[Radiographic evidence of small amounts of pleural effusion in infant stage individuals. Animal experiments (author's transl)].

The pleural cavity of ten young pigs was punctured and pilled with specific amounts of fluid. Subsequently chest films were obtained in ap and lateral erect and in cross-table lateral decubitus position. Only 2 cases of these pigs radiographed in the erect position (after injection of 80 and even 100 ml of fluid) gave clear evidence of pleural effusion. Indirect signs of pleural effusion (such as blunting of the costophrenic angles; unsharpness, flatting or "elevation" of the diaphragm as a sign of subpulmonic effusion), however, were already noted with only 20 ml of pleural fluid. In contrast to these exams done in the erect position, the radiographs obtained in cross table lateral decubitus position showed as little as 5 ml of fluid. Therefore, the cross table lateral decubitus position is ideal for the work up of suspected pleural effusion.

The dependence of coronary collateral blood flow on regional vascular resistances. Pharmacological studies with glyceryl trinitrate, adenosine and verapamil.

A method was applied in anesthetized dogs enabling the measurement of regional resistances up to and behind the start of collaterals and the collateral resistance. The studies show that peripheral coronary pressure, i.e. perfusion pressure of the collaterals, can change when the ratio of pre- and post-collateral resistance alters. Drugs can influence collateral blood flow not only by directly effecting the collaterals but also by altering collateral perfusion pressure. Glyceryl trinitrate given in minor doses improved collateral blood flow by directly dilating the collaterals and also by increasing collateral perfusion pressure. Higher doses did not improve collateral flow due to a decrease of collateral perfusion pressure. A steal-phenomenon occurred in some cases. Adenosine and verapamil had no direct influence on the collateral resistance. Verapamil given in small doses increased perfusion pressure slightly but not enough to improve collateral blood flow. High doses of verapamil, like low doses of adenosine, had no significant influence on collateral perfusion pressure and collateral blood flow. Adenosine given in high dosage led to a diminution of collateral flow by decreasing collateral perfusion pressure, i.e. a steal-phenomenon.

Provider attitudes toward STARPAHC: a telemedicine project on the Papago reservation.

Space Technology Applied to Rural Papago Advanced Health Care (STARPAHC), is a large-scale telemedicine project, sponsored jointly by the Indian Health Service (IHS), NASA, and the Papago tribe, and in operation on the Papago Indian Reservation outside Tucson Arizona, for the past two years. STARPAHC uses a mobile health unit (MHU), staffed by non-M.D. providers and linked by two-way television, radio, and remote telemetry to an IHS hospital up to 100 miles away, to make medical care available in remote areas of the reservation. Over a two-year-period beginning in January, 1975, 47 individual providers, including 21 physicians, were interviewed, at five intervals, to determine their receptivity to and acceptance of telemedicine; because of staff turnover, not all providers were interviewed at each different interval. Data suggests that television equipment was considered costly and in some cases inconvenient to M.D. providers; it was not considered always essential for providers to be able to diagnose and treat patients. The major problems providers cited were the unreliability of equipment and the time required for television consultations. The major benefit cited was improved access to health care for a population not previously receiving such care near their homes. Non-M.D. providers considered the link they were provided to physicians via television and voice communications from remote areas to be a major benefit.

[Simultaneous diagnostic studies of bacteriuria as related to method of urine collection].

In a direct comparative series were compared 204 functional, catheter, and mid-stream urines which were got simultaneously in patients with chronic pyelonephritis. In suprapubic vesical puncture 113 had a sterile urine, 91 showed a bacteriuria. After catheterisation 103 specimens were still sterile and 101 were bacterially infected. Of the mid-stream urines only 90 proved sterile, and 114 times pathogenic agents could be proved. While in males mid-stream urines achieved good results, in females a contamination was pretty frequent. Repeated investigations, estimation of the number of germs, and optimum taking essentially improve evidence. Though in females by means of catheterisation better bacteriological results may be achieved, in rare cases there is the danger of the invasion of germs. A clear judgment is possible, using the suprapubic vesical puncture.
