Pressure wave with apnoea evaluated by sleep level in patient with ventricular dilation.

Seventeen patients who were suspected of having hydrocephalus, because of ventricular dilation from various causes, were included in this study of the pathophysiologic basis of the appearance of pressure waves (PWs). Pressure waves accompanied by apnoeas originated in arousal responses in the resting state of these patients. Frequent fits of apnoea were included in the entire sleep apnoea syndrome. Most pressure waves characteristically appeared in the state of non-REM sleep. During the appearance of such pressure oscillations, intracranial pressure was elevated transiently. This coincidence in the appearance of pressure oscillations with sleep apnoea was the most characteristic pathophysiologic result from this polygraphic study of the patients.

Effect of diet type on enhanced intestinal protein synthesis by the gut microflora in the chick.

An experiment was conducted to investigate the effect of diet type on enhanced protein synthesis by the gut microflora in the chick intestine. Both germ-free (GF) and conventional (CV) White Leghorn chicks were fed ad libitum from 4 to 14 d of age either a nonpurified diet or a purified diet having 19% crude protein and 12.1 kJ/g metabolizable energy value. At 14 d of age, protein synthesis was measured in duodenum, jejunoileum and ceca after a large dose of L-[4-3H]phenylalanine injected intravenously via a wing vein. The results showed that wet tissue weights for all intestinal sections were significantly greater in CV birds than in GF counterparts with consistently larger differences between the two environments in chicks fed the nonpurified diet than in those fed the purified diet. Protein synthesis in all intestinal sections was significantly enhanced by the presence of gut microflora in terms of both fractional rate (%/d) and absolute rate [mg/(100 g BW.d)]. The effect of diet type on fractional and absolute rates of protein synthesis was most remarkable in ceca where the significant difference between GF and CV states was found only in chicks fed the nonpurified diet but not in those fed the purified diet.

Empty sella in control subjects and patients with hypopituitarism.

The frequency and distribution of various degrees of empty sellae have been examined in subjects without any pituitary disorder and in patients with hypopituitarism. Among them none had sellar enlargement. Sellar computed tomography (CT) with contiguous 2 mm slices (thickness in the axial projection) was performed in 56 control subjects. The CT findings on sella turcica were graded into 4 groups (0, 1+, 2+, and 3+), and grades 2+ and 3+ indicated moderate and marked empty sellae. Thirty-nine percent of the control subjects had empty sellae of grade 2+ or 3+. Sellar CT scans with contiguous 2 mm slices were also performed in 11 patients with hypopituitarism. The sellar volume ranged from 224 to 715 mm3. CT scan was carried out more than 2 years after the onset of hypopituitarism in 10 of 11 patients, and showed typical empty sellae of grade 3+ in all 10 patients. There was no empty sella in a patient with hypopituitarism whose CT scan was carried out 3 months after the massive postpartum hemorrhage. Our results indicate that moderate empty sella of grade 2+ can be seen in subjects without any pituitary disorder, and that a typical empty sella of grade 3+ is present in hypopituitarism with a normal sized sella turcica. An empty sella associated with hypopituitarism may be due to shrinkage of the pituitary gland related to its hypofunction.

[Changes in tissue T1 of brain tumor following GD-DTPA administration--differences in T1 time course and tissue-blood ratio by histological types].

Effect of Gd-DTPA on tissue T1 were studied in 10 patients with intracranial tumors, including 4 meningiomas, 2 gliomas, 3 metastatic tumors and 1 brain abscess. T1 values were comparatively measured in the various portions of tumor tissue, peritumoral edema and normal brain before and after intravenous injection of 0.1 mmol Gd-DTPA per kg weight, with the time ranges between 5 and 60 minutes. In vivo T1 was determined by means of field focusing technique of a FONAR QED 80-alpha system (0.05T). T1 values of blood sample were also seriously measured and applied to estimate the enhancement pattern in the varied tumors. Meningiomas showed the maximal decrease of T1 5 minutes after Gd-DTPA administration and rapid recovery of T1 fairly paralleling to that of whole blood. On the other hand, such tumors as gliomas and metastasis revealed very slowly recovery of T1 followed by the similar T1 decrease just after administration of Gd-DTPA. In the portions of peritumoral edema and normal brain, there were no significant changes in tissue T1 by Gd-DTPA administration. In order to evaluate the clearance pattern of Gd-DTPA in tumor tissue, tissue-blood ratio (TBR) was calculated from the relaxation values at the terms of 5 and 30 minutes following Gd-DTPA administration. The ratios of relaxation rates, TBR 30/TBR 5, were ranged from 1.0 to 1.5 in cases of meningiomas, whereas they distributed in the ranges from 1.5 to 3.0 in the other types of brain tumors. This value of TBR 30/TBR 5 convincingly depends upon the differences in histological types of tumors.(ABSTRACT TRUNCATED AT 250 WORDS)

Influence of the gut microflora on protein synthesis in tissues and in the whole body of chicks.

