[The effect of intra-aortic balloon pumping on the graft of coronary artery bypass grafting].

The effect of intra-aortic balloon pumping (IABP) on the graft of coronary artery bypass grafting (CABG) was examined. These patients needed IABP when they were weaned from cardiopulmonary bypass. The flow wave pattern in the graft was acquired and analyze by the slope, peak flow rate and area of systolic phase and diastolic phase. The flow wave pattern different between graft to a left anterior branch and the graft to the right coronary artery. IABP was significantly effective for the flow to the left anterior branch. However, the effect of IABP on the graft flow to the right coronary artery was not significant. In the natural heart, the flow rate of the left coronary artery predominates during the diastolic phase. However this predominance is not clear in the right coronary artery. Our results suggested that this phenomenon in the natural state caused a difference the flow wave pattern between graft of left coronary artery and that of the right coronary artery.

[Prevalence of specific IgE antibodies for Dermatophagoides farinae and six inhalants in the general population].

A seroepidemiologic study of house dust allergy and pollinosis was performed among a general population aged 17 to 69 in Aichi Prefecture, Japan. The prevalence of blood containing IgE antibodies to four perennial antigens and three seasonal antigens was examined among 198 persons whose serum total IgE antibodies were above 40 IU/ml. The prevalence of blood containing IgE antibodies was from 9 to 18 percent among perennial antigens and from 6 to 30 percent among seasonal antigens. These results show that Dermatophagoides farinae, Cryptomeria japonica pollen and Anthoxanthumodoratum pollen are important antigens in this population.

Pulsed-field gel electrophoretic analysis of Campylobacter jejuni DNA for use in epidemiological studies.

The cleavage patterns of the genomic DNA of 42 clinical isolates of Campylobacter jejuni determined by pulsed-field gel electrophoresis (PFGE) were compared with their Lior and TCK serotypes. The fragment patterns of DNA obtained with SalI and SmaI restriction enzymes did not always accord with the corresponding serotypes but strains of the same serotype could be further divided into subtypes by their cleavage patterns. This PFGE method may prove useful for subclassifying C. jejuni.

Production of a monoclonal antibody inhibiting the killer activity.

We established the hybridoma producing the monoclonal antibody (mAb) 3C3 by immunizing rats with mouse natural killer (NK)-like cells. The 3C3 mAb seemed to react mainly with T cells and T-lineage cell lines. The 3C3 antigen also seemed to be coincidentally expressed on a part of asialo GM1+ cells from nude mice, suggesting its expression on NK cells. Treatment of effector cells with 3C3 mAb markedly inhibited the killer activity against RL male-1 cells, but less so against YAC-1 cells, in vitro. It is suggested that the cell surface molecule defined by 3C3 mAb was closely associated with the killer activity of T cells and NK cells.

Detection of Clostridium perfringens enterotoxin gene by the polymerase chain reaction amplification procedure.

The polymerase chain reaction (PCR) procedure was evaluated to see if it is a simple and rapid method to detect Clostridium perfringens enterotoxin gene. The method, involving the use of two synthesised primers and gene amplification by the PCR procedure, detects a DNA fragment of 364 base pairs of C. perfringens enterotoxin gene by gel electrophoresis. The enterotoxin gene of strains was detected by use of purified chromosomal DNA. The supernatant of sporulating cultures in a sporulating medium was able to be used as template DNA. Template DNA can be obtained by merely culturing the strain in DS medium, a sporulating medium, for 48 h at 37 degrees C. All C. perfringens strains showing positive results in the PCR procedure were demonstrated to produce enterotoxin by a conventional method and all strains showing negative results were enterotoxin negative. To detect the enterotoxin gene in stool specimens by the PCR procedure, the specimen was heat-treated for 10 min at 90 degrees C and cultured in a sporulating medium, the supernatant of which was used as template DNA. From the stool specimens showing positive results in the PCR procedure by this method, enterotoxigenic C. perfringens was isolated from the heat-treated specimens. Thus, it is possible to detect enterotoxigenic C. perfringens in stools without isolation of the organism.

Properties of perception of laterally moving objects in persons with profound retardation.

