Perinatal outcome in fetuses with loss of end-diastolic blood flow velocities in the descending aorta and/or umbilical arteries.

Continuous- and pulsed-wave Doppler velocimetry was used as an additional method of antenatal surveillance in high-risk pregnancy care. During a 30-month period with 4,169 deliveries 226 patients (5.4%) with complicated pregnancies (intrauterine growth retardation, diabetes, hypertension) were investigated. The results had only marginal influence on obstetrical management. In 38 fetuses absence of end-diastolic blood flow velocities (AEDF) was detected in the descending fetal aorta and/or umbilical arteries. Mean birth weight of these fetuses was 1,441 +/- (SD) 690 g at a mean gestational age of 31.8 +/- 6.8 weeks at birth. Twenty fetuses (53%) were growth-retarded (less than 3rd centile). Six fetuses died in utero and 7 during the perinatal period (34% mortality). There was a significant deterioration of perinatal outcome with AEDF in the umbilical arteries at the last antenatal Doppler scan. With AEDF in the descending fetal aorta the Doppler parameter Vmean and with AEDF in the umbilical arteries the pulsatility index yielded additional prognostic information. Doppler velocimetry of the fetal and umbilical vascular system could be of additional value for antenatal surveillance in high-risk pregnancies.

[Use of the dura in the surgical treatment of bladder cancer].

It is reported on 9 patients in whom a dura graft was performed on account of a carcinoma of the urinary bladder. Apart from one patient who before underwent radiotherapy the dura healed. As to complications three times formations of calculi and twice rejection of larger parts of the dura were found. Indications to dura graft as well as its advantages and disadvantages are discussed and it is referred to immunological problems which are still to be clarified.

[Use of infrared technic in veterinary medicine. (Review)].

More effectiveness of veterinary examination of the living animal is an important demand of socialist practice which applies equally to conditions of stepwise transition to industrialised production processes and to conventional production. The physiological and physico-technical prerequisites for possible use of infrared techniques in veterinary attention and production monitoring are discussed on the basis of available literature, with particular reference being made also to human medicine.

[The maternal and perinatal mortality and morbitity in attendance to Caesarean section in the period from 1956--1976 at the UFK-Jena (author's transl)].

In the period from 1956--76 1 468 caesarean sections were accomplished at the UFK-Jena. 44 043 children were delivery during the 21-year period. The number of caesarean sections rose from 1,08% to 1956 to 10,8% in 1976. The fetal indication in 1956 was zero, on the other side it amounts to 34,3% in 1976. First place of the maternal indication in the 21-year period took in the cephalo pelvine disproportion. In the same time the maternal mortality in attendance of caesarean section was 12,3%. Considering the period between 1969--76, the maternal mortality fell to 2%. Maternal morbidity diminished from 1965 and obtained 23% in the period between 1974/1976. The feverish childbed took in the first place of the maternal morbidity (30%). The perinatal mortality fall from 28,57% in the period between 1956/1958 to 3,71% in the period between 1974/1976. The perinatal morbidity we could'nt state certainly. In 8,3% we found perinatal morbidity by acidosis. 5,5% of children, delivered by caesarean section needed medical care. 16,4% of all children were either small for gestational age or immature. The problem of caesarean section as an important method for termination of labor is discussed.

[The relationship between haptoglobin type and serum agglutination titers against group G streptococci with the T4-antigen in pregnant women and newborn infants].

The relationship between haptoglobin type and titer of agglutination against streptococci carrying the T4-antigen was examined in 100 pairs of sera (mother/child). In healthy pregnant women high titers were predominant in individuals of haptoglobin type 2--2. Lower titers were found in sera of women with diseases during pregnancy. In 4% of the newborns only the haptoglobin type could be recognized. In comparison of maternal and neonate titers of agglutination pointed out the antibody-like activity of haptoglobins.

[Value of the electrophoresis mobility test in the differential diagnosis of obscure kidney findings].

The differential diagnosis of unclear carcinoma-suspicious renal findings often finishes with the test-exposition and nephrectomy. Also when all modern diagnostic possibilities are used not always a clear establishment of the diagnosis may be achieved. The cell electrophoresis mobility test after Field and Caspary may contribute to the differential-diagnostic clarification of the findings, when adequate antigens are used. For this purpose 10 own cases are discussed and these results are compared with suitable comparative groups. Summarizingly by the present study can be shown that the immunologic diagnostics of tumours in the field of unclear renal tumours may be a very good help in the diagnostics. When adequate antigens are used in the cell electrophoresis mobility test a coordination concerning the histospecificity can be achieved.

[A contribution to bladder amyloidosis].