1. The influence of the gut microflora on protein synthesis in individual tissues and in the whole body of young chicks was investigated by the large-dose injection of [3H]phenylalanine. 2. Growth of germ-free chicks was significantly better than that of conventional controls. Wet weights of liver, spleen, duodenum, jejunum + ileum and caeca were heavier in conventional birds than in germ-free counterparts. 3. Fractional rates of protein synthesis were higher in jejunum + ileum and whole body of conventional birds than in those of germ-free birds. Amounts of protein synthesized were larger in liver, jejunum + ileum and caeca in the presence of the gut microflora. 4. When tissues were classified into gut + liver and the remainder of the carcass, in the presence of the gut microflora an enhanced protein synthesis in fractional and absolute rate was found in the gut + liver, which is in direct contact or in close association with micro-organisms, whereas virtually no effect of the gut micro-organisms was detected in the remainder of the carcass. 5. The contribution of protein synthesis of gut + liver to that of the whole body was larger in conventional chicks than in germ-free birds, whereas the reverse was true for the remainder of the carcass.

Spin-lattice relaxation times of bound water--its determination and implications for tissue discrimination.

Measurements were made of T1 of bound water (T1b) and bound water fraction (alpha) for gelatin solutions and human tissues (sera, brain tumor, cerebral white matter). Bound water fraction in each sample was measured by means of thermal analysis (differential scanning calorimetry: DSC). T1 values were measured by FONAR QED 80-alpha. T1b values were calculated by an equation derived from the fast-exchange two-state model. In the study of gelatin solutions, the relationship between T1 and water content differed depending on the sort of solutions. This was considered to be due to differences in T1b values. In each biological tissue the values of T1b and alpha had different distributions. These results indicate that values of T1b and alpha for biological tissues may be altered in correspondence to the changes in pathophysiological states in those tissues.

Evaluation of diseased coronary arterial branches by polar representations of thallium-201 rotational myocardial imaging.

The perfusion territories in polar representations of stress Tl-201 rotational myocardial imaging in patients with angina pectoris who had one diseased coronary segment were analyzed. The lesions proximal or distal to the first major septal perforator in left anterior descending arteries were detected by the presence or absence of defects at the base of the anterior septum. Right coronary artery lesions were detected by the presence of defects at the basal posterior septum, in contrast to the preservation of myocardial uptake at this portion in lesions of the left circumflex artery. The specific defect patterns were detected in cases with lesions at the first diagonal, obtuse marginal, and posterolateral branches. Recognition of these defects in the polar maps allows detailed detection of diseased coronary arterial branches.

Bronchial arteries: CT demonstration with arteriographic correlation.

Dynamic computed tomography (CT) was performed in nine patients with enlarged bronchial arteries documented by arteriography. Plain and contrast-material-enhanced CT scans of two more patients with prominent bronchial arteries were retrospectively reviewed. The study was conducted to determine visibility of the bronchial artery with CT and to depict the anatomic relationship of its mediastinal portion to surrounding structures. The mediastinal portion of the bronchial artery was successfully outlined as nodular or linear densities on all dynamic CT scans. The right bronchial artery was confirmed to arise from the medial wall of the thoracic aorta, whereas the left arises from the anterior wall. Because of its retroesophageal location, the enlarged right bronchial artery can compress the posterior wall of the esophagus. It is postulated that the left bronchial artery occasionally traverses the aorticopulmonary window, recognized as nodular or linear densities below the aortic arch on CT scans.

Correlative analysis of longitudinal changes in bronchoalveolar lavage, 67gallium scanning, serum angiotensin-converting enzyme activity, chest X-ray, and pulmonary function tests in pulmonary sarcoidosis.

Despite the relatively high cost and complicated procedures, Gallium-67 (67Ga) scanning and bronchoalveolar lavage (BAL) are increasingly advocated as more sensitive indicators of disease activity in sarcoidosis than chest X-ray and serum angiotensin-converting enzyme activity (SACE). To evaluate the clinical usefulness of 67Ga scanning and BAL, we followed 31 patients with pulmonary sarcoidosis, using these four parameters, at 9- to 24-month intervals over periods of 9 to 48 months. We obtained 68 complete evaluations. Close correlations were observed among chest X-ray, 67Ga scanning, SACE, and the percentage of lymphocytes in BAL fluid (p less than 0.1 to 0.001). Longitudinal changes were also well correlated in these four parameters (p less than 0.001) and paralleled the changes in vital capacity (p less than 0.01 to 0.001). However, we were unable to predict the patients' outcome from the initial evaluation of these four parameters. These results suggest that, in terms of their usefulness for estimating disease activity, the differences among these four indicators are negligible. We therefore conclude that chest X-ray and SACE sufficiently reflect disease activity and that, at present, routine evaluation by 67Ga scanning and BAL are not necessarily indicated in the long-term management of pulmonary sarcoidosis.

