Antibiotics and topical surgical preparation solution in septal surgery.

One thousand forty patients undergoing septal surgery of the nose were included in this 5-year study of no prophylactic antibiotics and no topical surgical preparation solution before surgery. Minor nasal infections developed in only five patients (0.48%) postoperatively. All five of these patients responded to oral antibiotic therapy and did not require hospitalization or intravenous antibiotic treatment. The incidence of infectious complications resulting from nasal surgery without the use of topical surgical preparation solution and without prophylactic antibiotics is minimal. No topical surgical preparation solution and no prophylactic antibiotic technique is a safe and acceptable approach for septoplasty and rhinoplasty surgery.

Aanerobic isolates in hemovac lines.

Seventeen of 68 patients (25 percent) who required the use of hemovac drainage system after major head and neck procedures had positive anaerobic islates. Co-isolation of aerobic (94 percent) and other anaerobic (29 percent) microorganisms are common. There was an increased rate of psoitive anaerobic islates with procedures requiring exposure of mucosa. Radiotherapy, except in doses greater than 6,800 rads, did not affect the culture rate. Eight of 17 patients (47 percent) with positive anaerobic isolates had infectious-related complications while five of 51 patients (10 percent) with negative cultures had similar complications. Positive anaerobic isolates in hemovac lines can be correlated with pathogens isolated from subsequent complications.

Infectious and noninfectious complications in major head and neck surgery.

The Hemovac drainage system was required postoperatively for 156 patients who had undergone major head and neck surgical treatment. This study distinguishes infectious complications from noninfectious complications by the aerobic and anaerobic cultures that were taken from the Hemovac line drainage and any subsequent complications. The 33% complication rate included only those of the head and neck and excluded general medical complications. Of the 52 patients having complications, 19 had infectious, 24 had noninfectious, and nine had both infectious and noninfectious complications.
