The relationship of magnetic source imaging to ictal electrocorticography in a neuronavigational workspace.

Magnetic source imaging (MSI) registers magnetoencephalographic (MEG) activity to a three-dimensional MRI volume. State-of-the-art MSI allows concurrent whole head coverage, but is practically restricted to interictal recording. However, the purpose of the presurgical evaluation of epileptic patients, in which MSI is playing an increasing role, is the elucidation of the ictal epileptogenic focus. The manner in which interictal MSI activity relates to the ictal focus has not yet been adequately examined. To facilitate this analysis, we are developing techniques to precisely coregister MSI to the ictal onset zone as defined by extraoperative intracranial grid/strip monitoring. The neuronavigational workspace is a convenient area in which to precisely coregister these (and other) imaging and physiological data sets.

Magnetoencephalographic patterns of epileptiform activity in children with regressive autism spectrum disorders.

BACKGROUND: One-third of children diagnosed with autism spectrum disorders (ASDs) are reported to have had normal early development followed by an autistic regression between the ages of 2 and 3 years. This clinical profile partly parallels that seen in Landau-Kleffner syndrome (LKS), an acquired language disorder (aphasia) believed to be caused by epileptiform activity. Given the additional observation that one-third of autistic children experience one or more seizures by adolescence, epileptiform activity may play a causal role in some cases of autism. OBJECTIVE: To compare and contrast patterns of epileptiform activity in children with autistic regressions versus classic LKS to determine if there is neurobiological overlap between these conditions. It was hypothesized that many children with regressive ASDs would show epileptiform activity in a multifocal pattern that includes the same brain regions implicated in LKS. DESIGN: Magnetoencephalography (MEG), a noninvasive method for identifying zones of abnormal brain electrophysiology, was used to evaluate patterns of epileptiform activity during stage III sleep in 6 children with classic LKS and 50 children with regressive ASDs with onset between 20 and 36 months of age (16 with autism and 34 with pervasive developmental disorder-not otherwise specified). Whereas 5 of the 6 children with LKS had been previously diagnosed with complex-partial seizures, a clinical seizure disorder had been diagnosed for only 15 of the 50 ASD children. However, all the children in this study had been reported to occasionally demonstrate unusual behaviors (eg, rapid blinking, holding of the hands to the ears, unprovoked crying episodes, and/or brief staring spells) which, if exhibited by a normal child, might be interpreted as indicative of a subclinical epileptiform condition. MEG data were compared with simultaneously recorded electroencephalography (EEG) data, and with data from previous 1-hour and/or 24-hour clinical EEG, when available. Multiple-dipole, spatiotemporal modeling was used to identify sites of origin and propagation for epileptiform transients. RESULTS: The MEG of all children with LKS showed primary or secondary epileptiform involvement of the left intra/perisylvian region, with all but 1 child showing additional involvement of the right sylvian region. In all cases of LKS, independent epileptiform activity beyond the sylvian region was absent, although propagation of activity to frontal or parietal regions was seen occasionally. MEG identified epileptiform activity in 41 of the 50 (82%) children with ASDs. In contrast, simultaneous EEG revealed epileptiform activity in only 68%. When epileptiform activity was present in the ASDs, the same intra/perisylvian regions seen to be epileptiform in LKS were active in 85% of the cases. Whereas primary activity outside of the sylvian regions was not seen for any of the children with LKS, 75% of the ASD children with epileptiform activity demonstrated additional nonsylvian zones of independent epileptiform activity. Despite the multifocal nature of the epileptiform activity in the ASDs, neurosurgical intervention aimed at control has lead to a reduction of autistic features and improvement in language skills in 12 of 18 cases. CONCLUSIONS: This study demonstrates that there is a subset of children with ASDs who demonstrate clinically relevant epileptiform activity during slow-wave sleep, and that this activity may be present even in the absence of a clinical seizure disorder. MEG showed significantly greater sensitivity to this epileptiform activity than simultaneous EEG, 1-hour clinical EEG, and 24-hour clinical EEG. The multifocal epileptiform pattern identified by MEG in the ASDs typically includes the same perisylvian brain regions identified as abnormal in LKS. Dose-dependent effects of Cerebrolysin on EEG and short-term memory of healthy volunteers during control and hyperventilation induced cerebral ischemia.

