Alkaline phosphatase activity in normal and inflamed dental pulps.

Alkaline phosphatase (ALP) seems to be important in the formation of mineralized tissues. High levels of ALP have been demonstrated in dental pulp cells. In the present study ALP activity was analyzed in normal healthy human dental pulps, in reversible pulpitis, and in irreversible pulpitis. Enzymatic ALP control values for the normal healthy pulps were 110.96+/-20.93. In the reversible pulpitis specimens the ALP activity increased almost eight times to 853.6+/-148.27. In the irreversible pulpitis specimens the values decreased sharply to 137.15+/-21.28 and were roughly equivalent to those seen in normal healthy pulps. The differences between the groups (control vs. reversible pulpitis and reversible pulpitis vs. irreversible pulpitis) were statistically significant. These results could point to a role of ALP in the initial pulp response after injury.

Aspartate aminotransferase activity in human healthy and inflamed dental pulps.

Aspartate aminotransferase (AST) seems to be an important mediator of inflammatory processes. Its role in the progression and detection of inflammatory periodontal disease has been increasingly recognized in recent years. In the present study AST activity was analyzed in normal healthy human dental pulps, in reversible pulpitis, and in irreversible pulpitis. Enzymatic AST activity showed that the control values for the healthy pulps were 4.8 +/- 0.7 units/mg of pulp tissue. In reversible pulpitis specimens the AST activity increased to 7.98 +/- 2.1 units/mg of pulp tissue. In irreversible pulpitis specimens the values decreased to 2.28 +/- 1.7 units/mg of pulp tissue. Differences between the groups (control versus reversible pulpitis and reversible pulpitis versus irreversible pulpitis) were statistically significant (p = 0.0015). These results could point to a role of AST in the early events that lead to development of pulpal inflammation.

P53 protein expression in odontogenic cysts.

p53 protein seems to be related to the suppression of cell proliferation. p53-positive tissues seem to have a higher proliferative activity than p53-negative ones. Odontogenic keratocyst (OKC) has a different behavior from other types of cysts because it is more aggressive, with a tendency to recurrence. Twenty-two dentigerous cysts, 24 radicular cysts, and 20 OKCs were used in the present study. Two dentigerous cysts (9.1%), 2 radicular cysts (8.3%), and 9 OKCs (45%) expressed the p53 protein. The differences between the three groups were statistically significant (p = 0.003). In 10 cases of OKCs epithelial dysplasia was found. One of the 10 OKCs without dysplasia and 8 of the 10 OKCs with dysplasia were p53-positive: the difference between the two groups was statistically significant (p = 0.007). The overexpression of p53 protein was not on the other hand correlated with the occurrence of multiple, bilateral, and recurrent OKCs. Moreover the distribution of p53-positive cells was parabasal in contrast with other types of cysts. These qualitative and quantitative differences in proliferative activity in OKCs seem to point to an alteration in cell cycle control.

Oral focal mucinosis of the gingiva: case report.

BACKGROUND: Oral focal mucinosis is a rare disease of unknown etiology, where the connective tissue undergoes a focal myxoid degeneration. METHODS: We describe a 48-year-old patient who was referred for a firm, tender mass located on the gingiva of the left central mandibular incisor. The first clinical impression at examination was that of a periodontal abscess. RESULTS: The lesion underwent a biopsy, and the final microscopic diagnosis was oral focal mucinosis. CONCLUSIONS: It must be stressed that in most focal gingival lesions, a preoperative diagnosis can be almost impossible.

Spindle-cell lipoma of the cheek: a case report.

Spindle-cell lipoma (SCL) is a distinct histological variant of lipoma. Clinically, it appears as a solitary, subcutaneous, circumscribed lesion. SCL accounts for about 1.5% of all adipocytic tumours. Only nine cases of intraoral SCL were found in the literature. Microscopically, mature adipocytes and spindle cells are immersed in a myxoid stroma. SCL needs only local excision, and it does not recur.

bcl-2, p53, and MIB-1 in human adult dental pulp.

