily excreted by the kidney, is demonstrated by means of 2 examples. In one case, the patient complained of colicky pain in the right flank, whereas tumour nephrectomy had been performed in the other patient. The hypotheses presented in medical literature regarding ectopic excretion of contrast medium, are mentioned, but it does not appear possible to safely explain the observed extrarenal excretions.

[Central register and combined study of testicular tumors (author's transl)].

To successfully combat cancer, research needs to be based on prospective investigations and central collation of data. An initial report is presented concerning one such investigation that has been in progress since September, 1976: "central register and combined study of testicular tumors--Bonn." The aims are to collate epidemiologic data, achieve a standardization of the stages of the cycle of the seminiferous epithelium and of classification, on the model of the WHO, as well as to attain maximum efficiency of diagnosis and therapy. Fourteen participating clinics in the German Federal Republic, Austria, and Switzerland, and 70 submissions of data up to the 1st of March, 1977, amply justify the efforts so far expended and encourage us to solicit the cooperation of previous nonparticipants.
