Role of right ventricular asynergy and tricuspid regurgitation in hemodynamic alterations during acute inferior myocardial infarction.

To elucidate the role of right ventricular asynergy and tricuspid regurgitation (TR) in hemodynamic alterations occurring during right ventricular infarction, left and right ventriculography with pressure measurements were performed in 22 patients with acute inferior myocardial infarction. Twelve patients with a proximal right coronary artery (RCA) occlusion (Group I) had elevated right atrial pressure (16 +/- 4 vs 8 +/- 5 mmHg, p less than 0.01), low cardiac output (2.5 +/- 0.7 vs 3.5 +/- 0.6 l/min/m2, p less than 0.05) and a greater degree of TR, compared with 10 patients with a distal RCA occlusion (Group II). Inferoposterior wall asynergy of the right and left ventricles was similar in Groups I and II. In Group I, there was an additional asynergy of the anterolateral free wall of the right ventricle. Cardiac output in those patients was not related to the left ventricular ejection fraction (EF) but to the right ventricular EF. A greater degree of TR was found in association with a lower right ventricular EF. The decrease in cardiac output was closely related to the extent of TR. These results indicate that right ventricular asynergy and TR due to proximal RCA occlusion may play important roles as causes of hemodynamic alterations seen during the acute phase of inferior myocardial infarction.

[Segmental arterio-portal chemo-embolization in hepatocellular carcinoma (cement therapy)].

When a large amount of iodized oil (LPD) is selectively administered to targeted segments of the liver, the tumor vessels and sinusoids around the tumor are filled with LPD, subsequently regurgitating excessive LPD via the hepatic arterioportal communication into the portal branches surrounding the tumor. Segmental arterioportal chemoembolization in HCC with Doxorubicin-in-oil emulsion (DOE), which we named cement therapy, was performed in 18 patients with HCC. DOE was administered selectively into one segment division of Couinaud in ten patients, two segments in seven and three segments in one. Computed tomography and ultrasonography following the cement therapy revealed an overall response rate of 39%. The one-year cumulative survival rate was 80%, and that for three-years was 33%. Three cases underwent segmental hepatectomy and these specimens revealed total necrosis of the main tumor and whole daughter nodules, followed by various degrees of hepatic parenchymal infarction. This cement therapy realizes the simultaneous chemoembolization via the artery and portal vein, and was considered an effective treatment for extra-capsular infiltration and daughter nodules of HCC nourished by the portal vein blood flow. Limited administration of DOE to the targeted segments enables transarterial oily chemoembolization (TOCE) without any serious side effects.

[Myocardial perfusion assessed by dynamic computed tomography, with respect to viable muscle in the infarcted region].

To clarify the significance of regional myocardial perfusion, 31 patients of old myocardial infarction including 11 cases undergoing PTCA with a left anterior descending artery lesion were studied using dynamic transmission computed tomography with excellent time resolution. Serial one-second dynamic scans with an electrocardiographic triggering system were performed at the middle level of the left ventricle using a bolus injection of contrast medium via the inferior vena cava. The F/V (F = flow, V = volume) ratio, a parameter of perfusion per unit of myocardium, was calculated from gamma-variate fitted time density curves obtained in the myocardium and in the left ventricular cavity. The F/V ratio was significantly lower in patients not only with severe but also mild and no coronary artery stenosis (post PTCA: 185 +/- 54, 50-75% stenosis: 193 +/- 47, 90% stenosis: 181 +/- 51, 99% stenosis: 140 +/- 34, 100% stenosis: 142 +/- 27 ml/min/100 g, control value: 243 +/- 51 ml/min/100 g, post PTCA, 50-75% stenosis, 90% stenosis vs control p less than 0.05, 99%, 100% stenosis vs control p less than 0.005). The functional images depicting myocardial perfusion frequently revealed abnormal perfusion findings in patients not only with severe but also mild and no coronary stenosis. In the patients with mild or insignificant coronary stenosis, the F/V ratio was dependent on the severity of left ventricular wall motion abnormalities (hypokinesis: 192 +/- 51, akinesis or dyskinesis: 141 +/- 32 ml/min/100 g; p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of PTCR and PTCA on serial changes in left and right ventricular wall motion in acute myocardial infarction].

To evaluate the effects of early recanalization on left and right ventricular wall motion in acute myocardial infarction (AMI), we serially observed their degrees in 66 patients with AMI. The patients were categorized as Group 1:17 with spontaneous recanalization within 6 hours of onset of the chest pain; Group 2:34 with effective recanalization within 6 hours (10 by PTCR, 10 by PTCR+ PTCA, and 14 by direct PTCA), and Group 3:21 without effective recanalization. The Group 2 patients were classified in 3 subgroups according to the time intervals from onset of symptoms to recanalization; 11 patients with recanalization within 2 hours (Group 2a), 10 between 2 and 4 hours (Group 2b), and 13 between 4 and 6 hours (Group 2c). The left and right ventricular wall motion abnormality indexes (WMAI) were defined as the means of point scores for the degrees of regional wall motion abnormality at 11 segments of the left ventricle and seven segments of the right ventricle on serial two-dimensional echocardiograms. Results were as follows: 1. The LV-WMAI of Group 1 was smaller on day 1, and improved on day 28 as compared to those of the other groups (0.63 +/- 0.35 to 0.18 +/- 0.18, p less than 0.001). 2. The improvements of the LV-WMAI from days 1 to 28 in Group 2a (WMAI: 1.01 +/- 0.57 to 0.26 +/- 0.26, delta WMAI: 82 +/- 14%) and Group 2b (1.03 +/- 0.38 to 0.52 +/- 0.48, 54 +/- 36%) were greater than that in Group 2c (1.01 +/- 0.46 to 0.64 +/- 0.52, 38 +/- 47%).(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of teratoma of the posterior mediastinum].

