[Reconstruction of tendon sheaths from veins in the "no-man's-land" of the hand].

A special procedure is reported for reconstruction of damaged flexor tendons in "no-mans-land" after severe crush injuries and extensive scarring. In the first stage, the scarred flexor tendons will be excised and replaced by an autogenous vein graft. A silicone rod is placed within the vein graft. In the second stage, 3 to 4 weeks later, the silicone rod is replaced with a free tendon graft. The endothelial lining of the vein tunnel provides a good gliding surface for the vein grafts. The minimal scarring permits better function.

[Angiography for the changes of arterial network in experimental tubular pedicle flaps (author's transl)].

Authors performed on rabbits experimental tubular pedicles and pedicle transfers investigating by means of fill up angiography the changes occuring in the arterial network. They conclude that in the tubular pedicles the adapting of blood supply to the changed circumstances result in transformation of arterial network. There are similar alterations after transposition of the tubular pedicles. Details of changes in vascularisation are given.

[Infected pseudoarthroses of long tubular bones. (Principles of treatment and results in the material of the University Hospital) (author's transl)].

Based on clinical experiences author takes part for principles in the treatment of infected nonunions of long tubular bones. Main characteristics of his treatment are: Baside an approach saving the blood supply of bone, stable osteosynthesis, sequestrotomy and removal of necrotic tissues (focus elimination) filling in with autologous spongy bone chips the residual bone cavities, in acute stadium the application of PMMA balls as preparatory measure for a following cancellous bone, transplantation. Author comments at the same time on drainage, rinsing and sucking. Selective use of antibiotics may complete the therapy. Data of treatment in 42 cases of infected nonunions are given.

Bone Transplantation

Reconstruction of the digital tendon-sheath in "no-man's" land with autologous transplanted vein-graft.

A two-stage procedure is described for severely damaged flexor tendons in "no man's land". In the first stage the damaged tendons and scarred tendon sheath are excised and a segment of the saphenous vein, cannulated by a silicone rubber rod is transplanted into the tendon bed. In the second stage the silicone-rod is replaced by a free transplanted tendon. The endothelial lining of the vein is a guarantee of a good gliding surface and an obstacle to adhesion formation, and so it ensures good function. A review of the method and a survey of thirty-nine cases are presented.

[Can we reduce complications of treatment of femoral neck fractures with better stabilisation of the osteosynthesis? Results of a compression-osteosynthesis (author's transl)].

The results of a compression-osteosynthesis of the fracture of the femoral neck on the evidence of 172 cases realizing the principle of a drawing-screw-compression of the AO have been reported. The results will be compared with the data of collected statistics of other types of nailing of the femoral neck and it will be established, that the better stabilisation of fractur will be manifest in the results too and will be realised in the decrease of complications (slipping of the nail, redislocation, pseudarthrosis, avascular necrosis of the head).