It is reported on a localised primary amyolidosis of the urinary bladder with a progressing course. In contrast to the prevailing observations of other authors during two years an increasing bilateral urinary stasis developed so that finally after an at first unilateral nephrostomy due to an infected retention kidney the bilateral ureter-intestine-anastomosis became necessary. Apart from a short description of the amyloidosis from the general and especially urologi-al point of view it is particularly referred to diagnostic difficulties, since the various forms of amyloidosis have a different prognosis and demand a therapy.

[DNFB reaction in bladder cancer, an immunological study].

It is stated that the determination of the reactivity against the primary antigen dinitrofluorbenzol by means of the epicutaneous skin test before the beginning of a therapy may give an essential reference to the spreading of the tumour and to the prognosis. The course control in patients with carcinoma of the urinary bladder should therefore consist not only of clinical but also of immunological examinations.

[Organ preservation in intra- and extracorporeal kidney surgery].

With the help of cases of own patients is reported on the possibility of organ preservation in intra- and extracorporal renal surgery. In 5 patients with tumours, 3 patients with coral calculi and one patient with an extended disease of the ureter five times the extracorporal renal surgery with following autotransplantation and initial pulsatile fall stream perfusion was performed. Four times the in-situ-perfusion with hypothermic Collins solution and in-situ-operation was performed. Indication and advantages for the different methods are demarcated.

[Ultrasonographic differential diagnosis of imminent abortion (author's transl)].

Two-hundred patients, hospitalised for imminent abortion in the sixth to 16th weeks of pregnancy, underwent 294 ultrasonic compound B scans. Gravidity was found to be intact in 68 per cent of all cases, but 13.6 per cent of these ended in abortion prior to the 28th week of pregnancy. Four hydatiform moles and 17 missed abortions were reliably diagnosed. Ultrasonography, even without grey-scale equipment, may be of greatest usefulness for differential diagnosis between intact early pregnancy, missed abortion, blighted ovum, and hydatiform mole, when used by an experienced specialist in close cooperation with the clinical practitioner.

A contribution to immunological tumour diagnostics in urology.

Fifty-four patients with malignant and other diseases of the urinary bladder and the prostate were examined for lymphocyte sensitization by means of electrophoretic mobility test (EM test). In this antigens from brain tissue (EF) and from malignomas of kidney, urinary bladder and prostate were used. Positive HEP values were obtained in 40 patients out of 42 with malignant tumours and in 5 patients out of 12 with other kidney diseases. With the corresponding tumour-associated antigens (TAA) in the EM-test, the histological tumour diagnosis could be confirmed in every case. Positive findings after using EF as well as the corresponding TAA are with great probability an indication of a malignant disease.

[Lymphocyte transformation test with streptococcal antigens in newborn infants and their mothers].

Following streptococcal mitogens and antigens were tested in the lymphocyte transformation test with human cord blood and maternal blood: streptococcal M1- and M19-proteins, mucopeptide, erythrogenic toxins and additional phytohemagglutinin. There are no significant differences in the blastogenic response with the different mitogens and antigens in maternal and cord blood. The cord blood lymphocytes show the same degree of blastogenic reaction to the mitogens and antigens as the maternal lymphocytes, that may reflect the state of readiness for cellular immunological defence. The spontaneous incorporation of 3H-labelled thymidine into cord blood lymphoblasts is significant higher than in lymphoblasts of maternal blood.--The results of specific and unspecific stimulation by means of steptococcal antigens are discussed.

[Perfusion technic during clinical intra- and extracorporeal tumor removal from a solitary kidney].

On the basis of own experiences in operations of tumour-carrying kidneys two perfusion techniques for the intra- or extracorporal renal surgery are exolained and weighed one to another. In the in-situ-perfusion the pulsatile mechanical perfusion with supervisable flow, perfusate pressure and under direct measuring of temperature on organs has proved. The disadvantage of the in-situ-perfusion in contrast to the extra-corporal surgery consists in the impossibility of the local after irradiation.

[The clinical picture and pathology of benign tumors of the female urethra].

Benign neoplasia of urethra are of frequent by aged women. Epithelial tumours (polyps, papilloms) are distinguished from mesenchymal tumours (fibroms, myoms, angioms) histologically. Dislocations of the mucosa and real malignoms of the urethra are marked off from these. The authors report about the rarity of mesenchymal neoplasia and represent a case by which it comes to retention of urine because of a big leiomyoma. Diagnosis and therapy are described in detail.

[Course of cellular immunity in urologic neoplasms].

Apart from a purely qualitative evidence concerning the presence of a specific sensibilisation it is possible to estimate also its strength or the blocking factors appearing in the serum of tumour carriers by means of the electrophoresis mobility test. By this it seems to be possible to make an evidence concerning the immunological reaction position of the tumour patient with the possibility to make conclusions as to the prognosis of the patient.