[Thermal analysis on water components in brain tissue--quantitative determination of free and bound water fractions].

In the living system, tissue water is considered to be composed of both free and bound water. Bound water encompasses the structural water of the cell wall and of various biological substances of high molecular weight, such as proteins and polypeptides. The present study was designed to measure thermoanalytically free and bound water on a quantitative basis in fresh brain of rats using differential scanning calorimetry (DSC). Our intention was to determine the fraction of freezable water in tissue. Freezable water in a tissue represents the fraction of free water. In the present study, freezing was conducted at a constant rate of -10 degrees C/min from room temperature to -75 degrees C by a SSC/560 S (Seiko Electronics). The system allows for calculation of the amount of free water from the differential scanning calorimetry curve employing a coloric constant of 79.4 cal/mg. Aluminium oxide was used as calorimetric reference. The fraction of bound water was calculated by subtraction of the amount of free water from that of total tissue water. Water binding to solid tissue component was estimated from tissue dry weight and the bound water fraction. Mean water content of normal gray matter in adult Wistar rats was 76.9 +/- 1.4% (SD). 88.9% of total tissue water was free whereas 11.1 +/- 2.8% (SD) was bound. Bound water of brain parenchyma amounted to 0.44 +/- 0.12 mg/mg dry weight. As compared to other tissues such as cardiac muscle and liver, brain parenchyma obviously exceeded in free water content. The total water content of serum was 94.4 +/- 1.2%; 90.7 +/- 2.6% was free and 9.3 +/- 2.6% (SD) was bound.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of peritumoral edema by proton T1 values with special remarks on time courses following intracranial surgery].

Proton NMR is said to be sensitive to change in tissue water content. Much more attention has been paid to clinical NMR technique for evaluating pathophysiological states including intracranial edema dynamics. This paper describes the follow up data in NMR imaging and spin-lattice relaxation time (T1) measurement in peritumoral brain tissues. Special attention was directed to the time course of focal T1 values before and after surgery. Twenty-one patients with brain tumor, including 6 meningiomas, 7 gliomas and 8 metastatic tumors, were subjected to the present studies in the term from March 1983 to September 1985. Age distribution of this series ranged from 39 to 74 years (average: 58.2). Fonar QED 80-alpha system was applied for NMR examinations of dual modes (static magnetic field: 433 gauss), i.e. image display by steady state free precession (SSFP) and in vivo T1 measurement by field focussing technique. Standard T1 values for healthy brain were 290 +/- 41 ms and 230 +/- 34 ms (mean +/- SD) in cerebral gray and white matter, respectively. Both T1 values in tumors and in peritumoral tissues before craniotomy were obviously prolonged as compared to standard T1 of corresponding site. However, it remained difficult to discriminate statistically these histological differences by preoperative T1 values. Time course of peritumoral T1 was characteristic in accordance with the sorts of brain tumor. T1 values in the brain parenchyma around meningioma were shortened after operation in the earlier stage up to 1 month, showing relatively rapid normalization in T1.(ABSTRACT TRUNCATED AT 250 WORDS)

Pulseless disease associated with multiple intracranial aneurysms.

A case is presented of a 64-year old woman with pulseless disease associated with multiple intracranial aneurysms which were located in the internal carotid circulation at the left internal carotid-ophthalmic artery and the left internal carotid-posterior communicating junction, and in the basilar-vertebral circulation, at the left basilo-anterior inferior cerebellar artery junction and at the top of the basilar artery. Autopsy verified that her death was due to rupture of the aneurysm at the top of the basilar artery. From the cerebral angiographic and autopsy findings, it can be presumed that overloaded intracranial hemodynamic forces played some role in the development of the aneurysms. The present case is discussed together with our three previously reported cases.

Tortuous vertebral artery simulating extradural spinal tumour.

A rare case of cervical bone erosion resulting from pressure exerted by a congenital tortuous vertebral artery is presented with seven additional cases from the literature. The 3-4 level, especially on the left side, has been the most commonly affected site. The importance of the knowledge of this entity lies in the fact that it causes a surgically treatable radiculopathy, and also that this condition could easily be mistaken for a tumour, leading to accidental bleeding if carelessly approached.
