Recurrent hyperparathyroidism due to parathyroid carcinoma.

The case is described of a woman who died with a functioning parathyroid carcinoma 19 years after removal of two parathyroid tumours, considered at the time to be benign. Following operation hyperparathyroidism subsided, with a short period of hypocalcaemia, and severe osteitis fibrosa cystica healed. Five years before death progressive renal failure developed with normal and later raised serum calcium levels. At necropsy a parathyroid carcinoma infiltrated the thyroid at the site of the previous operation and there was a solitary hepatic metastasis. A remaining parathyroid gland was nodularly hyperplastic and the bones showed evidence of active osteitis fibrosa. The relationship between parathyroid hyperplasia and neoplasia is discussed. Acceptable cases of parathyroid carcinoma are rare, but carcinoma is an important cause of recurrent hyperparathyroidism even months or years after removal of an ;adenoma'.

A year's experience with a mobile coronary resuscitation unit.

A mobile unit for the emergency treatment of cases of coronary thrombosis in the patient's home and for their supervised conveyance to hospital has been operating in the City of Newcastle upon Tyne for a year. In that time 134 cases have been attended, and of these 39 benefited appreciably from use of the unit. The value of such a unit must be balanced in relation to the current mortality of the disease outside and inside hospital and the staff available

Evaluating interviewing techniques for the sexual practices history. Use of video trigger tapes to assess patient comfort.

BACKGROUND: Although physicians recognize the importance of assessing a patient's risk for human immunodeficiency virus (HIV) infection, many are reluctant to explore a person's sexual history, a principal determinant of this risk. OBJECTIVE: To examine the feasibility of a research design that uses responses to viewing a videotaped interview as a proxy for how patients might feel if they were interviewed using a specific approach, and to determine comfort levels with a variety of interview approaches for conducting HIV risk assessment. METHODS: Individuals responded to a videotape of several interviewing approaches for HIV risk assessment. Responses to the following aspects were collected: introduction of HIV risk assessment, assessment of patient comfort with the topic, techniques for collecting HIV risk and sexual information, and exploration of sensitive issues not previously identified. Participants expressed levels of comfort by means of a Likert scale to rate their comfort with each approach. RESULTS: Participants expressed higher comfort levels with an introduction that used a ubiquity statement or lifestyle bridge question. Also, they expressed greater comfort when the interviewer addressed how they felt about responding to questions about their HIV risk. Participants reported highest levels of comfort with both patient-centered and closed-ended interviewing techniques. Women were less comfortable with an open-ended interviewing technique. CONCLUSIONS: Individuals have different comfort levels for approaches used to assess HIV risk. This trigger tape experimental design to assess comfort levels for interview techniques is feasible for exploring other aspects of physician-patient communication; additional validation studies are recommended.

Assessment of PCB estrogenicity in a human breast cancer cell line.

Lower chlorinated, ortho substituted, non coplanar polychlorinated biphenyls (PCBs) are weakly estrogenic in rodents and in some in vitro assays. The estrogenic potency of six PCB congeners and one of their para-hydroxylated metabolites have been tested in an estrogen-responsive MCF-7 human breast-cancer cell-culture system, to evaluate the utility of this system for assessment of PCB and hydroxylated PCB estrogenic activity. This assay is based on the estrogen receptor-mediated induction of postconfluent cell proliferation. The results of the limited test series were generally consistent with, but not absolute in the requirement for ortho-chlorine substitution and para-hydroxylation for estrogenic potency, demonstrating the usefulness of the MCF-7 focus assay for estrogenic structure-activity evaluation of PCBs.

Teaching "family": evaluating a multidimensional clerkship curriculum.

BACKGROUND: A multidimensional program that includes workshops on family systems, family interviewing, a home visit, and faculty precepting at East Tennessee State University (ETSU) is described and evaluated. METHODS: Over a 3-year period, ETSU junior clerks received systematic instruction on family issues in medical practice. Evaluations were obtained at the conclusion of the experience. RESULTS: The number of students who rated the importance of family in family practice as "very strong" increased from 14 (28%) to 33 (66%) between the beginning and the end of the rotation. The majority of students reported that each of the curricular elements on family had a "strong positive impact" and rated home visits as most important in promoting their understanding of families. CONCLUSIONS: The curriculum on family had a significant role in enhancing students' appreciation of the importance of the family in medicine.

Clinical expression of myotonic dystrophy: the predictive role of DNA diagnosis.

Myotonic dystrophy (DM), the most common muscular dystrophy of adult life, presents with a variety of clinical and genetic challenges to all involved; patients, their families, and clinicians. The clinical picture is extremely variable and may range from mild adult onset myotonia to severe congenital hypotonia associated with respiratory distress. An infant born to a mother with DM had remarkable hypotonia, expressionless face, respiratory difficulties, and club feet. Direct molecular genetic testing of the newborn and the mother showed trinucleotide repeat expansion mutations. Genetic counseling issues as well as the value of prenatal diagnosis are presented.
