Stimulation of postsynaptic 5-HT1A receptors is responsible for the anticonflict effect of ipsapirone in rats.

Ipsapirone (1.25-10 mg kg-1), a non-benzodiazepine anxiolytic drug with high affinity for 5-hydroxytryptamine1A (5-HT1A) receptors, increased dose-dependently the number of punished licks in the drinking conflict test (Vogel test) in rats. The anticonflict effect of the drug administered at a dose of 5 mg kg-1 was not modified in animals with lesions of 5-HT neurones, produced by p-chloroamphetamine (PCA, 2 x 10 mg kg-1). The anticonflict effect of ipsapirone in PCA-pretreated rats was antagonized by the 5-HT1A receptor and alpha 1-adrenoceptor antagonist NAN-190 (1-(2-methoxyphenyl)-4-[4-(2-phthalimmido)]butylpiperazine hydrobromide; 0.5-1 mg kg-1), but not by the selective alpha 1-adrenoceptor blocker prazosin (0.5 mg kg-1). Neither NAN-190 nor prazosin affected the punished response in PCA-pretreated rats. The present results indicate that the anticonflict effect of ipsapirone depends on stimulation of postsynaptic 5-HT1A receptors.

[The incidence, etiopathogenesis and therapeutic approaches in injuries to the eye and its adnexa].

The authors study the consequences of the ocular traumatisms hospitalised and treated in the Department from the Central Military Hospital in Bucharest, on a group of 1144 patients. This group represent 7.84% from a total of 14,641 patients hospitalised in that period of time. There are also included the patients who suffered ocular traumatisms during the revolution in December 1989.

Malondialdehyde and 4-hydroxynonenal protein adducts in plasma and liver of rats with iron overload.

In hepatic iron overload, iron-catalyzed lipid peroxidation has been implicated in the mechanisms of hepatocellular injury. Lipid peroxidation may produce reactive aldehydes such as malondialdehyde (MDA) and 4-hydroxynonenal (4-HNE), which may form aldehyde-protein adducts. We investigated whether lipid peroxidation occurred in rats fed a diet containing 3% carbonyl iron for 5-13 wk, and if this resulted in the formation of MDA- and 4-HNE- protein adducts. Chronic iron feeding resulted in hepatic iron overload (greater than 10-fold) and concomitantly induced a 2-fold increase in hepatic lipid peroxidation. Using an antiserum specific for MDA-lysine protein adducts, we demonstrated by immunohistochemistry the presence of aldehyde-protein adducts in the cytosol of periportal hepatocytes, which co-localized with iron. In addition, MDA- and 4-HNE-lysine adducts were found in plasma proteins of animals with iron overload. Only MDA adducts were detected in albumin, while other plasma proteins including a approximately 120-kD protein had both MDA and 4-HNE adducts. In this animal model of hepatic iron overload, injury occurs primarily in periportal hepatocytes, where MDA-lysine protein adducts and excess iron co-localized.

Aldehydes

[Present problems in health services and health care].

In order to deal with and resolve contemporary problems of the health services and health care it is important to outline their concept, contents and definition as well as those of their fundamental constituents--the material, technical basis, workers and their activities, which determine the state health policy. The author analyzes from this aspect the hitherto accomplished development with regard to all basic constituents and discusses the problem of the relationship of the intensity and extensity of activities of the health services and health care. A foremost part is played by medical is played by medical science--medicine, its state, knowledge assembled by students during their undergraduate studies and medical graduates during their postgraduate training. A substantial and decisive component of the health services as regards therapeutic and preventive care is formed by health workers and the quality of their work which depends on objective as well as subjective factors which are analyzed. Health services do not comprise only practical activities but social medicine and the organization of health services are very important scientific disciplines which participate in the quality of activities. This scientific discipline was founded by prominent personalities. It must be stated that in practical medical work leading authorities do not always respect the scientific character of the discipline and thus very complicated problems arise as regards scientific control. All these problems from the aspect of the department of social medicine and organization of health services as well as subjective and scientific control must be resolved from the aspect of reconstruction of society.

[Psychosocial and socio-medical problems in the medical profession].

Under conditions of contemporary highly developed medicine and allied disciplines of the new scientific concept of health care it is essential that doctors and other health workers should be harmoniously developed personalities. One of the main characteristics of such a personality is the overall health potential. Doctors should not only propagate a healthy lifestyle but should in this respect serve as an example. Unfortunately in many instances paradoxical behaviour in relation to their own health is found. The problem of health protection of doctors must be conceived comprehensively not only in conjunction with the general social environment but also in conjunction with his specific working environment. In the contemporary medical literature in the world we encounter in connection with investigations of health protection, and in particular the morbidity of doctors and other health workers, the term morbus medicus. This involves a complicated and specific complex of factors ensuing from the complexity of the medical profession and its risks which does not cause specific but nevertheless increased sequelae in certain areas of morbidity. At present the majority of occupations is associated with neuropsychic stress. In the medical profession neuropsychic stress is much greater than in other occupations. Therefore prevention of negative stress in the medical profession must ensue from an endeavour to neutralize these negative factors - i.e. to create optimal working conditions and optimal interpersonal relations, and from improving the standard of the personality profile of doctors.

[Psycho-social and socio-medical problems with modern patients and diseases. II].

The complexity of psychosocial diagnosis is closely linked with sociomedical problems of diagnosis and treatment of contemporary patients. Contemporary patients are a complicated complex as regards factors and conditions of the genesis and development of disease and its symptomatology. As to the main external factors which influence the development of disease, it is important to mention in particular neuropsychic strain, adynamia, unbalanced diet, undesirable habits and contaminants--allergens. What is characteristic of contemporary patients? Polymorbidity, an altered pathology of diseases as regards the character of external conditions in the present period, a considerable increase of the morbidity from neuroses and neurotic syndromes. A very urgent problem of contemporary patients is obesity and its complications. In conjunction with the development of disease we must take into account also the contemporary allergic background. Among patients there is a high rate of chronic foca diseases, undesirable habits, latent clinical symptomatology.