Cerebrolysin, a peptidergic nootropic drug, was to be effective on learning and other cognitive functions in animal experiments as well as in clinical studies. Hyperventilation (HV) as a model of brain ischemia induces slowing of the EEG and cognitive impairment. The aim of this study was to examine the potential dose-dependent effects of Cerebrolysin on HV related EEG changes and short term memory during chronic (10 days) application and the time dependency of these effects. In this single centre, double blind, randomized, placebo-controlled, parallel group study 48 healthy males were enrolled and received either 100 ml placebo (NaCl) or Cerebrolysin (10 ml or 30 ml or 50 ml) in a volume of 100 ml (NaCl) for 10 days. EEG at baseline and during HV as well as the cognitive performance was evaluated at Day 1 (baseline, 15 min p.i., 2 h p.i., 4 h p.i., 8 h p.i., 24 h p.i.), Day 10 (baseline, 15 min p.i., 2 h p.i.,) and at day 11 (24 h. after the last infusion). The main effects found during the study can be summarized as follows: At baseline we found an increase of the EEG power ratio (PR) for the grouptrated with 10 ml Cerebrolysin. The effect was most pronounced at the parietal cortex. The effect started after 15 min, was most expressed at 2 h and was kept until 8 h. During HV we found a relative PR decrease of the group (10 ml Cerebrolysin) at 2 hours. For short term memory, there is a trend towards less effective word recall for the baseline situation during the first 4 hours for the placebo. This effect was not observed in the Cerebrolysin treated groups. If chronic effects are concerned, the PR increased over the parietal regions at 24 h for the groups treated with 10 and 30 ml Cerebrolysin. The effect remains at day 10 and 11. But at 10 and 11 days there was also a trend for a relative increase of the PR in the group treated with 50ml Cerebrolysin. Signs of overdosage occurred with the highest concentrations of Cerebrolysin. The events were only mild and caused no harm to the volunteers. The highest concentration caused a small but significant reduction of blood pressure. The effects could be interpreted as those of an atypical nootropic with anti-ischemic properties.

Magnification survey and spot view mammography with a new microfocus X-ray unit: detail resolution and radiation exposure.

The aim of our study was to evaluate a mammography unit capable of magnification of up to fourfold at an equivalent or lower dose than with current systems. A prototype mammography tube with an electron-beam-focusing technology resulting in a focal spot size of 40-120 micro m was combined with a highly intensifying screen-film system. To evaluate contrast-detail resolution, phantom radiographs were performed with the prototype magnification mammography system using a magnification factor of 1.7 for survey views and a magnification factor of 4.0 for spot views. They were compared with unmagnified survey views and magnification spot views (magnification factor 1.9) of a state-of-the-art mammography system. The radiation exposure was measured and mean glandular doses were calculated. The contrast-detail resolution with both prototype (m = 1.7) and conventional (m = 1.1) survey views was equivalent while the entrance dose and the mean glandular dose were approximately 50 % lower with the prototype. For spot views, the contrast-detail resolution was substantially higher for the prototype than for conventional magnification while the dose was equivalent. Dose reduction and improved detail resolution are possible with this new technology.

[A new specimen radiography device with a maximum 20-fold magnification for breast diagnosis].

PURPOSE: A radiography system specially developed for specimen radiography and allowing maximal 20-fold magnification is presented. The efficiency of the system was tested and compared with that of conventional magnification mammography systems. METHODS: 23 surgical and 90 core biopsies of the breast were examined for detection of microcalcifications. As criteria the number of identifiable calcifications, their shape and configuration as well as tissue contrast were chosen. RESULTS: The new technique detected about 400% more microcalcifications, 200% more core and 50% more surgical biopsies containing calcifications. Thus, in a few cases, additional core biopsies were unnecessary. Moreover, this new system yielded additional information for the pathologist and surgeon concerning the exact localisation of suspicious lesions that facilitated working up specimens, or indicated additional surgical removal in special cases. CONCLUSIONS: By identification of malignant lesions not detectable with conventional magnification radiography systems, as well as a more exact localisation of suspicious lesions, false negative results may be reduced.

[Determination of contour clarity in contrast-enhanced MR angiography: definition and clinical evaluation exemplified by ECG-triggered imaging of the thoracic aorta].