Twenty-two persons with profound retardation were assessed for their perception of moving objects at a velocity of 5 degrees s-1. Abnormalities were not found in the ocular structures and optic discs of subjects upon ophthalmological examination. Stimuli consisted of six habituation trials and two test trials. Duration of eye movement in pursuit of stimulus was estimated by calculating the linearity and gradients of electro-oculograms. Subjects were divided into three groups based on the change of estimated time of pursuit eye movement. Those showing habituation as well as dishabituation (group 1) had higher developmental communicative ages than subjects with only habituation (group 2) and subjects who did not exhibit habituation (group 3). The results indicated a relationship between the perception of moving objects and the development of communication level in persons with profound retardation. The present study investigated abnormalities in the computer tomographic scanning of the subjects' heads and found cerebral disturbance of the visual cognition of moving objects in persons with profound retardation.

[Serological studies on Campylobacter jejuni/coli: establishment of national reference system for serological typing in Japan].

In order to establish a national reference system for Campylobacter serotyping in Japan, 7 local institutes of public health collaborated to prepare 30 serogrouping antisera including 26 antisera of Lior's serogrouping system and 4 antisera of TCK serogrouping system which was developed by the Tokyo Metropolitan Research Laboratory of Public Health. A total of 603 strains (92.2%) out of 654 isolates from 23 outbreaks of C. jejuni throughout Japan were serogrouped by the 30 antisera. Out of 1,198 strain isolated from sporadic cases of Camplyobacter gastroenteritis, 883 (73.7%) were typed and 298 strains (24.9%) were untypable. The remaining 17 strains belong to rough form were not used for serogrouping. A hundred thirteen out of 883 strains have reacted with more than one typing serum. Among C. jejuni isolated from 7 prefecture, Lior's serogroup 4 was most common followed by Lior's serogroup 4, 2, 11, 1 and TCK serogroup 1 and 12. We conclude from the experiment described above that this Lior's serogrouping system by combining with TCK serogroup for Campylobacter jejuni/coli is useful in epidemiological investigations in Japan.

[An outbreak of food poisoning suspected due to Aeromonas and characteristics of the isolated strains].

On June 7, 1990, food poisoning with main symptoms of abdominal pain and diarrhea occurred in Inuyama City, Aichi Prefecture. From results of bacteriological examination, three kinds of mesophilic Aeromonas spp. were detected from patients, leftover foods, well water, and cookers, one of which was A. hydrophila which was shown to produce haemolysin thought to be the cause of the food poisoning. On the other hand, A. sobria and A. caviae isolated from various materials did not produce haemolysin. The latter two strains of mesophilic Aeromonas spp. did not produce any other enterotoxins. Therefore, it appears that the well water containing A. hydrophila, A. sobria and A. caviae polluted the foods, which then had caused the food poisoning. Of nine strains of A. hydrophila, five do not dissolve sucrose, and these are typed as serogroup O:22 or O:23. The other four strains dissolve sucrose, and these are typed as serogroup O:16. According to the drug sensitivity test, all of these nine strains of A. hydrophila were resistant to Ampicillin, Erythromycin and Cephaloridine.

Establishment of hybridoma cells with natural killer(NK)-like activity against syngenic tumor cells.

The 4D1D4 hybridoma cells were derived from the fusion of spleen cells from BALB/c nude mice with NS-1 mouse myeloma cells. The surface phenotypes of 4D1D4 hybridoma cells were Thy-1.2+, L3T4 (CD4)-, Lyt-2 (CD8)-, Asialo GM1+ and p-55 interleukin-2 (IL-2) receptor (CD25)-. This phenotypic pattern was consistent with the surface phenotype of NK cells. The 4D1D4 cells showed the definite killer activity against a syngenic tumor cell line, RL male-1, but not against an allogenic YAC-1 line. The killer activity of the 4D1D4 cells was not affected by the addition of exogenous IL-2. It was, therefore, suggested that 4D1D4 cells might be representative of resting NK cells with expression of no functional IL-2 receptors. The hybridoma technology might be useful for establishment of the cloned NK cells.

[Rapid detection of motile Salmonella in chicken meat by the 1-2 Test].

