Application of 99mTc-DTPA aerosol in evaluation of pulmonary epithelial permeability.

Measuring of the rate of radioactivity decrease of inhaled 99mTc-DTPA aerosol may detect disorders of alveocapillary membrane integrity. The study included 21 patients (11 non-smokers and 10 smokers) suffering from different pulmonary diseases (pulmonary embolism - PE; chronic obstructive pulmonary disease - COPD; pneumonia - PN; occupational diseases - OD) in order to detect disorders of pulmonary epithelial permeability (PEP) and 2 healthy individuals (non-smokers) with normal findings (NF). DTPA was labelled using the standard procedure with 1480 MBq 99mTc in 1 ml of physiologic saline. Patients with nasal obstruction inhaled aerosol for 2.5 min. particle size 0.8 micron produced in a nebulizer connected to O2. After that gamma scintillation camera and computer were used for data acquiring in dynamic mode. After that ventilation and perfusion scintigraphy of the lungs was performed in four standard projections. Data processing was conducted with ROI drawing after both lungs on the added image. Clearance value was expressed in T1/2 (min), while the curves had monoexponential shape in all patients. In the non-smoking group mean clearance value for both lungs in patients suffering from PE, COPD and PN did not differ from NF. Clearance in the part of the lungs affected with disease (pneumonia, embolism) was faster than in healthy pulmonary tissue. In sick smokers, however, mean pulmonary clearance value was higher than in non-smokers, irrespective of the type of disease. Pulmonary clearance in individuals suffering from occupational diseases was also accelerated, irrespective of the fact whether the patients smoked or not.

[Where is the right angle?].

Cephalometric radiographs of 49 cases with normal occlusion in the period of permanent dentition (all cases were orthodontically untreated) to study the angle of mandibular incisors obtained by Tweed's and Kubein-Meesenburg's methods. It is known that in both methods the angle should be 90 degrees in cases with normal occlusion. The results obtained were statistically treated and compared. It was found that majority of cases showed value of the angles to be 90 degrees or very close to it obtained by both methods. This finding indicated that two planes were parallel in majority of cases investigated. A change of the Tweed's angle produced change of the Kubein-Meesenburg's angle in such a way that sum of these two angles was 180 degrees. Inclination of the lower incisors showed a considerable variability which indicated an individual sagittal position of these teeth.

[Cephalometric and tomographic evaluation of the treatment results of Class II division 1 obtained by elastic activator].

A group of 10 patients with Class II division 1 malocclusion was treated by an elastic activator for 11 months in order to achieve an orthopaedic improvement in the anteroposterior jaw relationship. Another group of 8 untreated patients with the same malocclusion served as a control group. Both groups involved the patients between 8 and 10 years of age. A comparative cephalometric and tomographic study prior to the treatment and after completion of the treatment revealed the following results: an improvement in the occlusal relationships due to both skeletal (an anterior mandibular displacement and an increase in the mandibular length) and dentoalveolar changes; it was possible to produce a growth stimulation of the mandibular condyle associated with a translation of the glenoid fossa by using an elastic activator; there was a direct correlation between the effects of the treatment and the age period of the patients (mixed dentition).

Cylindrically modeled bone grafts.

A technique of autogenous grafting for repair of large defects in long bones is presented. Quadrangular grafts, taken from the external table of the posterior ilium, are modeled by bending them into cylindrical form, which has higher mechanical resistance as compared to flat struts. Cancellous chips are added. The original shape of the bone is immediately restored, and heavy internal fixation is avoided. combination of cortical and cancellous bone grafts provides both strength and rapid bone healing. There is no risk of fracture at the donor site. Of eight consecutive cases operated on, seven were successful. One failed because of relapsing infection. The procedure is suitable in children, adolescents, and young adults, for difficult cases of large defects in long bones left after resection of tumors and tumor-like conditions, as well as after diaphyseal distraction for leg lengthening.

Relocation of congenitally elevated scapula.