PURPOSE: The sharpness of the vessel contour in 3D contrast-enhanced MR angiography is defined by particular measurements of the signal intensity. To evaluate the facilities of an ECG-triggered data acquisition to optimize the MRA image quality of the thoracic aorta. METHOD: In phantom studies the sharpness of the vessel contour was defined as the distance of the extraluminal signal-minimum to the extraluminal maximum. Afterwards 30 examinations of the thoracic aorta with and without the use of an ECG-triggering were evaluated with regard to the sharpness of the vessel outline. RESULTS: The presented definition of the signal intensity is suitable for objective measurements of the vessel outline. At the level of the ascending aorta, ECG-triggered data acquisition increased the image quality significantly. CONCLUSIONS: An easy, practicable definition enables measurements of the sharpness of the vessel outline. ECG-triggered data acquisition is recommended for optimal visualization, especially of the ascending aorta.

Quantitative topographical analysis of EEG during nonstandardized and standardized hyperventilation.

The aim of this study was to compare the topographical quantitative EEG (qEEG) changes induced by nonstandardized hyperventilation and those induced by standardized hyperventilation (with the end-tidal PCO2 being maintained at 2 kPa [15 mm Hg]). We examined 18 healthy volunteers during nonstandardized and 20 during standardized hyperventilation. During nonstandardized hyperventilation, the mean spectral power density in this group significantly increased 1.9 fold within the delta-, 2.2 fold within the theta-, 1.8 fold within the alpha-, and 1.9 fold within the beta-frequency band. There was no significant change of the power ratio and was no topographic difference between 4 frequency bands investigated. During standardized hyperventilation, the mean spectral power density in the group significantly increased to 12.9 fold within the delta-, to 7.6 fold within the theta-, to 1.4 fold within the alpha-, and to 2.4 fold within the beta frequency band. The power ratio decreased significantly. Such a pronounced EEG slowing with delta and theta augmentation was never found during nonstandardized hyperventilation. We conclude that a consistent slowing of the qEEG in all leads including a constant topographical maximum can only be induced by standardized, sufficiently pronounced hyperventilation.

Compression device to reduce motion artifacts at contrast-enhanced MR imaging in the breast.

A prototype compression device for a bilateral breast surface coil was manufactured. It was tested in clinical practice (157 patients), and resultant images were compared with those obtained with simple ventral stuffing of the bottom of the coil (181 patients). Compression resulted in reduction of magnetic resonance (MR) examinations with severe motion artifacts from 20 (11%) of 181 examinations to three (2%) of 157 examinations (P = .001). Use of this compression device allowed the subtraction technique at MR imaging in the breast to become an effective method for ensuring more lesion conspicuity.

Storage phosphor direct magnification mammography in comparison with conventional screen-film mammography--a phantom study.

Contact mammography with current photostimulable storage phosphors is hampered by its low spatial resolution. Detail visualization can be improved by geometric magnification radiography which enlarges small details to exceed inherent image noise. This study compares storage phosphor mammography using a dedicated direct magnification system with state-of-the-art conventional screen-film mammography. Storage phosphor direct magnification survey views (1.7x) and spot views (4x) were obtained with a prototype mammography unit providing focal spot sizes of 120-40 microns. Conventional technique screen-film survey views (1.1x) and spot views (1.8x) served as comparison. A contrast detail study and a receiver operating characteristic (ROC) analysis using an anthropomorphic breast phantom with superimposed microcalcifications was performed. Contrast detail resolution in the digital and conventional survey views were equivalent. For the spot views, contrast detail resolution was significantly higher with the digital technique (p < 0.001). ROC analysis of 400 observations demonstrated a significantly higher performance (p < 0.001) with digital images versus conventional screen-film mammograms. The area under the ROC curve (Az) in the digital survey views was 0.76 +/- 0.07 versus 0.59 +/- 0.02 in the conventional technique. In digital spot views, Az was 0.82 +/- 0.07 as compared with 0.66 +/- 0.04 in the conventional spot views. These results suggest that storage phosphor digital mammography in conjunction with direct geometric magnification technique may be superior to conventional screen-film mammography in the detection of microcalcifications.