The 1-2 Test (BioControl) is an enrichment culture/immuno-diffusion assay for detecting Salmonella in foods by use of poly-H-antibodies. The performance of the 1-2 Test was compared with that of the traditional culture method on 85 chicken meat samples. The results were as follows: The detection frequency of Salmonella by the 1-2 Test 24-hour assay (28/85, 32.9%) was greater than that by the 8-hour assay (16/85, 18.8%) and almost equal to that by the traditional method (26/85, 30.6%). The 8-hour assay gave a 46.2% sensitivity and 93.2% specificity. The 24-hour assay gave an 84.6% sensitivity and 89.8% specificity. Detection appeared to show some influence from the number of Salmonella, number of viable cells (SPC) and coliform groups. Differences in serotypes of Salmonella detected by the 1-2 Test and the traditional methods were also observed. Although there were some differing results between the 1-2 Test and the traditional method, this system appears to be useful for detecting Salmonella in foods, since results can be obtained at least 24 hours earlier than by the conventional procedures.

In vitro studies on steroidogenesis in the presence of pregnenolone as precursors by the follicular tissue of the domestic fowl (Gallus domesticus).

Chicken granulosa and theca cells were isolated from F1 and F4-6 follicles 2-4 h before ovulation, and the amounts of progesterone, testosterone and oestradiol released in the medium during incubation for 3 h, in the presence or absence of pregnenolone as a percursor and stimulatory drugs or inhibitory drugs, were measured. Progesterone synthesis by granulosa cells was stimulated with oLH or theophylline. Much more progesterone was synthesized when pregnenolone was added to the medium. The amount of testosterone produced by the granulosa cells was similar to that produced by the theca cells. The production of testosterone was increased by the addition of oLH or theophylline. Oestradiol synthesis by F4-6 follicles was higher than by F1 follicles, and it was higher in the theca cells than in the granulosa cells. The addition of oLH or theophylline increased oestradiol synthesis in the theca cells and the granulosa cells of F4-6 follicles. The results indicate that oestradiol can be produced from pregnenolone by the theca cells alone. It is possible, however, that the theca cells also take in the precursors for the production of oestradiol from the granulosa cells.

[Cholera toxin producibility by Vibrio cholerae isolated during the cholera outbreak in the NTT Nagoya Hall].

An outbreak of cholera occurred among guests of the NTT Nagoya Hall in September 1989. Clinical findings showed that all but one were symptomatic infections out of 44 bacteriologically confirmed cases. In relation to the high incidence of symptomatic infections, we examined cholera toxin (CT) producibility of the isolated V. cholerae. 1. Strains of the NTT case produced 16-256 (mean 130) ng of CT per ml in CAYE-L medium at 30 degrees C and 32-256 (mean 142) ng of CT per ml by Polymyxin B treatment. But strains of past case produced 8-256 (mean 34), 8-128 (mean 44) ng of CT per ml, respectively. Strains of the NTT case produced a larger amount of CT than that of the past cases. 2. Strains of the NTT case produced 512-4096 (mean 2100) ng of CT per ml in CAYE-L medium at 37 degrees C and 1024-2048 (mean 1600) ng of CT per ml by Polymyxin B treatment. But strains of the past case produced 8-64 (mean 25), 8-128 (mean 45) ng of CT per ml, respectively. Strains of the NTT case produced a larger an amount of CT than them of past case. We observed the same phenomenon in AKI medium at 37 degrees C. The yield of CT in CAYE-L medium was greater at 37 degrees C than 30 degrees C. 3. Strains of NTT case grew faster than that of the past case in CAYE-L medium at 37 degrees C. But the growth rate was the same as both strains in AKI and CAYE media.(ABSTRACT TRUNCATED AT 250 WORDS)

[Assessment and treatment of respiratory dysfunction in severely handicapped children: respiratory disorders during sleep].

In severely handicapped patients, chronic respiratory insufficiencies and recurrent hypoxemia especially during sleep are one of the major problems to take care of them. To clarify the pathophysiology of respiratory disorders during sleep, we examined 5 severely handicapped patients with no respiratory distress during wakefulness, but with signs of latent respiratory insufficiency by means of polygraphical all-night study including the arterial oxygen saturation (SaO2) monitoring. In the severely handicapped patients, sleep hypoxemia occurred frequently. The value of SaO2 decreased in proportion to the intensity of apnea determined by polygraph. The relationship between the value of SaO2 and sleep stage was variable in each patient. Four patients had mainly obstructive respiratory disorders and one had a central type respiratory disorder. The density of REM stage decreased in the obstructive type. In the central type, the repeat of REM stage with normal REM density was not observed. In one patient with obstructive type and one with central type, hypoxemia was improved with an increased number of REM stage by O2 inhalation.