In the treatment of Sprengel's deformity, extraperiosteal greenstick fracture of the clavicle in conjunction with surgical release of all attachments between the scapula and the spine provides an easy, safe method of relocating the scapula to its normal level. The improved position is maintained by temporary fixation of the inferior border of the scapula to the eighth rib with slowly resorbable sutures. Section of the coracoclavicular ligaments, excision of the superior pole of the scapula, and suture fixation of the medial angle of the scapula to the spinous process of the fourth vertebra enables the procedure to be performed on patients until the age of puberty. This surgical technique has been performed in 28 consecutive patients with Sprengel's deformity, aged 4 to 19 years, without any neurovascular complications. A normal scapular position was achieved in 67% of cases, 1 to 2 cm elevation in 29%, and 5 cm in 4%.

Isolation of Streptomyces rimosus Mutants with Reduced Actinophage Susceptibility.

The infection of Streptomyces rimosus by the virulent actinophage RP1 was partially characterized. RP1 infection of the host cells results in a dramatic decrease in viable cell count, followed by reduced antibiotic production. Phage-resistant mutants were isolated after mutagenic treatment and RP1 selective pressure. Characterization of the isolated mutants has revealed that RP1 infection had no influence on their growth and antibiotic production. However, multiplication of the phage particles in the lawns of resistant mutants was detected. Since these strains differ from the wild type in RP1 relative efficiency of plating, plaque morphology, and the time necessary for plaque appearance, they are considered to be semiresistant mutants. The propagation of RP1 on semiresistant strains is characterized by lower adsorption of phage particles and longer latent and rise periods. As a consequence, the multiplication of the phage is slower than that of their host, which consequently reduces the ratio of phage to its host, thus diluting out the phage.

[Anesthesia in surgery of the veins].

Patients with varicose complex of low extremities do not present any special problem in anaesthesiology. Method recommended is inhalational anaesthetic, eadly mobilisation of patient and infusion of low molecular dextran, during and immediately after operative procedure. During thrombectomy from deep venous system there is real danger of pulmonary embolism. This group of patients has higher risk than former, too. Authors recommended their own method for prevention of preoperative centripetal mobilization of thrombus, using a standard Recklinhausen's cuff inflated against abdominal wall under a plexiglass plate.

[Use of Runac in patients with infections of the lower respiratory tract].

Thirty patients went through the outpatient treatment. Causative agent was established in sputum, throat and nose smears of 26 patients. Streptococcus pneumoniae (56%) and Haemophilus influenze (30%) are mostly isolated. Good effects of the medicine is characteristic by quick withdrawal of the symptoms characteristic for respiratory tract infections (cough and expectoration, febrility, physical finding) as well as by debacillosis after 10 days therapy. The effective result of medicine has been particularly noticed through quick reduction until complete disappearance of expectoration after three to four days since the onset of treatment. No side effects of the medicine have been found.

[Results of Visiren administration in patients with respiratory infections].

One of the most frequent causes of deterioration of chronic obstructive lung diseases is the infection. The existence of microorganisms resistant to antibiotics is becoming an increasingly serious therapeutical problem. Kinolones have a wider spectrum of action than the majority of other antibiotics, with strong effect on multiresistant gram-negative and gram-positive bacteria. Visirene (Ophloxacine) is a new antibiotic in this group. The efficiency of Visirene in the treatment of acute infections of the upper and lower respiratory organs, and acute deterioration or chronic obstructive lung diseases due infections, were investigated. The treatment was carried out on 30 patients in outpatient services. In all patients the appearance and duration of symptoms characteristic for the infection of the respiratory organs were examined (cough, coughing out, increase of body temperature, physical lung findings, dyspnea). The isolation of the causative agent was made from the sputum, throat and nose smears. Medical treatment began only if findings and antibiogram results were positive. The positive effect of Visirene was manifested in rapid withdrawal of symptoms and disappearance of bacteria in the sputum 7-10 day after the beginning of its administration. The administration of the drug provoked no side effects in these thirty patients. All findings suggested a very good effect of Visirene (Ophloxacine) in the treatment of infections of the respiratory system.

[The number of chronic patients contributing to total tuberculosis morbidity in the area of the city of Belgrade].

During four years (from 1984-1987) a group of 94 patients with chronic pulmonary tuberculosis were investigated. Mean age of patients was 51 years-the youngest was 27 and the oldest 78. Mean time of disease was 5.4 years. They made 4.8% of all bacteriologically positive nonchronic patients. One third of them were resistant to one, 18% on two, 22% on three and 10% on all known antituberculosis drugs. It is very interesting that 19 patients or 20% were sensitive to all known antituberculosis drugs but tuberculosis still persisted.
