Corticosteroid effects on cytokines and chemokines.

Cytokines as soluble proteins or growth factors involved in cellular interactions are major contributors to allergic and immune-mediated inflammation. Chemokines are chemoattractive cytokines subdivided into families based on cysteine residues. This review on corticosteroid effects on cytokines and chemokines will consider only a selected number of several of these proteins studied in our division and in other centers. Characteristics of several major cytokines up to IL-15, chemokine targets and the effect of corticosteroids on various cells, cytokines, and chemokines are reviewed. The effect of corticosteroid inhibitors of non-specific endothelial activation, selective activation of VCAM-1 expression, eosinophil priming and chemokine production related to allergic diseases is illustrated. The effect of nasal corticosteroids on IL-1 beta, RANTES, IL-6, and IL-8 is also discussed.

Simultaneous pulmonary leiomyosarcoma and leiomyoma in pediatric HIV infection.

The case of a 7-year-old girl with acquired immunodeficiency syndrome treated for 5 years with AZT and intravenous gamma globulin is reported. Shortly before her demise she developed a pulmonary leiomyosarcoma and leiomyoma. Does prolonged survival in pediatric acquired immune deficiency syndrome increase the incidence of secondary malignancies?

Herbal therapies: what allergist-immunologists should know regarding patient use of complementary and alternative medicine.

The use of complementary and alternative medicine has dramatically increased in the United States, which necessitates that physicians have knowledge of different modalities, possible benefits, and potential adverse reactions. Internet access and numerous magazines are available to patients for self-administration. Numerous herbal interventions have been used for asthma and allergic disorders, and conventional physicians should question their patients about their use and be knowledgeable about toxicity and scientifically well-controlled studies. This article will focus on allergic rhinitis and asthma, though numerous other therapies are utilized in complementary and alternative medicine. The allergist-immunologist should meet the challenge to understand the patient's need for alternative healing while monitoring for potential allergic reactions.