[Does tissue perfusion after free microvascular tissue transplantation stay autonomous?].

In a prospective study including 60 patients after free latissimus dorsi transfer (FLDT) to the lower leg, we found persistence of pedicle blood flow up to 10 years postoperatively. After uncomplicated FLDT the pedicle supported the flap in all cases, whereas after complicated FLDT (hematoma, thrombosis, infection) we found only 50% of all flaps autonomously perfused by the vascular pedicle.

[Results of a standardized treatment concept in primary hyperparathyroidism].

BACKGROUND AND OBJECTIVE: Primary hyperparathyroidism (PHPT) is being diagnosed ever more frequently. After diabetes mellitus, ovarian dysfunction and metabolic disorders it is now among the most frequent metabolic disease. Its surgical treatment has become standardized with increasing experience. A prospective study was undertaken to clarify to what extent our own standardized diagnostic and therapeutic measures conform to current standards in literature. PATIENTS AND METHODS: During 10 years (1. 1. 1987-31. 12. 1996) 478 patients with PHPT were treated surgically (solitary adenoma: 317 (76.5%), multiple lobe hyperplasias 66 (15.9%), double adenoma 28 (6.8%), and carcinoma 4 (0.8%). PHPT was part of the MEN (multiple endocrine neoplasia) syndrome in 20 patients. Typical symptoms of PHPT had been present in 400 patients (83.7%), while 78 (16.3%) had no manifest symptoms. Ultrasound examination of the soft tissues was the only preoperative test of localization. More extensive preoperative diagnosis was practised only before reoperations. All patients had bilateral exploration of the neck to try and demonstrate all four epithelial bodies. RESULTS: A primary operation normalized the calcium level in 405 of 414 patients (97.8%). In 64 previously unsuccessfully operated patients who were reoperated for persisting HPT the elevated calcium levels could be normalized in 90.6%. Hypercalcaemia was finally abolished in all 478 patients. Average follow-up duration for 319 patients was more than 4.8 (0.5-8.5) years. The perioperative mortality rate was 0.8 (4 of 478), with 41 deaths during the later follow-up period. Permanent recurrent laryngeal nerve palsy occurred in 6 patients (1.8%). Long-term calcium substitution was necessary postoperatively in four patients (1.2%). CONCLUSION: Our findings support a liberal policy towards indication for bilateral surgical exploration of the neck in PHPT.

[Digital magnification mammography in computed radiography. Initial clinical results].

INTRODUCTION: The combination of direct magnification mammography and computed radiography provides an improvement in spatial resolution of storage phosphor-based digital systems. A clinical study comparing conventional and digital direct magnification mammograms was performed. METHODS: 100 survey mammograms in 1.5-or 1.7-fold magnification and 50 4-fold spot magnification views were obtained with a prototype direct magnification mammography system and a storage phosphor-based digital system. An intraindividual comparison of these with previous conventional radiograms of the same patients was carried out. RESULTS: The diagnostic value of digital survey mammograms using the direct magnification technique is comparable to that of conventional radiograms of the breast, especially with regard to the identification of microcalcifications and lesions and the clinical consequences. Spot magnification views performed with this combination of techniques allowed improvement in the evaluation of microcalcifications. In 15% of cases, diagnostic procedures were adjusted accordingly. CONCLUSION: The combination of the direct magnification technique with digital storage phosphor radiography systems allows the performance of digital mammography by improving the overall spatial resolution. The diagnostic value of digital direct magnification survey mammograms was comparable to that of conventional mammograms. Digital 4-fold spot magnification views improved visualisation of the morphologic aspects of microcalcifications.

[Direct radiographic magnification mammography with a new microfocus tube. Experimental studies of resolution and radiation exposure].

A recently developed X-ray unit for mammography using microfocal (spot size 0.12-0.05 mm) direct magnification radiography will soon be introduced into clinical practice. Unit arrangement and tube construction have been demonstrated. Mean surface dose was measured using PMMA-phantoms. A dose reduction of up to 50% was obtained in 1.7-fold magnification mammography (full sized views) compared with conventional techniques. The image quality was almost equivalent. However, in comparison with 1.7-fold magnification mammography, a comparable dose range for 4-fold magnifications with the microfocus system was measured with a substantial gain in spatial resolution. These findings satisfy the demands of a modern mammographic unit.