We reported a case of teratoma of the posterior mediastinum, whose common location in the mediastinum is anterosuperior portion. A 10-year-old female with teratoma of the posterior mediastinum received thoracotomy in our department. Magnetic Resonance Imaging was useful in this case for the clinical diagnosis of the nature of tumorous shadow using T1-weighted, T2-weighted SE images and IR images.

Structure of cCF10, a peptide sex pheromone which induces conjugative transfer of the Streptococcus faecalis tetracycline resistance plasmid, pCF10.

The peptide pheromone, cCF10, which induces aggregation and high frequency plasmid transfer in Streptococcus faecalis cells carrying the tetracycline resistance plasmid, pCF10, was isolated and its structure determined. The molecular weight of cCF10 is 789, and its amino acid sequence is H-Leu-Val-Thr-Leu-Val-Phe-Val-OH. Pheromone activity, as determined by a clumping induction assay, was detectable at a concentration of 2.5 x 10(-11) M. A peptide of the same sequence as that of the cCF10 produced by S. faecalis cells was synthesized by the liquid-phase method. The synthetic pheromone showed biological activity and chromatographic behavior that was identical to that of the cCF10 of bacterial origin. When the response of S. faecalis cells to various concentrations of synthetic cCF10 was monitored by measuring both the frequency of plasmid transfer and the synthesis of pheromone-inducible antigens, an excellent correlation was observed between donor ability and the appearance of a 150-kilodalton protein that appears to be involved in formation of mating aggregates. The dose-response data in the range of concentrations where the amount of pheromone became limiting (10(-11)-10(-12) M) were consistent with the notion that as few as one or two molecules per donor cell may be sufficient to induce a mating response.

Increased 201Tl uptake by the chest wall following cardioversion.

A patient with acute inferior infarct is described who showed increased 201Tl uptake in the anterior chest wall following cardioversion which mimicked increased lung uptake of the tracer. The site of accumulation of 201Tl corresponded to that of 99mTc-pyrophosphate (PYP) and the right sided catheterization revealed almost normal hemodynamic data.

Purification and characterization of salmolysin, an extracellular hemolytic toxin from Aeromonas salmonicida.

An extracellular hemolytic toxin of Aeromonas salmonicida, termed salmolysin, was purified 945-fold by ammonium sulfate precipitation, anion-exchange chromatography on DEAE-cellulose, and gel filtration chromatography on Sephadex G-100 and Sepharose 2B. Salmolysin appeared homogeneous upon cellulose acetate membrane electrophoresis and immunodiffusion analysis. The molecular weight of the toxin was estimated to be approximately 200,000 by the sedimentation equilibrium method. The UV absorption spectrum showed a maximum at 275 nm and a minimum at 262 nm. The isoelectric point was found to be at pI 5.4. Carbohydrate and protein analyses and other biochemical data indicated that salmolysin is a glycoprotein, containing approximately 62% carbohydrates. The toxin is a heat-labile substance and is stable at a neutral pH value. Ferrous ion inhibited the activity, whereas metal-chelating agents did not affect the activity. Sulfhydryl reagents did not inhibit the toxin, whereas reducing agents, such as L-cysteine and reduced glutathione, inhibited the toxin to a certain extent. Salmolysin was inactivated by a nonionic detergent but was stimulated by an anionic detergent, sodium deoxycholate, at a low concentration. The toxin was also inactivated by subtilisin and trypsin but was not inhibited by papain and pepsin. Salmolysin, with a remarkable hemolytic activity against salmonid erythrocytes, was lethal to rainbow trout when it was injected intramuscularly.

Study on the receptor subtypes mediating the analgesic action of an enkephalin analog, Tyr-D-Met(O)-Gly-EtPhe-NHNHCOCH3.AcOH (EK-399).

The analgesic action of the enkephalin analog EK-399 (Tyr-D-Met(O)-Gly-EtPhe-NHNHCOCH3.AcOH) and the subtypes of the opiate receptors mediating the action were studied. The analgesic effect of subcutaneously injected EK-399 was ten times as potent as that of morphine in the rat tail flick test. EK-399 had a longer latency time and duration time than morphine. The analgesic action of EK-399 injected into the rat spinal subarachnoid space was about 800 (1800 in molar ratio) times as potent as that of morphine in the hot plate test. EK-399 had high affinities for both mu and delta opiate receptors in the rat brain receptor binding assay. The apparent pA2 values with naloxone were 7.65 for morphine and 5.98 for EK-399 in the rat tail flick test; the difference was significant. A cross tolerance between EK-399 and morphine was examined in the rat tail flick test. Although morphine tolerant rats showed no tolerance to EK-399, EK-399 tolerant rats showed a clear tolerance to morphine. These results indicate that EK-399 has a potent and long lasting analgesic effect via opiate receptors in rats. In addition to mu-receptors, delta-receptors may be involved in its analgesic mechanism.
