Bone and joint tuberculosis: a continuing problem.

Although tuberculous disease of bones and joints is becoming uncommon, it still occurs and may cause devastating sequelae. It is frequently not diagnosed prior to the onset of permanent damage to the joints or spine; the most important reason for this delay may be the fact that it is not considered in the differential diagnosis of monoarthritis or back pain. Most persons with the disease have other evidence of tuberculosis. Not infrequently an aggressive approach (including synovial biopsy or surgical exploration of the back) is needed to confirm the diagnosis when there are no other clues.

Hypothalamic projection to the pulvinar-LP complex in the cat: a study by the silver impregnation method.

This study was designated to determine the exact terminal portion of the hypothalamo-thalamic projection found in our previous study by the HRP method [7]. Electrolytic lesions were made in the brains of 13 adult cats, and the ensuing degenerating fibers were impregnated with Nauta-Gygax and Fink-Heimer methods. Following the lesions of the hypothalamic small nucleus which had been reported to receive abundant HRP transport from the pulvinar-LP complex, terminal degeneration concentrated in the dorsolateral marginal area of LP, forming a well-defined and dorsomedially stretched slim column along the medial border of the pulvinar. The level of its appearance approximately corresponds with the stereotaxic coronal plane, Fr. 7.0. In the contralateral same thalamic area the degenerating fibers were also traced though they were much fewer.

Alpha-fetoprotein producing rhabdomyosarcoma of the adult liver.

An autopsy case of a rare primary tumor in a 76-year-old man is described. The tumor is histologically comprised of rhabdomyoblasts massively infiltrating without any epithelial elements referable to be as hepatoma. There was no evidence that regarded the hepatic tumor as metastatic secondary tumor in histological examination of other organs. Furthermore, interesting was the production of alpha-fetoprotein from the tumor cells that was detected by immunofluorescent antibody technique. Review of the literature on primary liver rhabdomyosarcomas or adult hepatoblastomas shows no similar case.

Activities of hepatic enzymes related to ethanol oxidation and effects of ethanol on hepatic metabolites in rats with chronic liver injury.

To study the effects of ethanol on liver chronically injured by CCl4, activities of hepatic enzymes related to ethanol oxidation, influences of ethanol on hepatic metabolites, and blood ethanol disappearance were observed. (1) Activities of alcohol dehydrogenase, low- and high-Km aldehyde dehydrogenase, microsomal ethanol-oxidizing system and drug-metabolizing enzyme were remarkably decreased in the injured liver. (2) Increases in lactate/pyruvate and beta-hydroxybutyrate/acetacetate ratios were shown in control liver 2 h after ethanol ingestion. Similar but less pronounced effects of ethanol on the 'redox state' were also seen in rats with chronic liver injury. (3) Delay in ethanol disappearance was not observed until 12 h after ethanol ingestion. The ethanol-induced changes in the redox state in the injured liver were similar to those in controls. Higher ethanol concentrations in blood from rats with chronic liver injury could be related to potentiate the injured liver.

Induction of hepatic tumors in rats by senkirkine and symphytine.

The carcinogenicity of the pyrrolizidine alkaloids senkirkine and symphytine was studied in male inbred ACI rats. Animals were divided into 3 groups: Group I received ip injections of freshly prepared senkirkine at a dose of 10% of the median lethal dose (LD50) twice weekly for 4 weeks and then once a week for 52 weeks. Group II received ip injections of symphytine at a dose of 10% of the LD50 by the same injection schedule as in group I. The control group was given ip injections of a 0.9% NaCl solution following the same injection schedule as in experimental groups. All group I rats survived for more than 290 days after the start of injections, and 9 of 20 rats developed liver cell adenoma. All group II animals survived for more than 330 days after the start of injections. Of 20 rats, 4 had liver tumors, 3 had hemangioendothelial sarcomas, and 1 had liver cell adenoma. The hemangioendothelial sarcomas showed metastasis in the lungs of 2 rats. The control group had no liver tumors.

Immuno-chemotherapy of malignant lymphoma using OK-432, a streptococcal agent.

Clinical trials of immuno-chemotherapy were conducted on malignant lymphoma patients. Patients during the period from 1972 through 1977 were allocated to two groups retrospectively according to the mode of treatment, i.e., chemotherapy alone (historical control group, 35 patients) and chemotherapy with OK-432 (treated group, 15 patients). Comparisons were made of the two groups, which were homogeneous with regard to induction chemotherapy, maintenance chemotherapy, stage and histologic type of disease. The treated group had a higher remission rate, and a longer remission duration and survival than the control groups, especially in patients with Hodgkin's disease but the difference was not statistically significant owing to the limited number of cases.

Stereological analysis of Reinke's crystals in human Leydig cells.

Reinke's crystals in human Leydig cells were analyzed stereologically to assess their functional role. Testicular tissues were taken from seven older men (57--82 years old) with prostatic carcinoma and also from seven younger men (26--38 y.o.) complaining of male infertility. Sections 0.5 micrometer thick, stained with toluidine blue or Heidenhein's iron-hematoxylin were examined by a point-counting method and with a Particle Measurement Computor System (IIMC). When the patients were grouped by age, the mean crystal volume, the number of crystals per cell, the volume of crystals per cell and the volume ratio of crystals to cell were significantly larger in the older age group than in the younger age group. In particular, the latter three variables correlated well with the age of subjects, with correlation coefficients of r = 0.66--0.85. On the other hand, none of these variables had any correlation with the concentration of plasma testosterone. These results indicate that Reinke's crystals can be considered as degenerative products in cell life but not as facultative constituents for testosterone production.