[Digital storage phosphor mammography in a magnification technic: experimental studies for spatial resolution and for detection of microcalcifications].

PURPOSE: Since the routine use of storage phosphor systems for mammography has been limited by its inadequate spatial resolution of 5 linepairs/mm, a combination of a magnification mammography technique with storage phosphor plates was investigated to detect microcalcifications. MATERIAL AND METHODS: A new mammography system with a microfocus tube using an anode of 0.05-0.12 mm allowed to obtain survey views of the breast with 1.7x magnification (m), and spot views with 4x magnification. The digital image receptor comprised a high resolution storage phosphor plate. To determine spatial resolution, contrast transfer curves were obtained, and the detection of microcalcifications was investigated by ROC (receiver operating characteristic) analysis. RESULTS: Spatial resolution for digital survey views (m = 1.7) was 8 linepairs/mm and for spot views (m = 4) was 18 linepairs/mm. ROC analysis demonstrated a significantly higher performance of the digital magnification technique compared to the conventional screen-film mammography technique. CONCLUSIONS: The limitations of digital mammography with respect to spatial resolution can be overcome by using a high magnification technique.

[DIMA-magnification mammography. Phantom studies on image quality and radiation exposure of the patient].

PURPOSE: A mammographic system has been developed to provide direct radiographic magnification; investigation of image quality and radiation exposure for surveys and aimed exposures were carried out on phantoms and the results compared with two modern screen methods. MATERIAL AND METHOD: Image quality was defined by contrast transfer function, resolution of low contrast, contrast of large areas and recognition of low contrast detail, using suitable phantoms. Measurements of entrance surface dose were made with a 1 cm3 flat chamber. RESULTS: For survey examinations, the new mammographic system provided image quality equal to a screen system but using 50% radiation dose. For localised views, the new system used similar radiation dose but provided significantly better demonstration of diagnostic detail. CONCLUSION: Using a focal spot of 40-120 microns, the new mammographic system provides markedly improved direct radiographic magnification. With reduced radiation exposure at times, there is a significant gain in diagnostic information compared with current standard systems.

[Effect of boundary element discretization on forward calculation and the inverse problem in electroencephalography and magnetoencephalography].

Modelling in magnetoencephalography (MEG) and electroencephalography (EEG) is increasingly based on the boundary element method (BEM). We quantify the influence of boundary element discretization on the neuromagnetic and neuroelectric forward and inverse problem for different dipole depths, brain regions and the quasispherical correction. In particular we derive standards for the general use of BEM models in MEG/EEG source localization. For this purpose simulation with single current dipoles, and source reconstructions from somatosensory evoked potentials and magnetic fields were employed. It was found that both local and global discretization influence source reconstruction. Only at a minimum triangle side length of 10 mm was it possible to achieve stable results for MEG and EEG. In order to obtain acceptable errors within the stable region, the ratio of dipole depth to triangle side length must not be less than 0.5. The results obtained from a comparison of the different brain regions indicate that the similarity to spherical geometry might well have an influence on the estimated dipole location, but not so much on its strength. Source reconstruction employing quasispherical correction was found to be the most stable, in particular in the case of coarse BEM discretization.

[Digital image magnification mammography with the storage-screen technique. Standardized and findings-oriented image processing parameters].

Present day mammography has not been able to make use of the advantages of digital luminescence radiography because of the limited spatial resolution. The recent development of electromagnetic focusing x-ray tube with effective focal spot sizes from 0.04 to 0.12 mm allows radiographic direct magnification with less geometric blur. It is now possible to combine direct magnification mammography with digital luminescence radiography. By combining high quality storage phosphor screens with an HQ-workstation a spatial resolution of 8 lp/mm is possible for 1.7-fold magnification. For 4-fold spot magnification views spatial resolution can be theoretically increased to approx. 20 lp/mm. One important advantage of digital radiography is the possibility of image-post-processing. This article presents two sets of standard parameters and three sets of image dependent parameters for better imaging of specific lesions, such as microcalcifications. The introduction of the the storage phosphor screen technique in mammography is one step in the development of direct digital radiography. Further interesting perspectives for the future include post-processing programs for detecting suspicious lesions which are already being clinical tested.
