Renal aneurysmectomy in the ex vivo setting.

The use of ex vivo dissection and reconstruction as definitive surgical management in 3 cases of renal artery aneurysms is presented. The indications, rationale for therapy and review of technique are discussed.

Open ureterocolonic anastomoses.

A simple technique for open ureterocolonic anastomosis is presented. This technique provides adequate exposure for mucosal-to-mucosal approximation and allows the construction of an antireflux ureterocolonic anastomosis.

Sonography in the diagnosis of retroperitoneal fibrosis.

The ultrasonic appearance of retroperitoneal fibrosis is characteristic: a smooth-bordered and relatively echo-free mass anterior to the sacral promontory. Sonography can be used to confirm the diagnosis, follow response to therapy and detect hydronephrotic changes in the kidneys.

The ureter in pediatric renal allotransplantation.

The ureteral complications of 26 renal allografts in 20 transplant pediatric recipients are presented. The catastrophic events following extravasation are documented. The need for early diagnosis as an aggressive treatment is emphasized.

Successful allotransplantation of a kidney removed because of ureteral damage in an unrelated living donor.

The use of a kidney removed because of ureteral damage in an unrelated donor has produced a well functioning renal allograft in the transplant recipient. The kidney demonstrated calicectasis and hydronephrosis on an excretory urogram before removal of the ureteral cutaneous fistula. An excretory urogram performed 8 months after the renal allotransplantation of this kidney shows prompt function of a delicate collecting system without any residual calicectasis. This case emphasizes the need for us to explore the possible use of free kidneys removed from living unrelated donors for non-neoplastic diseases of the ureter.

The treatment of bilateral synchronous renal pelvic tumors with Bench surgery.

Bilateral nephrectomy, renal perfusion and bench surgery have been used in the management of synchronous bilateral renal pelvic tumors in a 35-year-old man. The possibility of bilateral nephrectomy followed by hemodialysis and eventual transplantation was considered but was not done because we believe for the present time that the nature of the immune response and its importance in the pathogenesis of cancer are complicated and poorly defined. It is likely that immunosuppression may tip the balance toward the tumor. Bench surgery allowed us to perform radical resection of these urothelial tumors yet preserve enough renal tissue to sustain life. This patient is now enjoying an active life and has a blood urea nitrogen of 30 mg. per cent and a serum creatinine of 1.2 mg. per cent from the functioning third of the former right kidney.