Little is known about the renewal of some groups of cells in dental pulp, and the occurrence and significance of physiological cell death in dental pulp is not yet understood. The possibility of odontoblast disappearance by apoptosis has been proposed, and the presence of apoptotic cells in the rat and human odontoblastic and subodontoblastic layers has been recently described. bcl-2 and p53 are proteins involved in the apoptotic pathway, whereas MIB-1 is a proliferating cell marker. The aim of our study was an immunohistochemical evaluation of bcl-2, p53, and MIB-1 in healthy normal pulps of young human subjects. With bcl-2 immunostaining, some positive cells were found in the odontoblastic and subodontoblastic layers, whereas with MIB-1, only a few stromal cells were positive, and all odontoblasts were consistently negative. No cells were positive to p53. The bcl-2 immunoreactivity of the cells of the odontoblastic and subodontoblastic layers could help to explain the presence of apoptotic cells found in these regions.

Hemangioma of the mandible presenting as a periapical radiolucency.

A case of an hemangioma of the mandible in a 53-yr-old female patient is presented. The lesion was removed, and no atypia or mitotic cells were found. The excision was curative, and the overall prognosis was excellent. A differential diagnosis of radiolucent periapical lesions of the mandibular symphysis is presented.

Gingival metastasis from a medullary thyroid carcinoma: case report.

BACKGROUND: Metastatic tumors to the oral cavity are rare, representing about 1% of oral tumors, and they affect jaws more often than the oral soft tissues. METHODS: Fifteen cases of metastases to the jaw bones from thyroid carcinoma were found in a recent review, with no cases located in the oral mucosa. RESULTS: The authors describe the first cases of gingival metastasis from a thyroid medullary carcinoma. CONCLUSIONS: Periodontists must recognize oral soft tissue metastases because they can be the first sign of an undiscovered malignancy, and they can be easily mistaken with several different benign lesions.

Peripheral ameloblastoma: a report of 2 cases.

Peripheral ameloblastoma (PA) is a rare odontogenic tumor most common in elderly patients. They are usually solitary; red or pink granular, nodular, or papillary lesions; up to 1.5 cm in diameter; and either not resorb bone or produce a shallow erosion. Peripheral ameloblastoma should be included in the differential diagnosis of soft tissue lumps on the gingiva or edentulous alveolus.

[Epidemiologic study of modality and state of preservation of dentures and the possible presence of mucosa lesions in subjects with removable dentures].

BACKGROUND: The replacement of lost teeth by fixed or removable dentures is very important in order to re-establish a correct phonetic and masticatory function and to restore a good aesthetical appearance to the patient. However when dental prosthesis, particularly the removable ones, become inadequate they may be responsible for a series of more or less dangerous lesions of the oral tissues. In this work a clinical and epidemiological study regarding the modality and the state of preservation of dentures in time has been carried out; moreover the possible presence of mucous membrane lesions in subjects with removable dentures has been investigated. METHODS: Seventy-height subjects with partial or totally removable dentures have been examined. After a general and stomatological anamnesis a thorough objective examination of the oral cavity was carried out in order to evaluate the removable denture state and the possible presence of mucous lesions correlated to them. RESULTS: Women from sixty to seventy represent the greatest part of the subjects with removable dentures examined in this study. Nine years is the medium range of removable denture permanence in the mouth; in the greatest part of acrylic dentures the resin has never been replaced, they are inadequate and show a poor state of preservation. Twenty-height percent of the subjects examined report different troubles and in 46% of the cases the presence of dentures causes mucous lesions. CONCLUSIONS: Since the greatest part of removable dentures examined in this study are inadequate and badly preserved, patients with removable dentures had better check them more often in order to allow the dentist to evaluate the functionality and the care of the dentures and to discover in time the possible presence of mucous lesions.
